PURPOSE: The records of 12 patients diagnosed with a fracture of the penis were reviewed to assess the usefulness, not only of penile ultrasonography, but also the outcome of an emergent surgical operation for a fracture of the penis. MATERIALS AND METHODS: Twelve patients with a fracture of the penis, as a result of blunt trauma (mean age 41 years old), were reviewed. Of the 12 patients, 9 fractured their penis during sexual intercourse, and 3 during penile manipulation. Patients were managed with an emergent surgical operation, which was performed on the basis of a preoperative cavernosography, penile ultrasonography and retrograde urethrography. All 12 patients were treated with a surgical operation, with follow-up examinations performed 2 weeks later. However, only 5 patients were followed-up over a long period (average 3.5 year). RESULTS: To identify the rupture site of the corpus cavernosum, both penile ultrasonography and cavernosography were performed, and the precise rupture site also confirmed. All patients presented successful outcomes, with the exception of 3 that complained of intermittent painful erections. Five patients were able to preserve potency during the long evaluation period following the operation. However, two patients complained about a mild curvature of the penis and three had intermittent painful erections. CONCLUSIONS: Using penile ultrasonography for the purpose of diagnosing patients with a fractured penis is useful as an investigative methods, and the use of an additional diagnostic tool is almost unnecessary. Despite minimal complications (2 penile curvatures and 3 intermittent painful erections), the result of an emergent surgical operation was excellent for the preservation of sexual potency in patient with a fracture of the penis.

The thyroid nodules are the most frequent encountered disorders of the endocrine system. A clinical study of 34 cases with thyroid nodules, which were treated by surgical excision and confirmed pathologically at the department of otolaryngology-head and neck surgery, Kon-Kuk University hospital, from January 1990 to July 1995, was conducted. This study was designed for early diagnosis and proper treatment of thyroid nodules through clinical analysis. The clinical analysis results were obtained as follows: 1) The sexual distribution showed the preponderance of female with 1:5.8. 2) The most prominent sign was palpable nodule on anterior neck. 3) The duration of illness within 3 months was most common(51.5%), and the size of thyroid nodules was parallel to the duration of illness. 4) Most nodules were unilateral in 29/34(85.2%), among them, right lobe in 47%, lower lobe in 38.2%. 5) The consistency of nodules was soft in 12 cases(31.5%) and most nodules were single in 26 cases(76.4%). 6) Fine needle aspiration biopsy showed consistent with surgical result in 13/19 cases(70%) at benign nodules and in 3/4 cases(75%) at malignant nodules. 7) The histopathologic classification of benign diseases in decreasing order of the frequency were adenomatous goiter, 17.28 cases(60.7%); follicular adenoma, 7/28 cases(25%); Graves' disease, 0 cases. The malignant diseases were papillary carcinoma, 6/6 cases(100%). 8) The frequent performed operative procedure was unilateral lobectomy 28/34(82.3%) in both benign and malignant disease. 9) The postoperative complication occured in 12/34 cases(35.2%) including 5 cases of mild dysphagia(14.7%) and 2 cases of transient hoarseness(5.8%) and 1 case of hypocalcemia(2.9%) and 1 case of ecchymosis(2.9%) in wound and were related to the extent of the surgical procedure.

The urinary bladder is seldom injured by external forces, as it is located in the pelvic cavity, behind the symphysis pubis, and protected by the bony pelvis. Gynecological procedures are a major cause of bladder injury. Herein, a case of extraperitoneal bladder rupture, identified 6 days after vaginal delivery in a 27-year-old woman, with no history of cesarean section or pelvic surgery, and who was cured of the bladder rupture using conservative treatment with the indwelling of a urethral Foley catheter, is reported.

PURPOSE: Intravesical prostatic protrusion (IPP) is a morphological change of prostate protruded into bladder and might be related to bladder outlet obstruction (BOO) depending on the extent of protrusion. There is a high possibility that lower urinary tract symptoms (LUTS) tends to get worse as IPP grows. Therefore, it is necessary to examine the effect of IPP on LUTS after revision of prostate volume in order to identify the direct effect of IPP on BOO and LUTS. MATERIALS AND METHODS: A retrospective study of 296 male patients diagnosed with benign prostate hyperplasia (BPH) between August 2006 and December 2008 were performed. The patients were evaluated with international prostate symptoms score (IPSS) and quality of life (QoL), uroflowmetry (UFR), post-void residual urine volume (PVR), prostate volume and IPP as measured by transurectal ultrasound (TRUS). The changes of IPSS after 8 weeks of medication treatment and the occurrence rates of transurethral resection of prostate (TURP) and acute urinary retention (AUR) were compared. By checking IPSS and UFR, the improvement of voiding symptom was evaluated. RESULTS: The population of patients with moderate enlargement (30-50gm) of prostate extent was 147 (49.6%) out of 296, the whole examined population. There were correlation between IPP and IPSS (p=0.002) and the storage symptoms score was significantly increased (p=0.014). After 8 weeks of medication treatment, both moderate BPH patients with IPP showed similar improvement in storage symptom compared to non-IPP groups. But, the TURP was significantly performed more with IPP (p=0.040) than non-IPP groups and more AUR has occurred (p=0.013). After TURP, IPP group resulted in improvement of IPSS, storage symptoms score and voiding symptoms score statistically compared to non-IPP group. As the change of UFR and IPSS after TURP had shown, the improvements of voiding volume, maximal flow rate (Qmax), and average flow rate (Qavg) in IPP groups were smaller however, it is no correlation in statistical view. CONCLUSIONS: Moderate BPH group with IPP has a higher possibility of having AUR and surgical treatment while showing significant correlation with storage symptoms. A further prospective study is necessary for identification of improvement of IPSS and UFR after TURP and IPP should be checked carefully during TRUS.

PURPOSE: Chronic pelvic pain syndrome(CPPS) is a debilitating condition characterized by pelvic pain and voiding symptoms. Pelvic floor tension myalgia is hypothesized to be a contributing factor in CPPS. The therapeutic effect of biofeedback and electrical stimulation for CPPS was evaluated prospectively. MATERIALS AND METHODS: Thirteen patients with CPPS intractable to conventional medication were enrolled. The patients were treated by domestic KontinenceTM equipment 2 times a week for 6 weeks. Patients completed the National Institute of Health Chronic Prostatitis Symptom Index(NIH-CPSI) before and after treatment. RESULTS: The average score for all 3 domains in NIH-CPSI significantly decreased. Pain domain decreased from 12.9+/-3.4 to 9.8+/-1.7(p=0.002), voiding symptoms from 4.5+/-1.6 to 3.2+/-0.9(p=0.006), and impact plus quality of life from 7.3+/-2.2 to 4.7+/-1.5(p=0.003). CONCLUSIONS: Biofeedback with electrical stimulation therapy can provide significant improvement in CPPS intractable to medication. Large, randomized trials are needed to confirm its efficacy.

BACKGROUND: Next-generation sequencing (NGS) can detect many more microorganisms of a microbiome than traditional methods. This study aimed to analyze the vaginal microbiomes of Korean women by using NGS that included bacteria and other microorganisms. The NGS results were compared with the results of other assays, and NGS was evaluated for its feasibility for predicting vaginitis. METHODS: In total, 89 vaginal swab specimens were collected. Microscopic examinations of Gram staining and microbiological cultures were conducted on 67 specimens. NGS was performed with GS junior system on all of the vaginal specimens for the 16S rRNA, internal transcribed spacer (ITS), and Tvk genes to detect bacteria, fungi, and Trichomonas vaginalis. In addition, DNA probe assays of the Candida spp., Gardnerella vaginalis, and Trichomonas vaginalis were performed. Various predictors of diversity that were obtained from the NGS data were analyzed to predict vaginitis. RESULTS: ITS sequences were obtained in most of the specimens (56.2%). The compositions of the intermediate and vaginitis Nugent score groups were similar to each other but differed from the composition of the normal score group. The fraction of the Lactobacillus spp. showed the highest area under the curve value (0.8559) in ROC curve analysis. The NGS and DNA probe assay results showed good agreement (range, 86.2-89.7%). CONCLUSIONS: Fungi as well as bacteria should be considered for the investigation of vaginal microbiome. The intermediate and vaginitis Nugent score groups were indistinguishable in NGS. NGS is a promising diagnostic tool of the vaginal microbiome and vaginitis, although some problems need to be resolved.

We report a suspicious case of spontaneous bacterial peritonitis (SBP) caused by Providencia rettgeri and Clostridium perfringens in a patient with alcoholic cirrhosis. The patient presented with altered mentality and was taken to the emergency room. He was diagnosed with SBP after abdominal paracentesis and computed tomography and was treated with ceftriaxone and metronidazole. The pathogens were identified under suspicion of polymicrobial infection because of Gram-staining discrepancies between broth from blood culture bottles and colonies on solid media. He died of septic shock despite transfer to the intensive care unit. Although we could not conclude which organism had the leading role in this case of SBP and septicemia, we did verify the importance of Gram staining in a microbiology laboratory in terms of quality assurance.
