2.A Case of Impetigo Herpetiformis.

We report herein a case of impetigo hepetiformis treated with etretinate. The patient was 30 year old female in the 7th month of her second pregnancy, who had suffered from generalized coalesced pustular eruption with slightly itching and hurning sensation with fever on whole body except face, both had and foot. The euption began as an erythemations patch which developed pustules along the mar gin. I'here was extensian by the develupment of fresh peripheral pustules and coaleence of these patches. Histopathological findings showed hyperkeratosis, subcorneal spongiform pustule, spon,iosis in the epidermis and vascular proliferation in the devmis, She was treated successfully with etretinate for 4 weeks.

5.A Clinical on Herpes Zoster in Inpatients During a 3 - Year - Period ( 1991 - 1993 ).

BACKGROUND: Data on the frequency of Herpes Zoster are sparse, especially in inpatients OBJECTIVE: The purpose of this study was to elucidate the epidemilogy and the clinical characteristics of herpes zoster in inpatients. METHODS: During a 3-year-Period from Jan. 1991 to Dec. 1991, 157patients with herpes zoster were evaluated in regard to annual, monthly and seasonal incidence age incidenee and sex ratio. Patients with herpes zoster were further assessed concerning associratel conditions, dermatomic distributions and complications. RESULTS: 1. The annual rate ranged from 2.6% to 3.3% (mean 2.8%) and the ratio of male to female was 0.85: 1. 2. There were no marked manthly of spasonal differences in herpsoster, ranging from 4.5% to 10.8% and 21.7% to 31.2%, respectively. 3. Herpes Zoster was seen most frequently in the 50-59 year age group(27.4%) 4. The dermatomic involvernent of herpes zoster was most freqer t of thoracic dermatome(51.6%) and others were trigeminal(19.1%), cervical(13.4%), lumbar(6.4%), sacral(5.7%), facial(1.9%) and generalized(1.9%). 5. The frequency of associated conditions in herpes zoster was in d scending order, hypertension, tuberculosis, diabetes mellitus, malignancy, SLE, gastric ulcer, chronic renal failiure, rheumatoid arthritis, idiopathic pulmonary fibrosis. 6. The most common complication of herpes zoster was postheret c neuralgia and others were ophathalmologic complications, secondary bacterial infection, keloicl, formation, generalized varicelliform eruption, recurrent herpes zoster. CONCLUSION: Herpes Zoster is a major problem in the Department of Dermatology, especially in inpatients.

8.A Case of Dysplastic Melanocytic Nevus.

The dysplasitc melanocytic nevus(DMN) of the skin is an atypical-appearing melanocytic tumor characterized by intraepidermal atypical melanocytic preliferation. Clinically, DMN has a diameter greater than 5mm, ill-defined irregular borders, irregularly distributed pigmentation surrounded by erythems,. Histopat- hologically, there are numerous discrete nests of atypical melanocytes situated at the dermoepidermal junction of elongated rete ridges. In the dermis, concentric lamellar fibroplasia, lymphocytic infiltrates and neovascularization are noted. We report herein a case of DMN developed on the trunk of 31-year-old Korean male.

9.A Clinical Observation on Systemic Lupus Erythematosus.

BACKGROUND: Systemic lupus erythematosus(SLE) is a clinical syndrome of unknown causes characterized by inflammat.ion and multisystem involvement. Since Hargravas describe the LE cell in 1948, a profound change has occured in the concept of SLE. Many studies of the large series have been reported in the West, but few collective studies have been reported in Korea. OBJECTIVE: The goal of this study was to elucidate the clinical and laboratory characteristics of SLE in Korean people. METHODS: We carried out retrospective study of 64 patients, who were diagnosed as SLE on the base of the 1982 revised bacteria for the SLE by American Collepe of Rheumatology(formerly, American Rheumatism Association), at Korea University Hospital from 1982 to 1991. The results were compared with those of previous studies in Korea and western countries. RESULTS: 1. In 64 patients with SLE, female patients outnumbered male by 6.1 to 1 and the mean age of onset was 28.6 years. 2. The initial manifestations were arthritis or arthralgia, 25%; fatigue, malaise, weight, loss, 21.8%; facial rash or blush, 12.5%, fever, 12.5%. 3. The major clinical manifastations were hematologic(90.6 %), muccutaneous(89%), musculoskeleta (79.7%), and renal(73.4%) involvemant. 4. The ANA test were positive in 83.9% of the patients, and homoenous pattern was most common. There was no significant difference of organ involvement according to the nuclear patterns of ANA, but cardiopulmonary, renal involvement, hypocomplement,emia and positive anti-ds DNA were less frequent in ANA neghative group. 5. In 73.7% of the patients, the titers of anti-ds DNA antibody was elevated. The amount of anti-ds DNA Ab in serum did not always correlate with disease activity. 6. Photosensitivity, arthritis or arthralgia and CNS manifestatioris were less common, but hematologic findings, including anenia and leukopenia were more common in Korean than western reports. CONCLUSION: Patients with SLE are heterogenous in the natural h story of their disease, which is not an uncommon disease in Korea. Continued efforts to clarify the immunopathologic basis for SLE should eventually lead to more effective treatment.

10.A Case of Early Age Onset Hailey-Hailey Disease Treated with Surgical Operation.

Hailey-Hailey disease is a rare hereditary dermatosis that begins in the 2nd or 3rd decade of life. The skin lesion is characterized by a localized, recurrent eruption of small vesicles on an erythematous base. It courses remissions and exacerbations. It seldom begins in early childhood, and main treatment modalities are conservative ones. We report a case of Hailey-Hailey disease that began on a 7-month old infant and improved by surgical treatment. In according to review of the previous reports, it is probably the earliest onset age and it is may be the first case which was treated with surgery in Korea.
