3.Decision-making for Management of Acute AMominal Pain.

BACKGROUND: Acute abdominal pain is one of the most common problems in the family practice but the differential diagnosis of acute abdominal pain is difficult in first encounter. When family physicians are encountered with patients with acute abdominal pain in the ambulatory care settings, t,hey have to make a decision for management of acute abdominal pain such as admission, referral, discharge or follow-up without any definite diagnosis. If it is possible to predict the course or prognosis of acute abdominal pain by using certain data about patients, it will be helpful to make a decision for the management of acute abdominal pain. So we tested the hypothesis that acute abdominal pain with intermittent pain nature and normal simple abdomen X-ray finding is not serious and shows favorable outcome. METHODS: 126 patients with acute abdorninal pain were enrolled from Feb. to Aug. in 1995 at the emergency department of Kyungju hospital, Dongkuk University. 116 patients showed normal simple abdomen X ray finding and among them 94 patients were discharged and 21 patients were admitted. 92 patients were contacted in 1 week by phone call and they reported the outcome of their acute abdominal pain. RESULTS: Among 92 study populations, 44 patients were male and 48 patients were female. 72 patients complained intermittent abdominal pain and 21 patients complained continous abdominal pain. Frequencies of tentative diagnosis at emergency department were 45 acute gastroenteritis, 26 unknown, 14 functional gastointestinal disorders, 4 acute gastritis, 2 pelvic inflammatory diseases, and 1 ureter stone. Outcomes of patients with intermittent abdominal pain were more favorable than those with continous abdominal pain. CONCLUSIONS: If the patients with acute abdominal pain have intermittent pain nature and normal simple abdomen x ray finding, they will show favorable outcome and can be managed at ambulatory care settings.

4.Study on Determinants of Nursing Needs of Mothers with Hospitalized Children.

PURPOSE: The purpose of this study was to analyze the factors influencing nursing needs of mothers caring for children suffering from acute diseases and being treated in hospital. METHOD: The data were collected, using a nursing needs, social support, burden, and stress questionnaire. Data collection was done from July 10, to October 20, 2001 in the pediatric department of a general hospital. The data were analyzed by using descriptive statistics. RESULT: The results of this study are as follows. There were statistically significant differences in nursing needs according to differences in age, education level, marital status, religion, family income and relationship with spouse. The relationship between the nursing needs of the mother and anxiety (r=.758, p <.01), social support (r=-.659, p <.01), stress (r=.324, p <.01) were the most significant variables. The stepwise multiple regression analysis showed that anxiety, social support and stress define nursing needs at 57.5%, 4.3% and 1.1% respectively. CONCLUSION: Consideration needs to be given to anxiety, social support and stress when developing nursing intervention programs for mothers whose children are hospitalized with acute diseases.

5.The Occult Osseous Lesions on Magnetic Resonance Imaging Associated with Acute Anterior Cruciate Ligament Tears

Magnetic resonance imaging(MRI) is accepted as a useful tool for the diagnosis of injury of the knee recently. The anterior cruciate ligament tear is accompanied by the damage of the osseous and soft tissue of the knee. The occult osseous lesion is found on magnetic resonance imaging as bone density change that is not found on the simple radiograph. From Jan. 1992 to Apr. 1994, magnetic resonance imaging was performed on 44 patients with acute anterior cruciate ligament tears. MRI was done within 6 weeks of the index anterior cruciate ligament injury. The criteria of decreased signal intensity on the Tl weighted image and increased signal intensity on the T2 weighted image was applied for the incidence assessment of the occult osseous lesion. After arthroscopy with or without ACL reconstruction, follow-up simple radiograph was checked every 3 months. Occult osseous lesions were documented in 32(73%) of the 44 patients. Of the 32 patients with bony lesion, 81% had lesions of the lateral compartment. Lateral femoral condyle was involved in 15 cases(46.9%), and lateral tibial plateau in 22 cases (68.8%). Follow-up X-ray revealed no joint deperession in all of the 32 patients with the occult osseous lesion. This result suggests that there is a high correlation between the occult osseous lesion of lateral compartment of the knee on magnetic resonance imaging and anterior cruciate ligament tear. The presence of the occult osseous lesion in the patient with acute anterior cruciate ligament tear did not affect rehabilitation and weight bearing.

8.A Randomized Comparative Study of Blind versus Ultrasound Guided Glenohumeral Joint Injection of Corticosteroids for Treatment of Shoulder Stiffness.

BACKGROUND: We prospectively compared the response to blind and ultrasound-guided glenohumeral injection of corticosteroids for treatment of shoulder stiffness. METHODS: A total of 77 patients with shoulder stiffness between April 2008 and March 2012 were recruited. Patients were randomized to receive either a blind (group 1, n=39) or ultrasound-guided (group 2, n=38) glenohumeral injection of 40 mg triamcinolone. The clinical outcomes and shoulder range of motion (ROM) before injection, at 3, 6, and 12 months after injection and at the last follow-up were assessed. The same rehabilitation program was applied in both groups during the follow-up period. RESULTS: There was no significant difference in demographic data on age, sex, ROM, and symptom duration before injection between groups (p>0.05). There were no significant differences in ROM including forward flexion, external rotation at the side, external rotation at 90o abduction, and internal rotation, visual analogue scale for pain and functional outcomes including American Shoulder and Elbow Surgeons score, Simple Shoulder test between the two groups at any time point (p>0.05). CONCLUSIONS: Based on the current data, the result of ultrasound-guided glenohumeral injection was not superior to that of blind injection in the treatment of shoulder stiffness. We suggest that ultrasound-guided glenohumeral injection could be performed according to the patient's compliance and the surgeon's preference. Once familiar with the non-imaging-guided glenohumeral injection, it is an efficient and reliable method for the experienced surgeon. Ultrasound could be performed according to the surgeon's preference.

9.Psychiatric Consultation for 5 Years in a University Hospital.

OBJECTIVE: This study aimed to investigate the pattern of psychiatric consultation-liaison in a new general hospital over a 5-year period. METHOD: Retrospective chart review was conducted for psychiatric consultations performed from May 2004 to December 2008 in a new general hospital in Hwasun, Korea. RESULTS: A total of 1,852 patients who were referred to the Department of Psychiatry for consultation were included in the analysis. The main reasons for psychiatric consultation were changes in mental status(20.5%), depression(16.8%), insomnia(12.8%), and anxiety(7.9%). Psychiatric consultations were conducted with diagnoses of delirium(39.7%), depressive disorder(28.2%), adjustment disorder(7.9%), and anxiety disorder(4.1%). Patients with delirium were significantly older than were those with other psychiatric diagnoses(p<0.001). Delirium was more common in male patients than in female patients(47.1% vs. 29.9%, respectively), and depression was more common in female patients than in male patients(48.3% vs. 33.9%, respectively)(p<0.001). Delirium was more common in patients who underwent surgical operation than those who did not(p=0.010). CONCLUSION: Delirium was the most common diagnosis for psychiatric consultation followed by depression in a university hospital. Delirium was more prevalent in men than in women, while depression was more common in women.

10.Influence of Experiences of Witnessing Tobacco Advertising and Preferences of Tobacco Companies' Social Responsibility on Current and Future Smoking Intentions in Adolescents.

PURPOSE: To evaluate the influence of experiences of witnessing tobacco advertising and preferences of tobacco companies' social responsibility (CSR) on current and future smoking intentions in adolescents. METHODS: Data were collected from 700 adolescents living in Korea representing 7 metropolitan cities and 8 provinces in the method of proportional random sampling. The Gallup's data collection system was adopted using 1:1 face to face interviews. The witness of tobacco advertising was determined by 'yes', or 'no' responses to three survey items and to four survey items for preferences of tobacco CSR. For statistical analysis, chi2 test, Fisher's exact test and logistic regression were used. RESULTS: 98.4% of the adolescents had experiences of witnessing tobacco advertising. POP (OR=103.44, 95% CI: 8.22~1301.45) and magazine advertisement (OR=6.07, 95% CI: 1.34~22.58) had a significant effect on their current smoking. Also, POP (beta=.24, p<.001) and movie advertisement (beta=.42, p<.001) had a significant effect on their future smoking intentions. As for the preferences of tobacco CSR, cultural-art support activities (beta=.15, p=.025) and environmental purifying campaigns (beta=-.15, p=.034) had a significant effect on their future smoking intentions. CONCLUSION: Witness of tobacco advertising and positive responses to tobacco CSR are associated with current and future smoking intentions in adolescents.
