1.Delayed Cutaneous Hypersensitivity Reactions in a General Adult Population of Korea.

We messured delayed cutaneous hypersensitivity reactions by Multitest CMI Kit. Total 209 normal adults ranging in age from 17 to 55 year(male 106, female 103) were tested. The results were obtained as follows ; 1. Sum of average diameter of positive antigens wss 20.7mm in male and 15.6mm in female. 2. Number of positive antigens was 3.9 in male and 3.4 in female. 3. Rate of anergy was 1.4%(0.9% in male, 1.9% in female). 4. The rate of subjects below warning score(below 10mm in male, below 5mm in female) was 2.8% in male and 5.8% in female. 5. Antigen with the highest response was tuberculin in both sex(male 95.3%, female 89.3%) and antigen with the lowest response was trichophyton(22.5%). 6. Aversge score of positive response of every antigens was highest in tuberculin at both sex(male 2.8mm, female 2,6mm) 7. Average score of total antigens with positive response was 5.5mm in male and 4.6mm in female. 8. The rate of false positive reaction to 70% W/V glycerine was 1.4% totally.

2.A Case of Elastosis Perforans Serpiginosa Associated with Pseudoxanthoma Elasticum.

we report a case of elastosis perforans serpiginosa associated with pseudoxanthoma e1asticurn on t:he neck of 27-year-old male patient. The lesions showecl erythematous scaly hyperkeratotic papules forming serpiginous pattern on yellowish elevated plaques. A skin biopsy specimen from the neck lesion revealed dense accurmulations of coarse elastic fibers partly cngulfed by epithelium in the papillary dcrrnis and increase cif short, irregulary clumped and degenerated clastic fibers in the middermis. On funduscopic exarnination, angioid streak is noted around the optic disk.

3.Observations of Arrhythmias by 24 Hour Ambulatory ECG Monitoring in Early Convalescent Phase of Acute Myocardial Infarction.

In order to observe the arrhythmia which occured in early convalescent phase of acute myocardial infarction, a 24 hour ambulartory ECG monitiring was performed in 15 patients who is admitted at dept of Int Medicine, Ewha Womans University hospital from Aug. 1985 to Sept. 1986. during 7.5+/-3.4 days after admission. In all cases one or more of the following arrhythmias were observed;atrial premature beat, ventricular premature beat(VPB), transient paroxysmal ventricular thchycardia, atrial fibrillation, sinus arrest and junctional beat. Of 15 cases, 11(77.3%) had VPB's:Simple VPB's in 4 cases(26.7%) and complex VPB's(6.52+/-2.52)(P<0.05). But there were no difference of premature and abberancy index between complex VPB's and simple VPB's(P>0.1). In addition, we compaired the arrhythmias in CCU with those of early convalescent phase of acute myocardial infarction. But there was no correlation between them.

4.A Case of Mycosis Fungoides.

We report a case of 68-year-old male patient who was confirmed as d'emblee type of mycosis fungoides. The skin lesions showed sharply rnargined multiple reddish brown coIarcd papules, ndules, and tumors on the groin, thign, abdomen, and neck. The microscopic findings showed the epidermotropism and pautrier's micruabscess in the epidermis, and the infiltration of atypical cells in the dermis. The electron micrascopic findings showed irregular contour of the nuclear mernbrane within atypical lymphocytes. Thc lesions were gradually diminished by superficial radiation therapy.

5.The Longitudinal Mediating Effects of Perceived Parental Neglect on Changes in Korean Adolescents’ Life Satisfaction by Gender

Purpose:
The life satisfaction of Korean adolescents tends to decrease beginning in middle school, and this decrease has a negative effect on life satisfaction in adulthood. Gender and perceptions of parental neglect are important variables influencing the life satisfaction of adolescents in Korea. This study examined the longitudinal mediating effects of perceived parental neglect on the life satisfaction of male and female Korean adolescents to assess changes to the relationships between key variables over time.
Methods:
The present study analyzed data from 2,215 adolescents from the Korean Children and Youth Panel Survey's fourth-year elementary school panel spanning their first year of middle school (2013) to the first year of high school (2016). A latent growth model was applied to Korean national data.
Results:
The perceived parental neglect and life satisfaction intercepts differed by gender, and the intercept of perceived parental neglect influenced the life satisfaction intercept. The slope of perceived parental neglect, but not the life satisfaction slope, differed by gender, and the slope of perceived parental neglect influenced the life satisfaction slope. The Sobel method showed that the intercept of perceived parental neglect mediated gender differences in the life satisfaction intercept, and the slope of perceived parental neglect significantly influenced the gender difference in the life satisfaction slope.
Conclusion
Efforts to improve adolescent life satisfaction should be differentiated by gender regarding perceptions of parental neglect.

7.Analysis of Multiorgan Failure in Brain-Dead Patients.

BACKGROUND: Brain death is irreversible coma due to injury of brain hemisphere and brain stem regardless of any treatment. In brain-dead patients, diabetes insipidus, hypothermia, acute respiratory failure, and multiorgan failure occur due to brain stem compression injury. The primary goal of organ donor management is maintenance of optimal physiologic environment for organs prior to donation. This study is performed for suggesting the guideline of the prediction and management of multiorgan failure in the brain-dead patient. METHODS: We analyzed 16 brain-dead patients waiting for organ donation in the intensive care unit. The causes of brain death among the donors consisted of closed head injury in 8 patients, subarachnoid hemorrhage in 4, drowning in 1, aplastic anemia in 1, asthmatic attack in 1 and falling-down injury in 1. PaO2/FIO2 (arterial oxygen tension/fractional inspired O2 concentration) was analyzed to demonstrate the progress of respiratory failure. Body temperature, vital signs, urine output, serum osmolarity, urine osmolarity, serum K+, serum Na+, AST(aspartate aminotransferase), ALT(alanine aminotransferase), total bilirubin, BUN(blood urea nitrogen) and creatinine were also analyzed in all patients. RESULTS: Diabetes insipidus were found in 15 patients, hypothermia in 8, renal dysfunction in 2, hepatic dysfunction in 0, and acute respiratory failure in 2 at the time of arrival to intensive care unit. Diabetes insipidus was found in 16patients, hypothermia in 0, renal dysfunction in 0, hepatic dysfunction in 0, and acute respiratory failure in 9 at 16 hours after arrival to intensive care unit. CONCLUSIONS: We concluded that hepatic and renal functions were well preserved for long time after brain death and brain-dead patients rapidly progressed to acute respiratory failure. It can be suggested that organ procurement should be performed as soon as possible after brain death was confirmed for successeful organ transplantation.

9.Lethal pulmonary thromboembolism misdiagnosed as pneumonia: A case report.

Although pulmonary thromboembolism (PTE) is not rare, unfortunately for anesthesiologists, the signs and symptoms of PTE are unreliable and nonspecific. PTE is a potentially lethal condition without an accurate diagnosis and prompt treatment. We report a case of PTE misdiagnosed as simple pneumonia. A 60-year-old female, not receiving prophylactic anticoagulant therapy, underwent elective surgery for a left proximal tibial fracture. During induction of general anesthesia, a temporary bronchospasm occurred and subsided quickly. Because vital signs were stable and patient monitoring was normal, we did not identify a serious hypercarbic condition. After surgery, a massive hemoptysis occurred and the patient expired due to cardiopulmonary collapse. According to autopsy, the cause of death was a PTE originating in deep vein thrombosis.

10.Effects of Extracorporeal Shockwave Therapy in Chronic Stroke Patients With Knee Osteoarthritis: A Pilot Study.

OBJECTIVE: To evaluate the effects of extracorporeal shockwave therapy (ESWT) on pain, function, and ultrasonographic features of chronic stroke patients with knee osteoarthritis (OA). METHODS: A total of 18 chronic stroke patients (33 knee joints) with unilateral or bilateral knee OA (Kellgren-Lawrence grade ≥1) were enrolled in this study. The patients were randomly allocated to an experimental group receiving ESWT (n=9) or a control group receiving sham ESWT (n=9). For the ESWT group, patients received 1,000 pulses weekly for 3 weeks, totaling to an energy dose of 0.05 mJ/mm² on the proximal medial tibia of the affected knee. The assessments were performed before the treatment, immediately after the first treatment, and 1 week after the last treatment using the following: the visual analog scale (VAS) for pain; patient perception of the clinical severity of OA; the Korean version of Modified Barthel Index (ambulation and chair/bed transfer); the Functional Independence Measure scale (FIM; bed/chair/wheelchair transfer, toilet transfer, walking, and stairs); and ultrasonographic features (articular cartilage thickness, Doppler activity, and joint effusion height). RESULTS: The experimental group showed a significant improvement in VAS score (4.50±1.87 to 2.71±1.38) and patient perception of the clinical severity of OA (1.87±0.83 to 2.75±0.46). The bed/chair/wheelchair transfer components of the FIM score also improved significantly (4.12±1.55 to 4.62±1.30). In terms of the ultrasonographic features, increased Doppler activity was observed in the medial knee in the experimental group immediately following ESWT. CONCLUSION: It is suggested that ESWT may reduce pain and improve function in chronic stroke patients with OA, and may increase vascular activity at the target site.
