1.The Change of IgG Antibody Titer to Measles, Mumps and Rubella According to Age.

PURPOSE: We investigated the change of antibody titer to measles, mumps and rubella according to age after vaccination. METHODS: The IgG antibody titers to measles, mumps and rubella were tested on the residual serum from patients aged 7-20 years old after routine laboratory testing in the hospital with informed consent from the parents. RESULTS: Antibody to measles was present in 275 cases out of 408 cases with a positive rate of 67.4%, the mean IgG titer was 2.77+/-1.18 Index. Antibody to mumps was present in 112 cases out of 408 cases with a positive rate of 27.5%, the mean IgG titer was 2.08+/-1.29 Index. Antibody to rubella was present in 367 cases out of 408 cases with a positive rate of 90.0%, the mean IgG titer was 60.46+/-63.47 IU/mL. CONCLUSION: It is important to maintain a high rate of vaccination coverage in order to prevent an outbreak of measles, mumps, or rubella. It is also important to stress the maintenance of vaccination records for further reference.

2.A Study of the Clinical Features of Benign Convulsions with Acute Gastroenteritis in Children.

PURPOSE: We investigated the complexities of benign convulsions associated with gastroenteritis in children. METHODS: We selected children diagnosed as afebrile convulsion with acute gastroenteritis from June 2006 to May 2008 who had been admitted to the Kwandong Medical College Myongji Hospital. We investigated clinical features, EEGs, and brain imaging results for patients exhibiting complex-type seizures (Group B) and compared the data with that from patients with simple seizures (Group A). The statistical significance between the two groups was subsequently examined. RESULTS: A total of 43 patients showed afebrile benign convulsions with acute gastroenteritis, and 18 patients were diagnosed as CwG (benign convulsions with acute gastroenteritis). The median age of Group B was 26.3 month which was significantly higher than that of Group A (P < 0.05). The gender ratio was 3.5 times more in higher female of Group A compared to Group B (P < 0.05). The most common type of seizure in both groups was generalized tonic convulsions, and there were no statistical differences in the laboratory results between the two groups. Further, Group B showed more frequent abnormal findings than Group B with respect to EEG and brain imaging studies (P < 0.05). All of the abnormal findings in brain imaging studies were structural abnormalities unrelated with CwG, and patients with abnormal EEGs showed no recurrent seizure attacks and exhibited normal follow-up EEGs after discharge. CONCLUSION: CwG with complex-type seizure frequently occurred in children older than three years of age regardless of gender, and also in patients with underlying CNS diseases. Regardless of recurrent seizures or seizures lasting more than 15 minutes, patients diagnosed as CwG would receive a favorable prognosis.

3.Effects of glucosamine on Ca2+ signaling and K+ channel currents in T lymphocytes.

BACKGROUND/AIMS: Glucosamine is widely taken as a functional food, and some studies reported its anti-inflammatory effects. K+ channels and intracellular signal play important roles in the activation of immune cells such as T lymphocytes. Therefore we aimed to examine the effects of glucosamine on the cell physiological parameters. METHODS: In Jurkat-T lymphocytes, intracellular [Ca2+] ([Ca2+]i) was measured using fura-2 fluorimetry, and voltage-gated K+ current (I(Kv)) was measured using whole-cell clamp technique. Ca2+-activated K+ current (I(Kca)) was measured in HEK293 cells over expressing SK4 using inside-out patch clamp technique. RESULTS: An acute application of glucosamine (0.5 mM) affected neither the increase in [Ca2+]i induced by CD3 stimulation (anti-CD3 Ab, 5 microgram/mL) nor the I(Kv) in Jurkat-T cells. A chronic stimulation of with anti-CD3 Ab (5 microgram/mL, 24~36 hr) largely increased the amplitude of IKv. However, the combined treatment with glucosamine (0.1 mM) did not block the increase of I(Kv). The I(KCa) in SK4-overexpressing cells was slightly decreased by glucosamine (0.5 mM). CONCLUSIONS: While glucosamine had a minor inhibitory effect on SK4 K+ channels, the anti-inflammatory effects of glucosamine could not be explained by the effects on the Ca2+ signaling in T lymphocytes.

4.The change of oral volatile sulfur compounds(VSC) concentration after periodontal treatment.

Oral malodor may cause a significant social or psychological handicap to those suffering from it. Oral malodor has been correlated with the concentration of volatile sulfur compounds (VSC) produced in the oral cavity. Specific bacteria identified in the production of VSC have been reported and many of these bacteria are commonly suspected periodontal pathogens. The aim of this study was to estimate the change of the VSC concentration after periodontal treatment. Twenty subjects with probing depth (PD) > or =5mm (experimental group) and 20 subjects with PD <5mm (control group) participated. VSC concentration measurement was made with gas chromatography. VSC concentration was measured at pre-treatment, 2 weeks after scaling and 1 month after periodontal treatment(root planning and flap operation). Maximum probing depth and bleeding on probing(BOP) were also examed at pretreatment and 1 month after periodontal treatment. The conclusions were as follow: 1. In the experimental group VSC concentration and CH3SH/H2S ratio were higher than control group. (p<0.05) 2. Both VSC concentration and CH3SH/H2S ratio showed decrease after periodontal treatment. But only CH3SH/H2S ratio after 1 month periodontal treatment was statistically significantly different from pre-treatment. (p<0.05) 3. CH3SH/H2S ratio tended to be on increase according to maximum probing depth and bleeding on probing. Periodontal disease could be a factor that caused oral malodor and oral malodor could be decreased after periodontal treatment.

5.A radiographic evaluation of graft height changes after maxillary sinus augmentation and placement of dental implants.

The edentulous posterior maxilla generally provides a limited amount of bone height because of atrophy of the ridge and pneumatization of the maxillary sinus. Maxillary sinus augmentation is one of the surgical techniques for reconstruction of the severely resorbed posterior maxilla. The purpose of this study was to evaluate the survival rate of implants and the long-term changes of graft height after maxillary sinus augmentation by lateral window approach. From September 1996 to July 2004, maxillary sinus augmentation with mixed grafts of autograft, allograft, xenograft and alloplast were performed on 45 patients and 100 implants were placed. We evaluated the survival rate of implants and the changes of BL(bone length)/IL(implant length) according to time using panoramic radiographs. The survival rate of implants was 91.0% for follow-up period. The mean reduction of graft heights was 0.34mm(3.0%) for 6 months and 1.22mm(10.66%) for 3 years after augmentation. The total mean BL/IL was 1.34+/-0.21 during 5 year observation period after augmentation and decreased slightly over time. The result means that graft materials were stable above the implant apex. BL/ILs of 1 stage procedure were significantly decreased at 1-2 year, 3-4 year after augmentation and no statistically significant changes were observed in those of 2 stage procedure. The graft materials of both procedures were stable above the implant apex. No statistically significant changes of BL/IL were observed in the grafts combined with low amount of autogenous bone or without autogenous bone. The graft materials of both groups were stable above the implant apex. The results indicated that the placement of dental implants with maxillary sinus augmentation showed predictable clinical results and the grafts combined with low amount of autogenous bone or without autogenous bone had long-term resistance to resorption in maxillary sinus.

6.A Case Report of Food-Dependent Exercise-Induced Anaphylaxis in a Patient who was Sensitive to Pork.

Food-dependent exercise-induced anaphylaxis (FDEIA) is a specific variant of exercise-induced anaphylaxis that requires both vigorous physical activity and the ingestion of specific foods. In particular, it is rare occurrence for FDEIA to be associated with meat in Korea. A 15-year-old female had generalized urticaria, dyspnea, severe cough, headache, dizziness, and vomiting after singing and dancing for 1 hour and after ingesting grilled pork. Skin prick tests showed a strong positive reaction to pork, whereas the results of an oral food challenge and exercise provocation tests were negative. However, the exercise provocation test after pork ingestion showed a positive reaction manifested by generalized urticaria, cough, mild dyspnea, and a 23% decreased peak expiratory flow rate. Three allergens to pork (67 kDa, 90 kDa, and 15 kDa) reacted with the patient's serum on immunoglobulin E immunoblotting. We report a case of pork-dependent exercise-induced anaphylaxis in a patient who was sensitive to pork.

7.Clinical Implication of Microscopic Anthracotic Pigment in Mediastinal Staging of Non-Small Cell Lung Cancer by Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration.

Microscopic anthracotic pigment (MAP) is frequently observed in endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) specimen in non-small cell lung cancer, but its clinical interpretation is not well-known. The aim of this study was to evaluate the clinical implication of MAP in mediastinal staging of non-small cell lung cancer. From May 2010 to July 2011, consecutive potentially operable non-small cell lung cancer patients who underwent EBUS-TBNA for mediastinal staging were recruited. Of the total 133 patients, 102 (76.7%) were male patients. Median age was 68 yr. Total 279 mediastinal lymph nodes were sampled by EBUS-TBNA; station 4R (100, 35.8%) and station 7 (86, 30.8%) were the most common sites. Malignant lymph nodes were 100 (35.8%). MAP was observed in 61 (21.7%) lymph nodes, and among them only 3 were malignant lymph nodes (P < 0.001). The lymph nodes with MAP were smaller (9.0 vs 10.8 mm, P = 0.001) and showed low standard uptake values on FDG-PET (4.4 vs 4.7, P = 0.256). In multivariate analysis, MAP was negatively associated with malignant lymph node (adjusted OR, 0.12; 95% CI, 0.03-0.42; P < 0.001). In potentially operable non-small cell lung cancer patients, MAP in endobronchial ultrasound-guided transbronchial needle aspiration specimens is strongly associated with benign mediastinal and hilar lymph nodes.
