It has been generally known that hypotension may occur during total hip prosthesis with general anesthesia. However, The cause of hypotension is not well known. In these cases, of total hip prosthesis during general anesthesia, the following results were obstained. 1) during total hip prosthesis, blood pressure fell. 2) intravenous injection of effortil and dexamethasone improved the hypotension.

It has been generally understood that belladonna alkaloids in small doses produce bradycardia. However, it is also known that these agents increase heart rate during general anesthesia. In this study, a comparison of atropine and propantheline on human heart rate during ether anesthesia was made and the following results were obtained. 1) Atropine and propantheline increased heart rate. 2) Effect of propantheline on the heart rate was more marked than that of atropine.

Glycopyrrolate, a synthetic quarternary ammonium compound, has a similar pharmacologic property to that of scopolamine, a belladonna alkaloid. Since glycopyrrolate, a quarternary ammonium compound, dose not readily cross the blood brain barrier, it has less central effect than that of scopoiamine a tertiary-amine compound. The authors administered a small dose of scopolamine and glycopyrrolate to human volunteers, and examined the effect on the heart rate ofr both drugs. The results were as follows: 1) A small dose of scopolamine(0.1mg) showed significnat decrease in the heart rate. 2) A small dose of gylcopyrrolate(0.1mg) indeced little change in the heart rate. 3) A small dose of gylcopyrrolate(0.1mg), 10minutes after glycopyrrolate(0.1mg), showed marked increase in the heart rate. 4) A small dose of scopolamine(0.1mg), given 10 minutes after glycopyrrolate (0.1mg), showed tendency for decrease in heart kate. 5) A small dose of gylcopyrrolate(0.1mg), 10 minutes afterscopolamine(0.1mg), showed significant increases in the heart rate. 6) Scopolamine induced bradycardia was suppressed by an additional dose of glycopyrrolate and a preceding dose of glycopyrrolate.

Mandibular prognathism was defined by Johe Hunter as follows : 'The lower jaw projecting too far forwards so that the foreteeth pass before those of upper jaw, therefore disfigurement and malocclusion are the two of facial main characteristics.' Mandibular ramus osteotomy by vertical osteotomy or sagittal splitting osteotomy recently has come into more common use. Although preoperative and postoperative orthdontic management is indispensable in surgery of this type, we occasionally encounter patients who are intolerant to a long duration of orthodontic treatment and intermaxillary fixation. We describe surgical refinement accomplishing mandibular anterior segmental osteotomy, recession genioplasty by horizontal osteotomies and decortication of the anterior portion of the bony bar for a minor degree of mandibular prognathism. In results, the amount of setback is limited to 4 to 5mm, no intermaxillary fixation is required and no orthodontic treatment, in this operation, is needed. This procedure can obtain a rapid aesthetic improvement. We used this preocedure in 8 patients (3 males and 5 females) with a mild mandibular prognathism. No major complications occurred throughout the follow-up period except transient paresthesia of the mental nerves, otherwise all patients were satisfied with the result.

Fracture of the scaphoid constitute 60% to 70% of all diagnosed carpal injury. The acute fracture will heal approximately 90% of the time if recognized early and properly immobilized. But nonunion are common since the symtoms do not alert patients to seek early medical treatment and the diagnosis is easily missed. Authors have experienced 19 cases of scaphoid nonunion and accomplished good result in all case by Russe procedure. The results were summerized as follow: 1 The cause of fratures was mainly due to fall down dinjury (36.8%). 2. The most common mechanism of the fracture was fall on the outstretched hand (52.7%). 3. Fractures were shown on the anterior-posterior, lateral and billiards view in all cases. 4. In ten cases, the fractures were found on the waist of scaphoid. 5. The cases of nonunion were probably inadequate intial treatment and delayed diagnosis. 6. The good results were obtained by bone graft according to Russe procedure.

Purpose: A hyperextended knee is described as knee pain associated with an impaired knee extensor mechanism. Additionally, a hyperextended knee may involve reduced position sense of the knee joint that decreases the individual’s ability to control end-range knee extension movement. The purpose of this study was to investigate the effects of visual biofeedback information for plantar pressure distribution on knee joint angle and lower extremity muscle activities in participants with hyperextended knees. Methods: Twenty-three participants with hyperextended knees were recruited for the study. Surface electromyography signals were recorded for the biceps femoris, rectus femoris, gastrocnemius, and tibialis anterior muscle activities. The plantar pressure distribution was displayed and measured using a pressure distribution measuring plate. Knee joint angle kinematic parameters were recorded using a motion analysis system. The visual biofeedback condition was the point at which the difference between the forefoot and backfoot plantar foot pressure on the monitor was minimized. The Wilcoxon signed-rank test was used to determine the significance between the visual biofeedback condition and the preferred condition. Results: The knee joint angle was significantly decreased in the visual biofeedback condition compared to that in the preferred condition (p<0.05). The rectus femoris and gastrocnemius muscle activities were significantly different between the visual biofeedback and preferred conditions (p<0.05). Conclusion The results of this study showed that visual biofeedback of information about plantar pressure distribution is effective for correcting hyperextended knees.
