1.Prolapse of Bladder through Vesico-Vaginal Fistula.

Prolapse of bladder through vesico-vaginal fistula is quite a rare urological problem. A 38-year-old woman was admitted to the Woo Sok University Hospital with a baby head sized round mass attached to the vagina. Patient had a prolapse of uterus after having her second child birth about 10 years ago and cauterization with corrosives for the treatment of prolapse of uterus was undergone for two times. About one year prior to the admission, the prolapse of uterus recurred and cauterization with corrosives was tried again, resulting vesico-vaginal fistula through which the bladder was everted and prolapsed. Under the general anesthesia, the everted and prolapsed bladder was reduced manually in normal position and vesico-vaginal fistulectomy was performed with good result.

2.A Modified Cecil's Operation for Urethral Stricture.

3.Thrombospondin-1 and -2 Expressions in Hepatocellular Carcinomas: an Association with Tumor Angiogenesis and p53 Overexpression.

Background : It has been suggested that thrombospondin (TSP) is a p53-dependent negative regulator of tumor angiogenesis. TSP expression and localization in hepatocellular carcinomas (HCCs) and its association with overexpression of p53 protein were investigated. Methods : TSP-1 and -2 expressions were examined in 40 HCC specimens by immunohistochemical staining and in 4 HCC cell lines by Western blotting. In addition, p53 protein expression and microvessel density (MVD) were correlated with the TSP expression. Results : Strong immu- nopositivity for TSP-1 was observed in fibroblasts, vascular endothelial cells, and some vas- cular smooth muscle cells of the stroma in 18 cases (45%), and in tumor cells in 3 cases (7.5%) of 40 cases of HCC. Immunoreactivity for TSP-2 was observed in only the sinusoidal lining cells of the tumor in 15 cases (46%), and in tumor cells in 2 cases (6%) of 32 cases of HCC. TSP-1 expression was inversely correlated with MVD (p=0.028), but TSP-2 expression did not show any correlation with MVD. Although p53 was overexpressed in 17 cases, there was no significant correlation between TSP and p53 expressions. None of the HCC cell lines expressed TSP-1 or -2. Conclusions : These findings indicate that TSP-1 is mainly derived from nonparenchymal cells, and may decrease tumor angiogenesis in HCC.

4.Relapse Rates of Ulcerative Colitis in Remission and Factors Related to Relapse.

BACKGROUND/AIMS: Ulcerative colitis (UC) is a chronic inflammatory bowel disease with heterogeneous clinical features. Data on the disease course and prognosis of UC patients who have been regularly treated are lacking. We aimed to investigate relapse rates of UC in remission and factors related to relapse. METHODS: We retrospectively analyzed clinical courses of 84 patients (43 males, median age 43 years, ranged 20-73 years) diagnosed as UC at Ajou University Hospital between January 1997 and December 2005 based on clinical, endoscopic and pathologic findings, and who were regularly followed for at least one year after the remission. RESULTS: Study subjects consisted of 32 proctitis (38%), 21 left-sided colitis (25%), and 31 subtotal or total colitis (37%). Of 84 patients, relapse was observed in 52 patients (62%) during the follow-up period (ranged 1-9 years). The relapse rate was 24%, 41%, 51%, 65%, 71%, and 79% at 1 year, 2 years, 3 years, 4 years, 5 years and 6 years, respectively. Among sex, age, hemoglobin, ESR, and the extent of disease on admission, decrease of hemoglobin level was the only independent factor related to relapse (odds ratio=2.67, 95% CI (1.32-5.42), p<0.01). CONCLUSIONS: In Korea, relapse of UC in remission is not rare. Decrease of hemoglobin level is an independent risk factor related to its relapse, while the extent of disease is not.

5.A Case Report of Agenesis of Gallbladder which was Diagnosed after an Emergent operation Due to Ostructive Jundice with Impacted CBD Stones.

A case of Agenesis of gallbladder in 73-year old female patient who had an emergent operation due to obstructive jaundice with impacted CBD stones is reported. The patient suffered from icteric sclera and abdominal pain which waxed and waned for three days. Impacted CBD stones and an agenesis of the gallbladder was suggested by a computed tomogram of the abdomen. In the operating field, multiple adhesions around the biliary tree was noticed, but we could not find the gallbladder and cystic duct. She underwent choledocholithotomy, T-tube choledochostomy, operative cholangiography, and drainage. Operative cholangiogaphy showed no remained stones, and there was no structures suggesting gallbladder and cystic duct. The authors described the case with a recent review from the literature.

7.Patterns of Iron Utilization According to the Growth of Staphylococcus aureus.

To elucidate iron utilization patterns of Staphylococcus aureus according to the growth, we checked the residual iron concentration and the production of siderophores at the indicated times while culturing S. aureus ATCC 6538 and 25923 strains in brain heart infusion broth. By using streptonigrin susceptibility test and investigating growth curves in three culture media of which iron concentration is 0.2, 20, 45 uM, respectively, we found out that iron metabolism of 6538 strain was more active than that of 25923 strain. In point of tendency of iron consumption, 6538 strain steeply consumed iron just before the onset of stationary phase, but 25923 strain did gradually iron throughout the growth phase. Nevertheless, total amount of iron consumed by each strain during the growth was almost no difference between the strains. CAS diffusion assay in detecting siderophores showed that siderophore production followed iron consumption. These results suggest that the siderophores play significant role in iron utilization in vitro.

8.Effects of Alcohol on Neurocognitive Function, Psychomotor Performance and Subjective Response in Koreans with Different ALDH2 Genotypes.

OBJECTIVE: The purpose of this study was to evaluated the effects of alcohol on neurocognitive function, psychomotor performance and subjective response in healthy Korean adults with different ALDH2 genotypes. METHOD: A total of 24 males, half with active ALDH2*1/2*1 and the other with inactive ALDH2*1/2*1, was selected through genotyping using restriction fragment length polymorphism. In a double-blind, placebo-controlled cross-over design, each subject consumed 0.5g/kg dose of alcohol, given as a mixture of 40% vodka and orange juice, and placebo(orange juice) on two separate occasions on an average of weekly intervals. The blood alcohol concentrations(BACs) were measured using a breath analyzer at baseline and at 30, 60 minutes after drinking. P300s were measured at baseline and at 30 minutes after alcohol and placebo intake. Vital signs and psychomotor performance[Critical Flicker Fusion Threshold(CFFT), Choice Reaction Time(CRT), Digit Symbol Substitution(DSS)] were measured at baseline and at 60 minutes after alcohol and placebo intake. Subjective responses were measured at the end of the study. The statistical analysis focused on whether there were any differences between groups with different ALDH2 genotypes. RESULTS: The major results are as follows. 1) BACs in inactive group were overall equivalent to those in the active group. Only in terms of time, BACs were significantly higher overall at 30 minutes than at 60 minutes after alcohol intake. 2) Pulse rates were significantly increased after alcohol intake compared with placebo, and the increase was greater in the inactive than in the active group. 3) P300 latencies in leads Fz(frontal), Cz(cental) and Pz(parietal) were significantly increased after alcohol intake compared to placebo, and the increase was greater in the inactive than in the active group. P300 amplitudes in leads Cz and Pz were significantly decreased overall after alcohol intake compared to placebo. 4) Compared with placebo, alcohol produced significant effect on the psychomotor performance : impairment in the inactive group, improvement in the active group. 5) Compared with placebo, alcohol significantly induced a negative or an intense effect on the subjective responses in the inactive group, but little negative and even a somewhat positive effect in the active group. CONCLUSIONS: These results suggest that ALDH isozyme variance might be an important factor to determine the effects of acute dose of alcohol on the various psychobehavioural functions and also to determine the alcohol use pattern and to predict the future development of alcohol overuse and/or abuse.

9.Mucinous Adenocarcinoma of the Prostate: A Case Report.

The diagnosis of mucinous adenocarcinoma of the prostate (MAP) requires that at least 25% of the tumor consists of pools of extracellular mucin. According to the above criteria, MAP is rare and about 70 cases have been reported in literature. Here we report a case of MAP arising in a 70-year-old man. In the present case, the proportion of the mucinous component was up to 95% of the total tumor volume, and Gleason's score is 8 (4+4). The tumor cells were prostatic specific antigen (PSA)-immunoreactive in mucinous and nonmucinous components. Despite the mucin pools, the tumor cells themselves failed to stain with any of the mucin stains.

10.Remote Postoperative Epidural Hematoma after Brain Tumor Surgery.

A postoperative epidural hematoma (EDH) is a serious and embarrassing complication, which usually occurs at the site of operation after intracranial surgery. However, remote EDH is relatively rare. We report three cases of remote EDH after brain tumor surgery. All three cases seemed to have different causes of remote postoperative EDH; however, all patients were managed promptly and showed excellent outcomes. Although the exact mechanism of remote postoperative EDH is unknown, surgeons should be cautious of the speed of lowering intracranial pressure and implement basic procedures to prevent this hazardous complication of brain tumor surgery.
