The fibrosis involved in gluteus maximus causing limitation of flexion and adduction of the hip has become a recognized clinical entity since the first report by Fernandez de Valderrma in 1969. Its most constant and characteristic histologic feature was substitution of the fibrous tissue in the definitive etiology was unknown but presumed to be multiple intramuscular injections. Authors present three cases of the fibrosis involving gluteus maximus. In two cases Z-lengthening was performed on the thickened fibrous bands with good results.

5.Nerve Conduction Studies after Surgical Release of Carpal Tunnel Syndrome

Nerve Conduction studies are used not only in diagnosis of compression neuropathy, but also as a prognostic evaluation after is treatment. Authors analyzed the changes of nerve conduction studies in the 21 hands of 19 patients of carpal tunnel syndrome which was confirmed by clinical and electrodiagnostic methods, and treated by surgical release of transverse carpal ligament. The changes of nerve conduction and clinical studies were completed just before surgery, and at 2-week, 1-month, 3-month and 6-month of postoperative periods, and summarized as follows; 1. The distal sensory and motor latencies were significantly(P < 0.05) improved at the 3-month of postoperative periods, respectively. 2. The sensory conduction velocities across the carpal tunnel was significantly(P < 0.05) improved at the 6-month of postoperative periods. 3. The distal motor amplitude was significantly(P < 0.05) improved at the 6-month of postoperative periods. 4. The changes in sensory conduction studies were more sensitive value for evaluation of patients with carpal tunnel syndrome. 5. The distal latencies reflected the progress of clinical improvement, accurately. These conduction studies are considerable value in the evaluation of the post-carpal tunnel release patient.

6.Alteration of Phospholipase C Activity in Human Gastric Cancer Tissues.

Phospholipase C (PLC) plays a pivotal role in transmembrane signal transduction pathway for cellular proliferation differentiation and growth. Thus far, there have been few reports in which PLC activity was investigated in human malignant neoplastic tissues. In the present study, we evaluated PLC activity in 23 human gastric cancer tissues and normal mucosal tissues to investigate whether alteration of PLC activity is associated with gastric cancer. The amount of [14C] diacylglycerol, one of the earliest products of inositol phospholipid hydrolysis by PLC, was measured by thin layer chromatography. Also, expression of PLC-gamma1, which is one of the most important PLC isozymes,was examined by immunohistochemistry using specific monoclonal antibody directed against PLC-gamma1. The results are summarized as follows. PLC activity in all 23 gastric cancer tissues (1.35+/-1.04 units/mg of protein) was significantly higher than normal mucosal tissues (0.28+/-0.21 units/mg of protein) (P<0.001). PLC activity in gastric cancer tissues was not correlated with histologic grade (P>0.05). PLC-gamma1 immunoreactivity was detected in all of 23 cases studied. The intensity and extent of PLC-gamma1 immunoreactivity was not correlated with PLC enzyme activity, although stronger intensity was demonstrated in malignant cells in comparison to normal gland epithelial cells. The present study provides the first evidence of significant elevation of PLC activity in human stomach cancer tissues. Our results strongly suggest that PLC might be involved in tumorigenesis and/or progression(uncontrolled continuous cycling of cells) of human gastric cancer. Further studies are needed to elucidate the role of elevated PLC activity in cancer tissues.

8.Mortality Rates and Risk Factors in Community Based Dementia Patients.

OBJECTIVES: The aim of this study was to investigate mortality rates and risk factors in dementia patients in a rural cohort. METHODS: A total of 114 subjects with clinically diagnosed dementia were followed up for eight years from 1997 to 2005. Their mortality was compared with sociodemographic and clinical variables using the Cox proportional hazards models after adjusting age, sex, and education. RESULTS: During follow-up, the mortality rate of subjects was 80.2% and the mean (SD) duration of survival from at diagnosis to death was 4 years. Mortality in subjects with dementia depended on old age (relative risk [RR] : 1.05 ; 95% confidence interval [CI] : 1.01-1.08), male (RR : 1.61 ; CI : 1.00-2.59), low Clinical Dementia Rating scale (RR : 1.54 ; CI : 1.14-2.10), low Activities of Daily Living (RR : 0.72 ; CI : 0.59-0.89), low Instrumental Activities of Daily Living (RR : 0.83 ; CI : 0.75-0.92), no physical activity (RR : 0.44 ; CI : 0.28-0.70), smoking (RR : 1.74 ; CI : 1.05-2.89). CONCLUSION: Mortality in dementia depended on age, sex, CDR, ADL, IADL, physical activity, smoking. These findings have important implications that contribute to make the disease management of dementia patients.

10.Epidemiology of Klebsiella Isolates by Antimicrobial Resistance Gene Analysis.

One hundred of clinical isolates of Klebsiella spp. from three hospitals were analyzed by phenotypic and genotypic characteristics for epidemiologic investigation. Almost all isolates of Klebsiella spp. showed highly resistance to ampicillin, and carbenicillin and 4.5-7.9% of K. pneumoniae isolates were also resistant to cefotaxime and ceftazidime, and 10-15% to aminoglycoside antibiotics except amikacin. However, all strains were highly susceptible to imipenem, cefotetan, amikacin and ciprofloxacin. All Koxytoca strains were susceptible to antimicrobials tested except Ap, Am and Cb. Twelve strains of K. pneumoniae hybridized with TEM or SHV probe and extended spectrum B-lactamases from 7 strains were TEM type. Eleven conjugative R plasmids and their parental strains were analyzed. Among them, three couples of plasmids showed identical or nearly identical resistance phenotypes of B-lactams and aminoglycosides, molecular weights, and pI values by isoelectric focusing, and hybridized fragment patterns with TEM probe by Southern hybridization, EcoR1 restriction endonuclease fragment patterns. Their parental strains were isolated from sputum, tissue, and ascites of patients and had similar characteristics. These results indicate that the epidemic strains or epidemic plasmids were present in this hospital and antimicrobial resistance anlysis can be used to discriminate clinical isolates of multi-resistant K. pneumoniae.
