3.Prognostic parameters in Renal Cell Carcinoma.

This Study was designed to characterize the unfavorable prognostic factors of renal cell carcinoma through reviewing 27 cases with radical nephrectomy during the past 10 years from 1976 through 1986. The presence or absence of tumor invasion into renal capsule, perinephric fat, renal vein, regional lymph node, pelvis and/or ureter, hematuria at the time of first examination and the difference in the size of tumor and tumor cell type were studied. The 100 person-year mortality rates were analyzed with the chosen prognostic variables. Tests of significance between the difference in mortality rates of each two groups with or without a given characteristics were performed with Z-test. Author results as follows. : 1. The survival rate was decreased independently by the unfavorable factors such as the presence of tumor invasion into renal vein, regional lymph node, pelvis and/or ureter and tumor cell type other than clear cell(P<0.001, <0.001, <0.001, <0.05 respectively). 2. The survival rate was not affected by the following factors such as the presence or absence of hematuria at the time of first examination, tumor invasion into renal capsule, perinephric fat and the difference in size of tumor.

From May, 1971 to June, 1984, we performed extensive saucerization on the chronic osteomyelitis of long bones eradicating all pathologic foci; not only sequestra but sclerotic involocurum and necrotic original cortex embeded by new bone, which was different from the conventional methods such as sequestrectomy or guttering in its extensiveness. Clinical analysis on the base of radiologic evaluation after saucerization was done on the 16 patients; for average 5 years of follow-up. The results obtained were as follows; 1. The angulatory deformity of the long bones after pathologic fracture complicated by the chronic osteomyelitis was not corrected satisfactorily due to the bony sclerosis or hyperostosis at the fracture site, especially in cases of posterior or medial angulation. 2. The recurrence was closely related to the persisting non-sequestered original cortical lesion after incomplete saucerization, which was embeded by the new bone. So to prevent the recurrence the non-sequestered original cortical lesion must be removed completely. 3. The cortical defect after saucerization was restored completely in patients under the age of 14, but which was not the way in patients over the age of 16.

8.Clinical Analysis for Complications of Nd-YAG Laser Posterior Capsulotomy in Uveitis Patients.

Nd-YAG laser is known as a safe and effective treatment for posterior capsular opacity after cataract surgery. We studied the ocular complications after Nd-YAG laser treatment in uveitis patients. From January 1991 to December 1996, we retrospectively investigated the complications of uveitis in 22 persons(25 eyes) after Nd-YAG laser posterior capsulotomy, and also investigated that of nonuveitis in 563 persons(635 eyes) as a control group. The uveitis cases consisted of six anterior uveitis, eight intermediate uveitis, seven Behcet`s syndromes, three panuveitis, and one posterior uveitis. In the uveitis group, the complications(18 eyes) consisted of three retinal detachments, one giant retinal tear, three glaucoma, two aggravations of uveitis, two vitreous opacities, one retinal emorrhage, one proliferative vitreoretinopathy, one hyphema, and four transient high intraocular pressures(IOP), and in the control group, it consisted of two subluxations of intraocular lens(IOL), three glaucomas, one retinal tear, three transient high IOPs, two vitreous opacities, two retinal hemorrhages, three macular holes, two retinal detachments, and five cystoid macular edemas. From our experience, the incidence of complications after Nd-YAG laser posterior capsulotomy in uveitis was higher than that in nonuveitis. It was also noted that it is necessary to follow up thoroughly the complications after Nd-YAG laser posterior capsulotomy in uveitis patients.
