1.Tuberculin skin test in newly employed Health Care Workers.

OBJECTIVES: Health care workers(HCWs) are more likely to become infected with tuberculosis from patients in the hospital. When HCWs have an active tuberculosis infection, it is possible that they also become a source of infections to other HCWs and patients. METHODS: Tuberculin tests were performed with mantoux method using 5 units of tuberculin by infection control practitioners on Feb. 21, and Mar. 7, 1997. A total of 138 newly employed HCWs including 44interns and 94 nurses were tested. Results of skin test were interpreted by themselves according to the instructions. RESULTS: Among 138 personels, skin tests were negative in 27 personels (19.6%); 4 interns (9.1%) and 23 nurses (24.5%). There were 3 persons (2.2%) who had a history of tuberculosis which had been treated. CONCLUSION: The study showed that approximately 20% of newly employed HCWs had negative tuberculin skin test. Given the possibility of being infected with tuberculosis in the hospital, regular follow up of tuberculin skin test is warranted to protect HCWs,

3.Unusual Bladder Stones -Report of Three Cases-.

Bladder stone formation is secondarily associated with bladder outlet obstruction, such as neurogenic bladder and benign prostatic hyperplasia, and intravesical foreign body. 1. A case of unexpected giant bladder stone weighed 580gm associated with missed intravesical bullet after gun shut injury during Korean War in a 55-year-old man. 2. A case of giant bladder stone weighed 260gm associated with urinary stasis and urinary tract infection due to neurogenic bladder in a 46-year-old woman. 3. A case of multiple bladder stone, total number of 107, associated bladder outlet obstruction due to benign prostatic hyperplasia in a 63-year-old man.

4.Change of Femoral Anteversion after Experimental Dislocation of the Hip in Young Rabbit

An experimental study on the effect of the induced dislocation of the femoral head on the femoral anteversion was made in the hips of 55 rabbits, three to four weeks old. The results were as follows; l. In 49 of the 55 operated rabbits, a relative increase of anteversion was observed on the dislocated side, as compared with the nonoperated femur, the differences ranging from 3 to 55 degrees with an average of 20.0 degrees. 2. The anteversion angle of dislocated hip showed steady and gradual increase with the increase of age. 3. The anteversion increase was detected very early after operation; 3 of the 4 rabbits, belonging 4 to 7 days group after operation showed the anteversion increase. 4. The collapse of proximal femoral epiphysis was observed in 42 of the total 55 hips. The incidence of the collapse was higher in postoperative 4 weeks and more groups. 5. The color change, pale discoloration, of the articular cartilage was observed in 32 of the total 55 hips. The icnidence of pale discoloration was higher in the groups within 8 weeks after operation.

5.1 case of seminal vesicle cyst

About 20 or more cases of seminal vesicle cyst were reported up to nowadays, but CT findings are not yetdescribed. Seminal visicle cyst is often associated with ipsilateral renal agenesis, and the mechanism ofdevelopment is based on embryogenesis. A 26 year old male was confirmed as seminal vesicle cyst in S.N.U.H. inNov. 1981. CT of the seminal vesicle cyst revealed huge, globular and homogeneous mass with well-circumscribedsmooth margin between urinary bladder and rectum. Adilated tubular structure was also found lateral to the hugecystic mass. The ipsilateral kidney was not found in any level of abdomen from the diaphragmetic dome to thesymphisis pubis.

8.Perimpullary Carcinoma: A Study on Prognostic Factors Influencing long Term Survival After Pancreaticoduodenectomy.

From November 1987 to December 1995, 76 patients with periampullary cancer underwent resectional surgery in the department of surgery, Kosin medical college. The object of this study was to assess the prognostic factors of the periampullary cancer after pancreaticoduodenectomy. The postoperative mortality rate was 11.8%. Significant morbidity occurred in 39.5% of the patients, early reoperation was required in 9.2% of these patients, and the mean hospital stay was 21.7 days. Univariate analysis on all patients revealed that the survival rate was significantly related to the size of the tumor (> or =3.0 cm 13%, <3.0 cm 41.4%; p<0.05), lymph node status (invasion 8.7%, no invasion 31.8%; p<0.05) and the age of the patient (>50 20.5%, < or =50 34.5%; p<0.05). The site was less significant(distal CBD 27%, ampulla of Vater 16.7%, pancreas 9%, duodenum 2%; p>0.05). These results indicated that patients with a tumor size lesser than 3cm, without lymph node involvement, and under the age of 50 had a long term survival rate.

9.A Case of Rectal Cancer in 12 year Old Boy.

Children's rectal cancer is a very rare condition and its reported incidence is below 0.5%. The prognosis is very poor due to the advanced stage at diagnosis and a higher malignant potential. Recently some authors treated rectal cancer of children with surgery and they have had a better prognosis. We experienced a case of rectal cancer in a 12 year old boy treated with surgery and chemotherapy and reviewed related literature.

10.Change of Serum K+ Concentration after Injection of Succinylcholine in the Electric Burn Patient .

Transient hyperkalemia is well known to occur in man following intravenous administration of succinylcholine chloride. We studied the change of the serum potassium concentration after injection of succinylcholine in nonburn patients and electricburn patients, physical status 1 or 2 adopted by the American Society of Anesthesiologists. We compared two groups: Non-burn patient group(Group 1) as control, Electric burn patient group (Group 2) as experimental. The following results were obtained: 1) In non-burn patient group(Group 1) serum potassium concentration was slightly decreased in the 1st, 2nd, 3rd, 4th, 5th, 6th and 10th minute after succinylcholine injection compared with control, but nostatistical significance was noticed in this group. 2) In electric burn patient group (Group 2) serum potassium concentration was maximally increased compared with the controls. Statistical significance was noticed at the 2nd and 3rd minute after succinylcholine injection in this group.
