1.Use of separate single-tooth implant restorations to replace two or more consecutive posterior teeth: a prospective cohort study for up to 1 year.

PURPOSE: The aim of this study was to evaluate the periodontal and prosthodontic complications of multiple freestanding implants in the posterior jaws for up to 1 year of function. MATERIAL AND METHODS: Eight patients received 20 implants posterior to canines. Two or more implants were consecutively inserted to each patient. Single crowns were delivered onto the implants. Marginal bone loss, implant mobility, probing depth, and screw loosening were examined to evaluate the clinical success of such restorations for maximum 1 year of functional loading. RESULTS: All the implants performed well during the observation period. Neither periodontal nor prosthodontic complications were found except a slight porcelain chipping. While the marginal bone level was on average 0.09 mm lower around the implant after 6 months of loading, it was 0.15 mm higher after 1 year. CONCLUSION: Within the limits of this investigation, separate single-tooth implant restorations to replace consecutive missing teeth may clinically function well in the posterior jaw.

A choledochocele is a benign cyst-like herniation of the intramural segment of the distal common bile duct protruding into the duodenal lumen. Cholangiography is essential to demonstrate a choledochocele. Because choledochoceles are often associated with characteristic structual alterations of the papilla of Vater and the peripapillary area, ERCP is helpful in demonstrating a choledochocele. We present the clinical, endoscopic and radiographic (ERCP) findings in a patient with choledochocele.

4.Asian Female Hairline Surgery Using Follicular Unit Extraction.

BACKGROUND: Asian female hairline surgery is unique part among hair transplantation surgerys. Most of Asian female hairline surgery had been done by single strip harvesting technique which leave noticeable linear scar. As follicualr unit extraction is introduced1, female hairline surgery can take advantage of the Follicular Unit Extraction technique;leaving no linear donor scar. There has been a belief among surgeons who are somewhat reluctant to use follicular unit extraction for female hairline surgery that the inspection of the naturally occurring curl direction of hair, which is believed one of the critical steps for appropriate operative result in Asian female hairline surgery, is impossible in follicular unit extraction. So follicular unit extraction has to be 'plan B' in respect of Asian female hairline surgery. But author thought differently about this belief. METHODS: From December 2012 to May 2013, 78 consecutive cases of female hairline surgerys were done by follicular unit extraction. The ages of patients were 18 to 57 and mean was 34.5. The cases can be categorized in 3 groups; 1) Correction of fronttemporal recession only. 2) Whole hairline correction (including fronto-temporal recession, temporal recession and lowering of the central hairline). 3) Correction of the hairline with scar by previous aesthetic surgery just like forehead lift or forehead reduction surgery. Donor hair was harvested by follicular unit extraction and preparation of donor area was done by micro-strip shave pattern, by which no short haircut window was needed. Follow up periods were 4 months to 8 months. RESULTS: Aesthetic results of the all cases were satisfactory. There were three cases which needed small session (less than 100 hairs) for relatively less survival area (all of these areas were the anterior part of hair part) and two cases of small 'moth-eaten pattern' on donor area which need no treatment or another session for aesthetic improvement. There was no lumpy graft which can be look unnatural or literal wall of plug graft in anterior part of corrected hairline. There was no linear harvesting scar, which is in-evitable in single strip harvesting, on donor area. CONCLUSIONS: Follucualr unit extraction can be versatile technique for hair restoration surgery and also hold good for Asian female hairline surgery.

7.Endogenous candidal endophthalmitis associated with flame burn injury and advanced gastric cancer.

PURPOSE: We report a case of candidal endophthlmitis developed in a patient with advanced gastric cancer who recently injured by flame burn. He was treated successfully with intravitreous and intravenous antifungal agents. METHODS: A 40-year-old man visited with complaints of decreased binocular visual acuity 7 weeks after third degree flame burn injury to 25% of body surface. Indirect ophthalmoscope showed findings of fungal endophthalmitis and upper gastro-intestinal endoscopic examination diagnosed of fungal esophagitis and advanced gastric cancer. He was treated with amphotericin B intranvitreously and fluconazole and amphotericin B intravenously. RESULTS: His visual acuity was improved to 20/50 (OD), 20/30 (OS) following 4 weeks after treatment and intraocular inflammation was decreased. After 5 months, he recovered his visual acuity as 20/25 (OD), 20/20 (OS), and had no inflammation in anterior chamber and vitreous.

9.Cytokine Release from Organotypic Human Oral Tissue Following Exposure to Oral Care Product Chemicals

Purpose:
Measuring viability of a three-dimensional in vitro organotypic human oral tissue model has been suggested as an alternative test method to the oral mucosa irritation test of oral care products. The aim of this study was to investigate the production of two different cytokines using organotypic human oral tissue model following exposure to chemicals that are commonly used in oral care products.
Materials and Methods:
The organotypic human oral tissues were exposed to ethanol, sodium lauryl sulphate or hydrogen peroxide for 90 minutes. Following exposure, interleukin (IL)-1α and IL-8 productions were assessed and correlated with cell viability testing as well as histology of the organotypic human oral tissues.Result: High levels of IL-8 were released from organotypic human oral tissues in all of the test and control groups without any significant differences between them. In contrast, differences were found in IL-1α release between the test and control groups. Additionally, the trend of IL-1α release corresponded to the phenotypes observed in histological analysis while different trend existed between IL-1α release and cell viability.
Conclusion
The study concluded the non-specific release of IL-8 for the assessment of oral care product chemicals’ toxicity, while potential of measuring IL-1α cytokine level as the possible alternative test method.
10.Development of Postdural Puncture Headache Following Therapeutic Acupuncture Using a Long Acupuncture Needle.

Acupuncture appears to be a clinically effective treatment for acute and chronic pain. A considerable amount of research has been conducted to evaluate the role that acupuncture plays in pain suppression; however, few studies have been conducted to evaluate the side effects of the acupuncture procedure. This case report describes a suspected postdural puncture headache following acupuncture for lower back pain. Considering the high opening pressure, cerebrospinal fluid leakage, and the patient's history of acupuncture in the lower back area, our diagnosis was iatrogenic postdural puncture headache. Full relief of the headache was achieved after administration of an epidural blood patch.
