1.Familial Congenital Muscular Torticollis: A Case Report

Hereditary theory was first proposed by Joachimsthal in 1905. However, a family history of congenital muscular torticollis is uncommon. This is a report of two female children who are interrelated and have congenital muscular torticollis. They are sisters. No environmental factor could be identified that would predispose the the children to congenital muscular torticollis. In this family, it is apparent that genetic factors are influencing the development of this condition. The cause of congenital muscular torticollis is unknown, but heredity may play a more important role than has previously been supected.

2.Synovial Sarcoma of the Ulna: A Case Report

Synovial sarcoma is an uncommon, malignant tumor which originates from the synovial lining of the joints, bursae and tendon sheaths. But it rarely occurs from bones. We have experienced a synovial sarcoma of the left ulna. Microscopically cleft spaces lined by epithelioid cells and bundle formation by spindle spaped cells were seen. Above elbow amputation of the left arm was done.

3.A Clinical Study on Baseball Pitcher's Elbow

History taking, physical examination and x-ray study of both elbows were made on 55 senior high school, college and vocational baseball pitchers who visited the Department of Orthopedic Surgery of Hanyang University Hospital from March 1, 1980 to April 30, 1980. The results of this study were as follows: 1. Average age of 13 high school pitchers was 17.2 years, of 20 college pitchers was 20.1 years, and of 22 vocational pitchers was 23.4 years and average duration of pitching of high school pitchers was 3.8 years, college pitchers was 7.4 years, and vocational pitchers was 9.8 years. 2. Overhand type pitchers were 39(70.9%) and it was the most common pitching style. 3. 45(81.8%) complained elbow pain and the order of frequency was medial, lateral, and posterior. 4. 45(81.8%) showed hpertrophy of forearm and 38(69.1%) showed hypertrophy of arm. 5. Valgus deformity of the elbow joint was observed In 34(61.8%) and flexion contracture of the elbow joint was in 41 (74.5%). 6. Changes on x-ray were ossicle, traction spur, humeroulnar hypertrophy, narrowing of joint space, calciflcation of soft tissue and others. 7. Changes on x-ray were in proportion to duration of pitching and especially combined Iesions were observed more often ln overhand type pitchers.

6.Preliminary Results of Uncemented Metal on Metal Total Hip Replacement Arthroplasty.

Particulate wear debris(polyethylene, polymethylmethacrylate and metal particles, or combination of these) can initiate aseptic loosening. To eliminate polyethylene debris and reduce the total amount of wear, metal on metal bearings(Metasul) which were made of a wrought Co-28Cr-6Mo-0.2C alloy, were introduced. From May 1994 total hip replacements using a Metasul metal on metal articulation were performed by the authors. 34 patients with 40 hips with minimum follow-ups during 24 months were analysed in this study. The mean follow up period was 31 months(ranging from 24 to 47 months). We used Armor cups and standard cups as acetabular comporients and SL stems, CLS stems and cone stems as femoral components. Preoperative average Harris hip score of 48 points became postoperatively 93(from minimum 81 to maximum 100). And there were slight and occasional thigh pains in 2 cases(5%). There were no revisions for the early aseptic loosening of femoral stems or stem failures. Radiologic evaluation which revealed mild calcar atrophy was developed in 33 cases(82.5%), revealing cortical hypertrophy in 10 cases(25%). Osteolysis was not noted around the prosthetic stem nor around the cup. Radiolucent lines were found in 9 cases of femoral components(22.5%) and in 1 case of acetabular component(2.5%) and subsidence of more than 3mm in 1 case of femoral components(2.5%) were also found. There was 1 case of definitive loosening of acetabular component(2.5%). These also showed that endosteal bone formation adjacent to femoral prosthesis was developed in 22 cases(55%) but no distal pedestal and stem shift occurred. The short-term but satisfactory results of the contemporary metal on metal articulation investigated in this study were encouraging and warrant continued study.

10.Comparisons of the Oxygen Saturation Between Right Atrial and Pulmonary Arterial Blood Immediately After Emergence from Cardiopulmonary Bypass During Open Heart Surgery in Pediatric Patients.

BACKGROUND: Mixed venous oxygen saturation (SO2) monitoring turned out to be invaluable in following up systemic cardiovascular status. But balloon-tipped flow-directed thermodilution catheter (Swan-Ganz catheter) insertion carries additional inherent risks other than those of central venous catheterization, and it costs much more expense than simple venous catheter. There has been a lot of papers which argue that central venous catheterization may substitute for the Swan-Ganz catheterization. In addition, it may be very difficult or impossible to insert the Swan-Ganz catheter in pediatric patients. This study was performed to determine whether Swan-Ganz catheterization might be replaced by the central venous catheterization in regards to SO2. METHODS: In 17 pediatric open heart surgery (OHS) patients, from the central venous catheter, the tip of which had been located in the center of right atrium (RA) and pulmonary artery, blood samples were drawn for gas analysis simultaneously, which was done immediately after emergence from cardiopulmonary bypass. RESULTS: There were no significant differences between RA blood and mixed venous blood gas analyses except oxygen saturation. The oxygen saturations of both were linearly correlated with each other. The relation was SO2=17 0.8 SRAO2 (R=0.77, p<0.05) CONCLUSIONS: It may be concluded that RA blood may be used for blood gas analysis in place of mixed venous blood immediately after pediatric open heart surgery.
