2.Anterior Knee Pain after Tibia Intramedullary Nailing.

Treatments of tibia shaft fracture are closed reduction followed by cast immobilization, external fixation, intramedullary nailing and internal fixation with plate and screws. Among these, intramedullary nailing is commonly used method today. The tibia nailing has high rate of union, but malunion, infection, the joint stiffness and anterior knee pain develops frequently. We reviewed the clinical and radiological result of anterior knee pain retrospectively after tibia nailing. The result were as follows: l. Age distribution of tibia shaft fracture was 17 to 76, and mean age was 40 male patients were 35 and female 13. 2. The open fractures were 17 and the closed fractures were 34. 3. The causes of' tibia shaft fracture were traffic accidents(30cases), fall down injuries(6 cases), slip down injuries(1 1 cases) and other injuries(4 cases) Among the these, 19 cases were cornbined with other fractures or neuri.isurgical injuries. 4. The incision methods were recorded as follows: parapatella tendon incision were 9 cases, patella splitting incision were 25 cases and unrecorded were 17cases. 5. According to the radiological analysis, the average nail plateau distance was 10.3mm and the extent of nail protrusion beyond the anterior cortex was -1.73mm. Among these, protrusion of proximal targeting screws were 8 cases. 6. Insertion of nail through the patella tendon was associated with a hipher incidence of knee pain compared to paratendon site of insertion. The extent of nail protrusion were related to anterior knee pain after tibia intramedullary nailing.

3.Clinical Study on Surgical Treatment of Spondylolisthesis

Spondylolisthesis has been become of considerable causative disease for low back pain in orthopedic field recently. Twenty-seven patients with spondylolisthesis, who were taken operation during a 7-year period from Jan. 1978 to Dec. 1984 at Department of Orthopedic Surgery of Presbyterian Medical Center, were studied and analized on clinical and radiological aspects and the results of obtained were as follows: 1. The averake age was 41.7 years with a range of 20 to 63 years, and the incidence was more common in female than male in all types. 2. By Wiltse's classification, isthmic type was most common and 19 patients were included and level of spondylolisthesis was common at fourth lumbar vertebra in degenerative type and fifth lumbar vertebra in isthmic type. 3. By Meyerding's grading, 14 patients (52%) were Grade 1, which was most common and the slipping measured by Taillard's method was most common with a range 10% to 19%, which included 13 patients (48%) and average slipping was 21.4%. 4. Meschan's angle was ranged from 5° to 19° with an average of 13.3°. 5. Myelogram was done in 13 patients and 9 of them showed positive findings. 6. Spina bifida was most common developmental abnormal finding and 11 patients (41%) were included. 7. Posterolateral fusion was done in 24 patients and anterior interbody fusion was done in 3 patients. 8. 24 patients who had taken posterolateral fusion were permitted to ambulate early at 2 weeks with Norton-Brown back brace. 9. After over a year follow-up, 23(85%) of 27 patients had satisfactory result. 10. After follow-up, 21(88%) of 24 patients who had taken posterolateral fusion had successful fusion.

5.Arthroscopic Meniscus Repair: A Short Term Result.

The function of the meniscus and subsequently the importance of meniscus preservation are well known. Thirty-two patients who underwent arthroscopic meniscus repair between 1993 and 1995 were studied retrospectively. Twenty menisci were repaired using modified Henning inside-out technique and the remaining twelve meaisci, outside-in technique. The average delay in surgery was two hundred days. Follow-up was done from 1 year to 3 years (average 22.4 months). The medial meniscus was repaired in 22 cases and the lateral meniscus including two saucerized discoid meniscus in 10 cases. Fourteen patients had anterior cruciate ligament (ACL) tear and other associated knee injuries were lateral condyle fra'cture of femur in one case and lateral condyle &acture of tibia in one case. Assessment of the result was based on the evaluation form of the International Knee Documentation CommitteegKDC). Twenty-seven patients (84.3%) were normal to nearly normal. Abnormal and severely abnormal results were mainly affected by the result of treatment of ACL tear.

7.Analysis of the Urinary Calculi by Infrared Spectroscopy.

The exact analysis of urinary calculi is very important to study of the etiology of calculus formation, prevention of recurrence and medical treatment. Compared with all the other method, the infrared spectroscopic analysis furnished a permanent record in a very short time. And it is ideal method for qualitative identification and semiquantitative estimation of small fractions of calculi by simple procedure rapidly. A retrospective review of stone analysis by infrared spectroscopy, with KBr tablet method was performed in 105 cases of 219 urinary stone patients, who had been admitted to the Fatima Hospital from May, 1980 to December, 1983. And the following results were obtained 1. Mixed calculus is more than single calculus(1 : 1.56) . The most common type of calculi is calcium oxalatetribasic calcium phosphate (47.6%) . And the most common type of single calculi is calcium oxalate (29.5%) . 2. The most common type of component is calcium oxalate (55.6%) 3. Calcium oxalate-tribasic calcium phosphate is the most common type in the renal (46.2%) and the ureteral stones (53%) . But in the bladder stones, magnesium ammonium phosphate is the most ( 33.3%). 4. Among the 10 laminated stones, 4 cases contain the different components in nucleus and outermost layer 5. Pyuria is observed in 48.6% of all urinary stone patients, in 32.3% of calcium oxalate stone and in more than 80% of magnesium ammonium phosphate stone. 6. Calcium oxalate stone is most common in the age of 20, calcium oxalate-tribasic calcium phosphate stone are in the age of 20 & 30, and magnesium ammonium phosphate stone is above the age of 40.

9.Gallbladder carcinoma: Regional lymph nodes metastases on CT scan.

CT scans of 23 patients with gallbladder carcinoma were retropectively reviewed to determine the incidence of lymphatic spread and to access the pattern of regional lymph node metastases. The lymphatic spread were noted in 74%(17/23). The involved patterns of regional lymph node metastases were nodes of porta hepatis 65%, portocaval nodes 41%, superior pancreaticoduodenal nodes 47%, posterior pancreaticoduodenal nodes 59%, nodes around the celiac trunk 35%, nodes around the superior mesenteric artery 29% and paraaortic nodes 35%. The findings suggested that the pattern of regional lumph node metastases in gallbladder carcinoma occurred along the normal lymphatic pathway of the gallbladder.
