2.Two Cases of Eosinophilic Pustular Folliculitis.

We report 2 cases of eosinophilic pustular folliculitis, one in s 29-year-old woman and the other in a 24-year-old man. Crops of papules and pustules developed on well demarcated, walnut-sized, erythematous plsques on the cheeks and forehead. Eruptions were more pronounced on the periphery of the plaques with central clearing. Blood eosinophilia was evident in case 1. Histologic findings showed a dense inflammatory infiltration and abscesses composed mainly of eosinophils in and around hair follicles, sebaceous glands and around blood vessels. We could see beneficial responses by administration of dapsone in both cases, but its effect was just suppressive and temporary.

3.A Decreased Growth Rate of the Great Toe Nail Observed in Patients with Distal Subungual Onychomycosis.

BACKGROUND: There are prerequisites for fungal nail infection: fungal organism, susceptible host, and environments with damaged nail. OBJECTIVE: The purpose of this study was to evaluate the growth rate of the great toe nail in distal subungual onychomycosis(DSO). MATERIALS AND METHODS:: The subjects are 60 adult patients with DSO and 19 age- and sex-matched controls with tinea pedis but no DSO. The growth rate of the great toe nail was measured. Data were evaluated with Mann-Whitney U-test and regression analysis using the statistical program for PC. RESULTS: The growth rate of the patients group showed a decrease against that of the controls. The equation for expected nail growth rate of specific age and sex was as follows: the expected nail growth rate(mm/day) = 0.07382 + (0.01498 × G) - (0.00033 × Age) - (0.00669 × S): if a person has DSO, G is 1, and if he has no DSO, G is 2; if a person is male, S is 1, and, female, 2. CONCLUSION: Patients with DSO often have a decreased growth rate of the nail compared with that of the controls. The authors propose the equation of expected nail growth rate in persons with or without DSO, and think that the growth rate of the nail is one of factors in the pathogenesis of onychomycosis.

Castleman's disease(Giant lymph node hyperplasia) is a pathologic entity of unknown etilogy, as attested to bythe number of names it has received, lymphoid hamartoma, angiomatous hamartoma, and giant lymph node, etc.Although the mediastinum is its most common location, it also occures in other areas of the body, usually wherelymph nodes are normally found. Authors have been experienced 2 cases of histologically porven Castleman's diseaseduring recent 3 years in Kyung Hee University Hospital, and present its radiological and pathological findings asmediastinal mass.

5.Folinic Acid is an Active Chemosensitizer of 5-Fluorouracil in vivo against Human Transitional Cell Carcinoma.

While augmentation of 5-fluorouracil (5FU) cytotoxicity by folinic acid (FA) has clearly been demonstrated in vitro, its value in human is controversial. Unfortunately, essentially all tumors studied in vivo have been murine and none were transitional cell carcinoma (TCC); no in vivo studies of FA+5FU against TCC xenografts have been performed. The present study was initiated to test the effects of FA on 5FU induced cytotoxicity in two human bladder cancer xenografts, DU4184 and DU4284, and to further test the sequence dependence of FA+5FU synergism. To perform the in vivo chemosensitivity assay, we used modified nude mouse subrenal capsule assay. Before treatment, the mice were randomly allocated into four groups. Group A was given normal saline only as a control, group B was given 5FU (100 mg/kg i.p.) only, and group C was given FA (100 mg/kg i.p.) and 5FU simultaneously and group D was given FA and one hour later 5FU. Before treatment and after treatment tumor volumes were measured and tumor growth ratio (TGR) of each group was calculated. TGRs of group A, B, C, and D were 6.1+/-0.6, 3.8+/-0.3, 3.8+/-0.4, and 3.0+/-0.2, respectively.No difference in cytotoxicity was seen if 5FU was given simultaneously with FA (group C). However, pretreatment with FA one hour prior to 5FU resulted in statistically significant potentiation of 5FU efficacy (group D) (p<0.05). Using this preferred dose schedule, DU4184 vs. DU4284 (+/- FA) was evaluated. Comparative sensitivity of the two lines revealed that DU4284 is relatively resistant to 5FU. FA significantly enhanced the cytotoxicity of 5FU on both DU4184 and DU4284 xenografts (p<0.05, and p<0.01, respectively) and had a substantially greater impact in the intrinsically more 5FU-resistant tumor (DU4284). It is concluded that FA is an effective chemosensitizer of 5FU in vivo against human TCC. Nevertheless, dose schedule is critical; pre-loading of the tumor with FA prior to 5FU is essential. However, as yet undefined intrinsic biochemical differences between tumors [such as thymidine salvage pathways (not generally assessable by standard in vitro assays)] may modulate the extent of such efficacy. Further preclinical study with an expanded TCC xenograft battery is necessary and should be linked with a study of biochemical correlates to identify phenotypic differences in FA chemosensitivity of those benefiting from this regimen.

6.Mucinous Cystadenoma of Renal Collecting Duct Origin: A Case Report.

We report a case of mucinous epithelial tumor (mucinous cystadenoma) in the renal pelvis. This type of tumor shows a non-specific cystic mass, and sometimes has wall calcification, it very rarely originates in multi-potential renal pelvic epithelium. We report the imaging findings of the tumor and review the previous literature.

7.A Case of Herpes Zoster in a Normal 3 - Month - Old Infant.

Herpes zoster in infancy occurs very rarely. And it may be the primary clinical manifestation of reactivation of latent varicella-zoster virus infection acquired transplacentally during intrauterine life. We report a case of herpes zoster in right T2 dermatome in a normal 3-month-old boy, The patient had no history of chickenpox and varicella vaccination. And his mother did not recall herself varicella infection, though she was exposured to chickenpox patient at 7 months of pregnancy. Laboratory findings demonstrated that varicella-zoster Ig-G and Ig-M antibodies were 0.94, 0.12 respectively ( EIA method, negative, <0.8; positive, >1.0) at the second day of vesicular eruption. Fourteen days after vesicular developing, the Ig-G and Ig M titers were rising to 1.92, and 0.47 respectively. His mother's varicella-zoster Ig-G and Ig-M antihodies were 2.64, 0.14 respectively. The baby received acyclovir treatment and the skin lesions healed rapidly without sequela. In this case, although the cause was unclear whether it was a mother-child infection, the patient was suspected subclinical intrauterine varicella-zoster virus infection during pregnancy.

8.CT of mediastinal hemangioma: case report.

We decribe the CT findings in a case of mediastinal hemangioma. Precontrast enhanced CT demonstrated a homogeneous soft tissue mass with weveral nodular calcifications indicating phleboliths. Contrast enhanced CT revealed some areas of enhancement similar to that of adjacent vascular structures. Hemangiomas of the mediastinum, although rare, should be included in the differential diagnosis of enhancing mediastinal masses.

10.Comparison of Combined Spinal Epidural Anesthesia and Epidural Anesthesia for Cesarean Section.

BACKGROUND: Epidural anesthesia (EA) has become most popular for cesarean section, but has some drawbacks such as incomplete block, inadequate muscle relaxation and delayed onset. Combined spinal epidural anesthesia (CSEA) has gained an increasing interest as it combines a reliability of the spinal block and the flexibility of an epidural block. We investigated the efficacy of CSEA which combines main spinal and supporting epidural anesthesia, comparing with pH adjusted EA, for cesarean section. METHODS: Sixty four pregnant women at full term were divided into two groups. Group 1 (n=32) received CSEA with 1.5~1.6 ml of 0.5% hyperbaric bupivacaine intrathecally, followed 10 minutes after by 10 ml of 0.25% plain bupivacaine through the epidural catheter. Group 2 (n=32) received EA with 20~25 ml of 2% lidocaine which was added 0.1 ml of 0.1% epinephrine, 100 microgram of fentanyl and 1.5 ml of 8.4% sodium bicarbonate. The quality and side effects of surgical anesthesia, neonatal state and postoperative course were compared between two groups. RESULTS: While 22% (7 cases) of Group 2 complained of intraoperative pain but none of Group 1 did (p=0.011). Muscle relaxation and motor block were much better in group 1 (p<0.001 and p=0.011 each). Significantly more women in group 2 had shivering (p=0.001) and they also had nausea and vomiting more, though that difference is not significant. Not only time to T4 (9.7 vs. 8.3 min., mean, p<0.001), but also stay at PACU, recovery of sensory and motor block and start of postoperative pain were all significantly shorter in Group 1. None of both groups had post-dural puncture headache (PDPH). CONCLUSIONS: We can conclude that CSEA, when combining main spinal and supporting epidural anesthesia, has greater efficacy and less side effects for cesarean section than pH adjusted EA.
