1.A Case of Block Dot Ringworm Caused by trichophyton tonsurans.

Trichophyton tonsurans is a major causative agent of tinea capitis in the United States, Canada, Mezico and other Latin American countries, but the infection of T.tonsurans has not been reported in Korea. We report a case of tinea capitis caused by T. tonsurans in a 15-year-old student wrestler. He had a 3*3cm sized non-inflammatory alopecia with black dots on the right occipital area of the scalp. The diagnosis was established by a hght microscopic examination of the affected hairs and by mycological studies from cultures. To our knowledge, this is the first case in which T. tonsurans has been isolated in Korea, It is likely that the student acquired the infection through contact with a foreign wrestler during interaational games and practice. We should be concerned about the potential of spread of this infection in our country.

2.Three Cases of Coronary Artery Fistula from Right Coronay to Left Ventricle.

The coronary artery fistula from right coronary artery to left ventricle is a rare disease among coronary artery anomaly. We experienced three cases of rare coronary fistula and report with literature review. Although symptoms of coronary artery fistula are associated with arteriovenous shunt and coronary steal phenomenon, many cases are asymptomatic. In this report, all patients had no symptom. but incidental murmur was noted (two are continuous, one is diastolic rumbling). The diagnosis was made by transthoracic or transesophageal echocardiogram and selective coronary angiography. Because patients were relatively young and the diameter of dilated coronary artery were huge, we perfomed operation on three patients.

4.A Case of Primary cutaneous Leiomyosarcoma.

Primary cutaneous leiomyosarcoma is a rare neoplasm. The tumor presents as a nondescript subcutaneous mass, and may be painful or tender. The diagnosis depends on the histopathologic findings. We report a case of primary cutaneous leiomyosarcoma developed in a 57-year-old male. Five and half years ago, the patient noticed a rice grain sized nodule on the right deltoid area. Five years ago, the lesion was excised at a local medical office. From 2 years ago, the tumor recurred on the excision site as a well defined painful erythematous hard tumor, measuring 3.5*3cm. Biopsy specimen showed densely packed, interlacing bundles of smooth muscle cells which have numerous anaplastic nuclei, atypical giant cells with bizarre nuclei and 10 mitotic figures per 10 high power fields.

5.Efficacy of Intrathecal Fentanyl for Tourniquet Pain during Spinal Anesthesia with Hyperbaric Bupivacaine.

BACKGROUND: Tourniquet pain is probably mediated by C-fiber. The ability of fentanyl to interrupt this nociceptive conduction was studied by administering either fentanyl or saline intrathecally along with hyperbaric bupivacaine for spinal anesthesia. METHOD: The incidence of tourniquet pain was evaluated in 60 patients having orthopedic surgery of the lower extremities during spinal anesthesia by administering either 30 mcg fentanyl (group 2) or saline (group 1) along with 0.5% hyperbaric bupivacaine 10 mg. We measured the maximal sensory spread of analgesia to pinprick, the incidence of tourniquet pain, and the sensory anesthesia to pinprick at the onset of tourniquet pain. RESULTS: The average maximal sensory spread of analgesia was the same in both groups (T9). The incidence of tourniquet pain was significantly greater in group 1 (33%) than in group 2 (10%). The sensory levels of anesthesia at the onset of tourniquet pain were not different in two groups. CONCLUSIONS: Intrathecal fentanyl was effective against tourniquet pain for 2 hours of the orthopedic surgery of the lower extremities.

8.Predictive factors of restenosis after successful percutaneous transluminal coronary angioplasty.

OBJECTIVE: Restenosis after successful PTCA remains the main limitation of this technique. Restenosis is a response to injury of the vessel wall, platelet aggregation, thrombus formation, liberation of growth factors, cellular hyperplasia involving predominantly smooth muscle proliferation and migration, and intercellular matrix formation. The identification of risk factors for restenosis could help to prevent and reduce the impact of this phenomenon. We undertook this study to evaluate retrospectively the association between risk factors and restenosis after PTCA METHODS: We studied 123 patients and 174 lesions that underwent successful PTCA and the follow-up period (from PTCA to follow-up angiography) was 8.2+/-5.8 months in all patients. They were divided into two groups according to the restenosis. Clinical feature, lesional feature, PTCA procedural feature, and other risk factors were compared. Restenosis was defined as the cutoff point of >50% in diameter stenosis at angiographic follow-up. RESULTS: 1) Restenosis was found in 68 lesions after successful PTCA(39.1%). 2) Clinical features of restenosis group are similar to no restenosis group 3) Thrombus (p=0.002), total occlusion(p=0.001), severity of stenosis in the initial lesion(p=0.013) and residual stenosis(p=0.001) were found to be independently associated with restenosis, whereas other lesional characteristics were not. 4) Balloon inflation duration(p=0.017) and dissection(p=0.002) were found to be independently associated with restenosis, whereas other procedural characteristics were not. CONCLUSION: Thrombus, total occlusion, dissection, severity of stenosis in the initial lesion, balloon inflation time, residual stenosis were found to be associated with restenosis after PTCA in our study.

9.Tinea Barbae due to Trichophyton verrucosum: Report of 3 Cases.

Although there has been recently noted a nation-wide spread of Trichophyton verrucosum infection in cattle associated with promoted stock raising and dairy farming, there were no case reports in Korean dermatologic literature describing the characteristic clinical features of tinea barbae caused by that particular fungus. We report 3 cases of tinea barbae due to T. verrucosum that developed in male stock farmers breeding cattle. One Patient acted 37, experienced acutely inflamed bo99y tumors on the chin and sub-mandible. The others aged 43 and 46 experienced inflammatory lesions with multiple follicular pustules, crusts and loss of hairs on their upper lips. They were successfully treated with oral griseofulvin and local antifungal ointment combined with short-term oral antibiotics or corticosteroid for about one month.

10.Availability of Optical Coherence Tomography in Diagnosis and Classification of Choroidal Neovascularization.

PURPOSE: The author used optical coherence tomography (OCT) in the identification and classification of choroidal neovascularization to evaluate the potential of this imaging technique for better defining choroidal neovascularization. METHODS: Optical coherence tomography was performed on 20 eyes of 20 patients with the clinical diagnosis of choroidal neovascularization. Its result was compared with fluorescein angiography and indocyanine green angiography RESULTS: Of the 20 eyes, 6 eyes (30%) were identified as classic choroidal neovascularization on fluorescein angiography. Of the 6 eyes, 5 cases were identified as well-demarcated choroidal neovascularization on indocyanine green angiography and optical coherence tomography. One case was identified as fibrovascular pigment epithelial detachment on optical coherence tomograpphy. Of the 13 eyes identified as occult choroidal neovascularization on fluorescein angiography, 6 eyes were identified as well-demarcated choroidal neovascularization on indocyanine green angiography and 7 eyes were identified as well-demarcated choroidal neovascularization on optical coherence tomography. Of the 20 eyes identified as choroidal neovascularization on fluorescein angiography, 3 eyes were missed as choroidal neovascularization on optical coherence tomography. Optical coherence tomography was useful to identify the position and the size of choroidal neovascularization. CONCLUSIONS: OCT was able to distinguish the cross-sectional morphology of various lesions accompanying choroidal neovascularization. This study suggests that OCT may have potential in diagnosing and managing choroidal neovascularization.
