2.Choledochal cyst with ectopic distal location of the papilla of Vater.

In cholangiographic techniques, the close relationship between choledochal cyst and anomalous union of pancreaticobiliary duct has attracted medical attention. There have been rare cases in which the papilla of Vater was found in a position other than its normal position, and such cases have been reported sporadically. However, such cases are interesting in the anatomical context. In this review, we present our experience of choledochal cyst in a 30-month-old boy in whom the papilla of Vater was positioned in the third portion of the duodenum.

3.A Clinical Study on Antihypertensive Effects and Safety of Benidipine.

BACKGROUND: This study was designed to evaluate the antihypertensive efficacy and safety of benidipine hydrochloride, a Ca(++)-channel blocker, in patients with mild to moderate essential hypertension. METHODS: Benidipine was administered in 2-8mg once daily for 10 weeks in 16 hypertensive patients with diastolic blood pressure over 95mmHg and adverse effects were checked every two weeks after benidipine administration. Chest X-ray, ECG, funduscopy, and laboratory examination were performed before and after benidipine administration. RESULTS: The antihypertensive effect of benidipine was evaluated in 15 patients and the safety in 16 patients. The blood pressure significantly reduced from 170+/-12mmHg/102+/-5mmHg to 137+/-15mmHg/86+/-8mmHg at 10-week administration of benidipine and the overall effective rate was 100%. Heart rate was not affected by benidipine. The slight increase of total protein, BUN, potassium, and glucose was observed at 10 weeks of benidipine administration. Four cases of headache and 1 case of frequent urination were observed and the medication was discontinued in one patient due to headache. CONCLUSION: Benidipine proved effective and safe in the treatment of essential hypertension.

4.A Case of Supernumerary Kidney.

5.Bowen's Disease and Internal Malignacy.

It is well known that Bowen's.disease, a precancerous dermatosis can be associated with internal malignancy. We report 2 cases of Bowen's disease of probable arsenic origin associated with. internal malignancies. Case one, a 55 year-old man, who had the past history of arsenic intake and herb medication for the treatment of suspicious leprosy, had Bowen's disease and arsenical keratosis on his palms and soles with bronchogenic and stomach carcinoma. The other case, a 65 year-old man, who had history of intake of arsenic and herb medicine for epilepsy, had Bowen's disease and squamous cell carcinoma of the skin with stomach cancer. We suggest that in these 2 cases, the ingestion of organic arsenics might be the causative factor for the internal malignancy.

6.Biological Characterization of HIV-1 Isolates from Long-term non-progressors (LTNP) and Rapid Progressors (RP) in Korea.

To analyze the correlation between biological phenotypes of HIV-1 isolates and disease progression, we selected 9 long-term non-progressors (LTNP) and 12 rapid progressors (RP) from HIV-1 infected Korean. We isolated HIV-1 isolates by culture of PBMC of LTNP and RP with normal PBMC and measured HIV-1 p24 antigen production. The HIV-1 isolation rate from LTNP was 55.6% (5/9). And 4 HIV-1 LTNP isolates were non-syncytium inducing (NSI) phenotype and showed slow/low replication. The HIV-1 isolation rate from RP was 91.7% (l1/12) which was higher than that from LTNP. Besides 3 RP HIV-1 isolates which showed syncytium inducing (SI) phenotype, 8 RP HIV-1 isolates showed NSI phenotype in normal PBMC and MT-2 cell line. All RP HIV-1 isolates replicated more rapidly than LTNP HIV-1 isolates. Comparing the replication kinetics and syncytium forming capacity of HIV-1 isolates from LTNP and RP, we suggest that the difference of biological phenotype of HIV-1 isolates could be related with disease progression of HIV-1 infected persons.

9.The Role of Preoperative Fine Needle Aspiration Cytology (FNAC) in Differential Diagnosis of Thyroid Nodules.

BACKGROUND: Fine Needle Aspiration Cytology (FNAC) is widely accepted as the most accurate procedure to differentiate malignant from benign thyroid nodules. It is a simple, safe, and inexpensive procedure, and the accuracy of FNAC is relatively high; however, its success usually depends upon the skill in using the aspiration technique and the experience and skill of the cytologist. The authors confirmed the diagnostic value of preoperative FNAC in differential diagnosis of thyroid nodules treated by the surgical resection. METHODS: Three hundred fifty specimens were taken in the Department of Surgery, Inje University College of Medicine, from July 1990 to June 1996. Fifty-one males and 299 females with ages ranging from 11 to 73 years were enrolled. The cytology of FNAC was compared to the final histologic diagnosis. RESULTS: The number of inadequate smears (or sampling error) was 15.7%. Preoperative cytologic diagnoses was consisted of 198 cases of benign disease (92 cases of nodular goiters, 96 cases of follicular lesion types III, and 10 cases of thyroiditis), 12 cases of highly suspicious malignant lesions (follicular lesions type II), and 85 cases of malignant disease (71 cases of papillary carcinomas and 14 cases of follicular lesions type I). Histologic diagnoses consisted of nodular goiters in 116 cases, adenomas in 66 cases, thyroiditis in 9 cases, papillary carcinomas in 84 cases, and follicular carcinomas in 20 cases. These results has an 84.6% sensitivity, a 95.2% specificity, a 15.3% false negative rate, and a 4.7% false positive rate. In the fifty-five cases of inadequate sampling, 13 cases (23.6%) were malignant lesions histologically. In the 16 cases proven as false negative, nine cases were papillary carcinomas, and 7 cases were follicular carcinomas histologically. Thirteen cases (16.1% of 27 cases of cystic lesions) of cystic nodules revealed malignant neoplasms histologically. CONCLUSION: FNAC is a single, useful diagnostic procedure for the initial screening for malignancy in thyroid nodules. Performing repeated aspiration in cystic nodules and clinically suspicious cases could reduce inadeguate sampling and the false negative rate.

10.Clinical Observation on Renal Injury.

A clinical observation was made on 73 patients of renal injury at the Department of Urology, Catholic Medical College, during the period from January, 1972 to December, 1981. The results were as follows: 1. The most favorable age was 20-29 years for 31.5%. The sex ratio, male to female, was 2.6:1. 2. The most common causes of renal injury were traffic accidents for 35.6%, and then kick & blow, fall, sport and introgenic in order. 3. The renal injuries in this series were classified as minor injury (71.2%), major injury (27.4%), pedicle injury (1.4%), according to Scott classification. 4. The IVP was done in 68 patients and abnormal findings were obtained in 31 cases. The renal angiography was done in 8 patients and RGP in 5 patients. 5. The complications were noted in 10 cases.
