2.Phakomatosis Pigmentovascularis Type Ia.

Phakomatosis pigmentovascularis is characterized by the coexistence of pigmentary nevus and hemangioma. Since the term was first described by Ota et al in 1947, about 80 cases have been reported in the literature. In 1979, Hasegawa et al subclsssified the disorder into eight types : type I, nevus flammeus and nevus pigmentosus et verrucosus, type II, nevus flamme is and dermal melanocytosis, type III nevus flammeus and nev.us spilus, type IV, nevus flammeus, dermal melanocytosis and nevus spilus. Each type is subdivided into localized(a) and systemic(b) types, A 25-year-old male had brown papules on the left knee and thigh and purple patches on the left leg that had been present for approximately 7 years. He had no concon ittant systemic disease. The histopathologic findings on brown papules and purple patches are compatible with epidermal nevus and nevus flammeus respectively. We herein report a case of phakomatosis pigmentovascularis type I a.

3.Balloon Dilatation for Postoperative Stricture of Gastrointestinal Tract.

PURPOSE: To assess the effects and complications of balloon dilatation in the treatment of postoperative stricture at the gastrointestinal tract. MATERIALS AND METHODS: From February 1991 to July 1993, balloon dilatation was performed under fluoroscopic guidance on 5 patients (age:l month to 64 yrs, male:female=2:3) who previously had undergone abdominal surgery because of stomach cancer (n=2), congenital tracheoesophageal fistula (n=l), lye stricture (n=l), and colon cancer (n=l). Causes of the stricture were all benign (n=4) except for one (recurrence of malignant tumor). We dilatated the stricture site with balloon catheter (8--25 mm in diameter) 3 to 4 times per session which was repeated 1 to 5 times in each patient. Follow-up periods ranged 8 to 15 months after the treatment. RESULTS: Two out of four benign strictures were relieved at one session, and the remaining two needed repeated sessions of balloon dilatation. Symptoms did not improve in the patient with malignant stricture in~pite of balloon dilatation, and feeding jejunostomy was reginred. Partial tear of anastomosis site occurred in one patient, however, it did not require any treatment. CONCLUSION: Balloon dilatation is relatively safe and effective in the treatment of postoperative stricture at the gastrointestinal tract.

4.Musculoskeletal Disorders and Related Factors of Symphony Orchestra Players.

OBJECTIVES: Several studies have shown that performance related musculoskeletal disorders present a significant and common health problem for instrument players. This study was conducted to investigate the symptom prevalence rate of performance-related musculoskeletal disorders and risk factors on symphony orchestra players. METHODS: The symptom prevalence rate of musculoskeletal disorders and related factors including demographic factors, occupational factors, psychosocial factors, practice and performance-related behaviors of on 156 symphony orchestra players in Taegu city were surveyed with a self-administered questionnaire. RESULTS: Instrument players have performed for 14. 6 years in average and worked 4. 9 for years in average as a member of symphony orchestra. The subjects consisted of 106 string players, 48 wind players, and 8 percussion players. The symptom prevalence rate of musculoskeletal disorders according to the NIOSH surveillance criteria was 79. 6 % and operational criteria by authors was 45. 9 %. The prevalence rate of viola players was higher than violin players, and the prevalence rate of the bass players was higher than that of the cello players. In univariate analysis, job insecurity, irregular performance, stress from family problems, and performance related psychosomatic strains had a significant association with the prevalence rate of musculoskeletal disorders. In multiple logistic regression, job insecurity, irregular performance, and stress from family problems were significant variables affecting the prevalence rate of musculoskeletal disorders of instrument players. CONCLUSIONS: This study suggest that symphony orchestra players are high risk group of musculoskeletal disorders. Irregular performance schedule and psychosocial stress including job insecurity, and psychosomatic strain were important risk factors of players musculoskeletal disorders. An exercise program such as stretching before and after performance, regular performance schedule and reduction of psychosocial stress might be helpful for prevention of musculoskeletal disorders of instrument players.

5.Aplasia Cutis Congenita.

Aplasia cutie congenita is a rare congenital disease showing focal abscence of skin. In the msjority of instances it is limited ta the scalp especially on the vertex although other areas of the body may also be involved. Other congenital malformation have been reported to ccur with ACC odcasionally. Diagnosis depends on primarily clinical findings. The histologic finding virious from an absence of appendages to the involvement of varying layers of skin, occasionally extending to the bane or dura. Healed lesions often demonstrate a flattened epidermis, a proliferation of fibrobasts in a loose connective tissue stroma, newly formed capillaries, and a complete absence of adnexa stuctures. We observed a newborn infant with aplssia cutis congenita. The akir, defect was found on the posterior fontanelle. No associating cangenital anomalies were found in this case.

7.A Clinical Analysis of Pediatric Cardiopulmonary Resuscitation.

BACKGROUND: The pediatric CPR is rather uncommon practice than adult CPR in emergency room. Therefore, few interest and studies were focused in pediatric CPR. The object of this study is to make an analysis about pediatric CPR performed within hospital setting. METHOD: From January 1. 1990 to December 31. 1996, a total of 59 patients, less than 15-year-old, who received cardiopulmonary resuscitation were subject of this study. The study was done retrospective chart analysis for sex, age, weight, the place of cardiac arrest, initial EKG rhythms, endotracheal tube size, the causes of arrest, CPR time, ROSC, the amount and types of CPR drugs used during resuscitation. Wilcoxon rank sum test and chi2 test were used to compare the dose of drugs during CPR in each group. RESULTS: There were no statistical difference between ROSC and non-ROSC group in place of arrest, initial EKG rhythms, CPR time, dosage of CPR drugs. CONCLUSION: The uniform reporting guideline of pediatric advanced life support should be used for future pediatric CPR study, such as Utstein style guideline in pediatric CPR.
