2.A Case of Cutaneous Side Reaction of Collagen Implantation.

Injectable Collagen Implan, is composed of purified bovine dermal collagen emulsified in saline and lidocaine(0.3%) base, and has been promoted for correction of soft tissue contour defects. Although collagen is a relatively nontoxic, nonimmunologic and biolegically nonreactive biomaterial, some side reactions have been reported. We report herein a case of cutaneous side reaction to collagen implantation in a 38-year-old female. The patient presented with puritic, irritating, multiple, erythematous nodules on both the temporal and interorbital area, which had been observed since 1 day after collagen(KOKEN ATELOCOLLA-GEN IMPLANT-Japan) implantation. Histopathologic findings showed dense infiltration of acute and chronic inflammatory cells in the dermis, especially the perivascular and periadnexal area.

Leg length discrepancy in children occurs chiefly as an aftermath of poliomyelitis. In spite of immunization, there remain a few infantile paralysis patients with significant leg length discrepancy. The methods to correct leg length inequality have included epiphysiodesis, long bone shortening and growth stimulation. But the limitation of accomplishing equalization by these methods has made it clear that there is a need for an accurate and uncomplicated leg lengthening method. Anderson and others devised a well controlled distraction apparatus and performed surgery upon many, cases with few complications. This study is a report of 7 consecutive patients all operated on by a reverse method, with the application of the Charnley compression apparatus. The results are as follows. 1. Correction of 2. 5 to 4. 5 cm (average 3.4 cm) was achieved. 2. Major complications were delayed union in 5 cases, pin tract infection in 2 cases and nerve injury in 1 case. 3. When either inadequate union or failure of union of the distracted fragments is established in the mature patients, it is suggested that the fragments should be stabilized with bone graft as soon as possible.

5.CT and MR Findings of Persistent Hyperplastic Primary Vitreous(PH PV).

PURPOSE: The purpose of this paper is to discuss the characteristic CT and MR findings in persistent hyperplastic primary vitreous(PHPV) and to compare the detectability of those findings in each modality. MATERIALS AND METHODS: We retrospectively evaluated CT and MR findings in 32 patients with PHPV. Twenty-five patients had CT, 13 patients had MR, and 6 patients had both CT and MR. RESULTS: Major findings of PHPV in 32 patients on both imaging modalities were lens deformity(78%), shallow anterior chamber(72%), heterogeneous vitreous opacity(72%), enhancing hyaloid artery or remnant of fibrotic hand(69%), and microophthalmos(67%). Minor findings were retinal detachment(22%), and vitreous hemorrhage(6%). In MRI, lens deformity(92%) and shallow anterior chamber(85%) were detected most commonly whereas in CT, opaque vitreous(80%) was the most common finding. Findings of enhancing hyaloid vessel or remnant of fibrotic band, considered characteristic of PHPV, were more commonly detectable in MR (85%) than CT(52%). CONCLUSION: Characteristic MR and CT findings of PHPV were lena deformity, shallow anterior chanber, heterogeneons vitreons opacity, enhanciny hgalind artery or remnant fibrotic band, and microphthalmos. MR seemed to be more useful than CT in detecting Globe pathology.

6.The Clinical Application of C-Reactive Protein in Acute Abdominal Pain.

BACKGROUND: Abdominal pain is one of the most common complaints in the emergency department and the evaluation presents an extraordinary challenge to the emergency physician's skill. C-reactive protein, an acute phase protein which is increased in the presence of inflammation in various clinical conditions, has been proven useful in assessing disease severity, in monitoring the development of complications, and in evaluating the response to specific treatments. So we tried to determine whether CRP offers an advantage over other clinical or laboratory variables for decision-making in the management of acute abdominal pain in the emergency department. METHOD: Patients who came to a University Hospital ED with acute abdominal pain, between september 1, 1998 and November 30, 1998, were included in this study, Data collection included age, sex, duration of symptom, location of pain, and laboratory dta(white blood cell count, portion of neutrophils, ESR, CRP, amylase). RESULT: This study included 85 patients, 34 of whom were men. The mean age was 36.9+/-19.1 years. CRP might be useful to detect the serious condition, sensitivity 81%, but more useful to differentiate normal condition from serious condition in acute abdominal pain patients, specificity 83%. CONCLUSION: CRP is an useful indicator of decision-making to abdominal patients in the emergency department. Hospitalization or operation is very unlikely when CRP value is normal.

7.Resection of Impacted Bilateral Huge Frontal Osteoma Via Endoscopic Modified Lothrop Approach.

Osteomas in the paranasal sinus had been usually treated with external surgical approaches. These techniques are effective in certain conditions such as large size or bilateral involvement of osteomas, with intracranial or orbital complications. This modality, however, have some disadvantages like permanent scar formation, bleeding, mucocele, and paresthesias. The authors recently experienced a huge impacted frontal osteoma, which completely involved the bilateral frontal recess without enough working space to mobilize the tumor. The huge bilateral impacted frontal osteoma was removed with endoscopic modified Lothrop approach due to cosmetic problems. We hereby report this case with a review of literature.

8.Six Cases of Congenital Absence of the Vagina Managed by McIndoe Operation.

Mullerian agenesis or dysgenesis, rare developmental anomaly presenting as primary amenorrhea, is characterized by absence of vagina and absence or rudimentary development of the uterus. Anatomic anomalies of the axial skeleton and kindney often accompany this psychologically devatating condition. There are various methods of surgical treatment for congenital absence of the vagina. The six cases reviewed in this paper were all treated with McIndoe technique. All of 6 women experienced satisfactory sexual activity and were satisfied with vaginal depth following vaginal creation using a split thickness skin graft technique. Despite the existence of several alternative methods, the McIndoe technique is a relatively simple and highly successful procedure and hence is still prefered by many surgeons.

10.A Case of Multilocular Cystic Nephroma in Child.

We present a case of 15 months old male with Multilocular cystic nephroma (MLCN) which contains fibroblast, smooth muscles, skeletal muscles and well differentiated tubular structure in the septa without evidence of blastemal or embryonal elements. There is no evidence of metastasis and local recurrence after post-op 2 years.
