1.A Case of Lymphocytoma Cutis Treated with CD2 Slush.

A case of lymphocytoma cutis, the circumscribed form in 9 year-old girl is reported. This is seldom conclusive without histological examinatien for a wide variety of conditions has to be considered. The circumscribed form usually responds rapidly to radiotherapy, although recurrence is possible and good results from the use of procaine penicillin especially in the disseminated form. In this case, I experienced good results from CO, slush and topical application of 0.25 % fluocortolone 3 times a day without recurrence in 3 months after treatment.

2.A Clinical Study of Urticaria.

Urticaria is a common disease and a well known reaction pattern of the skin eharacterized by erythema or wheals and edema, that is the results of a local leakage ef plasma, from minute vessels into the connective tissue of the dermis. Seven hundred and sixty cases of urticaria visited to this department from January 1973 to December 1974 were studied clinically in various ways. Patients were classified as acute and chronic urticaria-acute form when the hives have been present for less than 8 weeks and chronic form when the hives lasted longer. In all patients, a detailed history, a complete physical examination, a total and differential leukocyte count, erythrocyte sedimentation rate and routine stool examination were performed. Foods were incriminated as the exciting causes of urticaria by history, diet elimination test and ingestion test. An ice cube test was done for corroboration of cold allergy. Cholinergic urticaria, was diagnosed by the appearance of small wheals surrounded by erythema after exercise, emotional disturbances or hot drinks. The diagnosis of urticaria due to drug was based on the history of urticaria following the taking of a certain drug. Infection as the inciting cause was determined by history and physical examination. The results: 1. The incidence of urticaria is 4. 6% to total number of dermatologic patients. 2. Among the 760 urticaria patients, acute cases were 498 which were 2 times more than chronic cases, and female patients were 474 cases that revealed marked high incidences than male patients. 3. The age of the patients ranged from 2 months to 87 years, and the patients between 20 and 39 years occupied more than half of the all patients. 4. Abnormal hematologic findings were more marked in acute form; leukocytosis and neutrophilia were dominate in acute form, and lymphocytosis and eosinophilia were more marked in chronic form. 5. Among the etiologic factors, the bacterial infection was the most frequent which occupied 22. 4% of the all urticaria patients. 6. Seven cases of urticaria due to parasites were clonorchis sinensis, in which I had experienced the complete recovery with the treatment of subcutaneous injection of clonorchis sinensis antigen (1: 10,000) 0.01- 0.1ml at the interval of one week. 7. In this study, the inciting or perpetuating causes could not be found in 50.8% of patients-46.2% of acute form and 59.5% of chronic form.

3.A Case of Insect Sting by Euponera chosensis.

Ants(Family Formicidae) are very successful organisms occuring in trernendous numbers worldwide in terrestrial habitats. All ant species may bite, and on species sting. 48 species in 26 genus have been reported in Korea, but only systemic reaction induced by the anti Brachyponera chinersis, has been reported in the Korea terature. A 48 year old man was stung on the neck, abdomen, and dorsum of the hand by flying stinging ants. He experienced moderate pain, burning and itching sensation. Clinical course and histopathologic findings of the lesions caused by the stinging ant was observed without specific treatment. The stinging ants collected from t,he patient.s skin and his house were identified as Euponera chosensis belonging to Subfamily Ponerinae. To the best of our know ledge, this is the first clinical repor of the insect sting by Euponera chosensis in the Korean literature.

4.Monilethrix with Keratosis Pilaris.

Monilethrix is a rare hereditary disorder of hair which was described by smith in 1879. The scalp hair is most frequently affected but the body or sexual hair are rarely affected. The affected hair shows periodic constrictions that break easily, so the hair could not grow more than a few millimetere long. It is thought that the node of the affected hair is normal and the thin internodal segment is deficient in hair matrix. Keratosis pilaris is generally an associated feature. We experienced a case of monilethrix with keratosis pilaris. The 4 years old male patient with monilethrix affected only scalp hair with keratosis. The family history was non-contributory.

5.A Clinical Study of the Severe Ankle Fracture

The primary goal of treatment of the adult ankle fracture is to effect the returning of normal func tion that is accomplished by the accurete reduction and rigid internal fixation followed by the early institution of motion. The authors have reviewed a series of 41 cases of the bimalleolar and trimalleolar ankle fractures that treated in Orthopedic Department of Eulji General Hospital, Seoul, from January 1982 to June 1986. With the duration of follow-up of average 13.2 months, we summarized the obtaind results as followings. l. Among the forty one patients, thirty one was male patient(M: F =3.1: 1). The average age was 33.2 years old. 2. The main cause of the fracture was traffic accident, 18 cases(43.9%) and other causes were slipping down(24.4%), fall from a height(14,6%), industrial injury(12.2%) and other injuries(4.9%). 3. According to the modified Lauge-Hansens classification, the supination-external type was most common (41.5%). 4. As a diagnostic method, the computerized tomography was also available. 5. All cases were treated by open reduction and internal fixation with the good radiologic results of 91.3% in bimalleolar fractures and 88.9% in trimalleolar fractures. 6. Of the bimalleolar fractures, ten patientz showed normal range of motion in 2 to 4 weeks, average 18.4 days, by early joint exercise.

6.A Study on the Seasonal and Topographic Variations of the Skin Color of Young Women.

Sixty-five volunteers whose skin had never been tanned intentionally participated in this experiment. The skin colors of extensor and flexor of forearm, fore-head, and cheek were measured with Color and Color Difference Meter(Nippon Denshoku Kogyo Co. CP6R 1001-DP) which represents .kin color as 'L, a, and b in values. The L, a', and b values mean the degree of ligntness, redness, and yellowness respectively. Measurement has been performed seven times repeatedly in a year at regular intervals. From the study, the following results were obtained. ]'lexor skin of forearm showed the highest lightness in a year compared to the other regions. All of the regions examined showed the highest lightness in spring compared to the other seasons. 3. Cheek skin showed the highest redness among the four checked regions in a year. 4, All of the checked regions showed the higher redness in summer than in spring. and autumn. 5 All of the checked regions showed the lowest yellowness in May.

7.Production of IFN-gamma by HBsAg - reactive T cells correlates with viral clearance in HBV infection.

The T cell responses to hepatitis B surface antigen (HBsAg) were analyzed in acute hepatitis patients, chronic active hepatitis (CAH) patients and asymptomatic carriers. Neither proliferative responses nor substantial cytokine production of peripheral blood mononuclear cells (PBMC) in response to HBsAg was detected. For further studies, HBsAg- reactive T cell lines were prepared from PBMC of the hepatitis patients and asymptomatic carriers. No proliferative response of the T cell lines was observed. Interestingly, however, T cell lines obtained from acute hepatitis patients were found to produce IFN-r, but not IL- 4, in response to HBsAg stimulation, whereas T cell lines obtained from CAH patients and carriers were not. Results of this study suggest that HBsAg-reactive T cells producing Thl type cytokines may play an important role in the viral clearance during acute infections, while defects in those T cells may be responsible for the viral persistency.

8.The Use of Lung Ultrasound in a Surgical Intensive Care Unit.

BACKGROUND: Pulmonary complications including pneumonia and pulmonary edema frequently develop in critically ill surgical patients. Lung ultrasound (LUS) is increasingly used as a powerful diagnostic tool for pulmonary complications. The purpose of this study was to report how LUS is used in a surgical intensive care unit (ICU). METHODS: This study retrospectively reviewed the medical records of 67 patients who underwent LUS in surgical ICU between May 2016 and December 2016. RESULTS: The indication for LUS included hypoxemia (n = 44, 65.7%), abnormal chest radiographs without hypoxemia (n = 17, 25.4%), fever without both hypoxemia and abnormal chest radiographs (n = 4, 6.0%), and difficult weaning (n = 2, 3.0%). Among 67 patients, 55 patients were diagnosed with pulmonary edema (n = 27, 41.8%), pneumonia (n = 20, 29.9%), diffuse interstitial pattern with anterior consolidation (n = 6, 10.9%), pneumothorax with effusion (n = 1, 1.5%), and diaphragm dysfunction (n = 1, 1.5%), respectively, via LUS. LUS results did not indicate lung complications for 12 patients. Based on the location of space opacification on the chest radiographs, among 45 patients with bilateral abnormality and normal findings, three (6.7%) and two (4.4%) patients were finally diagnosed with pneumonia and atelectasis, respectively. Furthermore, among 34 patients with unilateral abnormality and normal findings, two patients (5.9%) were finally diagnosed with pulmonary edema. There were 27 patients who were initially diagnosed with pulmonary edema via LUS. This diagnosis was later confirmed by other tests. There were 20 patients who were initially diagnosed with pneumonia via LUS. Among them, 16 and 4 patients were finally diagnosed with pneumonia and atelectasis, respectively. CONCLUSIONS: LUS is useful to detect pulmonary complications including pulmonary edema and pneumonia in surgically ill patients.

9.Temporal and Spatial Expression of Bone Morphogenetic Protein -2 and -4 mRNA in Distraction Osteogenesis and Fracture Healing.

Temporal and spatial expression of bmp-2 and bmp-4 was investigated in distraction osteogenesis and fracture healing models in order to delineate their roles in these new hone formation processes. Distraction osteogenesis was performed on the left tihia diaphyses of Sprague-Dawley rats, and was confirmed by serial radiographs and histologicaJ examination. Total RNA was isolated from the distraction gaps and fracture sites sequentially until the postoperative eighth week, and mRNA expression was quantitated hy competitive reverse transcription-polymerase chain reaction(RT-PCR) using specific primers for bmp-2 and bmp-4. In-situ hybridization was performed on the undemineralized tissue section of distraction osteogenesis group. Both bum-2 and bmp-4 mRNA expression increased during distraction osteogenesis and fracture healing. Increase in bmp-2 mRNA expression was more marked and prolonged in distraction osteogenesis compared with fracture healing, whereas bmp-4 mRNA expression was persistently increased in both groups. In-situ hybridization study revealed that hoth bmp-2 and bmp-4 were expressed at eariy osteohlasts producing osteoid, trabeculae-lining osteoblasts and osteocytes of regenerate bone. Enhanced new bone formation in distraction osteogenesis is associated with prolonged and increased expression of BMPs, especially BMP-2. lnspite of their structural similarity, BMP-2 and BMP-4 appear to have their distinct roles in new hone formation.

10.HAIR TRANSPLANTATION FOR MALE PATTERN BALDNESS AND OTHER ALOPECIAS.

The increasingly successful results of hair restoration surgery in the last years have developed the interest and the confidence of patients and it is the most frequently performed esthetic surgery among male patients. Currently, various kinds of techniques for hair restoration surgery including hair transplantation, scalp reduction, and scalp flaps are performed in our clinic. Among these, hair transplantation is the basic and the most popular procedure, which can be performed not only by itself but also in conjunction with other procedures. 123 consecutive cases of hair transplantations were peformed from Jan. 1995 to Feb. 1996 for male pattern baldness, traumatic alopecia, and female pattern baldness under out patient base. The authors introduced a new classification for male pattern baldness, which are the type M, O, C, U, M-O, and C-O after alphabet to make simple and easy for clinical application. The ancillary procedures were scalp reduction, preauricular flap, and scalp expansion. An ellipsis of hair bearing scalp taken from the occipital area was sliced into slit-, mega-, mini-, and micro-grafts. The survival rate of the grafts was over 90% with minimal complications. This hair transplantation technique enabled us to achieve a good density and more natural looking hair with avoidance of cobble stoning and apparent scar.
