1.Cutaneous Metastasis from Prostatic Carcinoma.

Prostatic carcinoma is one of the most common malignancies in males of western countries but it is a rare malignancy in Korea. If it occurs, it commonly metastasizes to other organs. In-spite of its high incidence of distant metastasis, the cutaneous metastasis of the prostatic carcinoma is very rare and seems to have a poor prognosis. A 67-year-old man presented multiple nontender nodules on the skin of the right thigh and had episodes of multiple metastases from prostatic carcinoma to the lungs and bones. Histopathologic findings of the skin showed atypical cells infiltration at the dermis. Immunohistochemical staining for prostate specific antigen showed a positive reaction, therefore, we reached the diagnosis of cutaneous metastasis from prostatic carcinoma.

2.The Analysis of the Normal Rauges of the Hip Motion in Busan Newborns and Infants

Normal ranges of hip motion ia newborns and infants is very important. The newborns and infants, it was very difficult to diffe nciate the limitation of hip motion affected by diseases from the normal ranges of hip motion in newborns and infants whose hips are alwsys flexed, abducted and externally rotated due to intrauterine position. Author measured he normal ranges of hip motion with monthly interval analysis in 400 Busan newborns and infants to note changes of ranges of their hip motion. The result obtained were as follows. 1. The ranges of flexion contracture of hip were varying from 10°–45° (25°±7.93°) in 0–1 month age group to 0°–1 month age group to 0°–20° (9°±4.93°) in 11–12 months age group with average 0–45° (16±7.88°), which menifested gradual decrease in growth. 2. The ranges of abduction were 50°–110° (80°±8.75°), internal rotation, 10°–45° (24±6.46), external rotation, 30°–75°(53°±10.3°) and menifested no changes in growth.

3.Pseudoxanthoma Elasticum.

Two cases of pseudoxanthoma elasticum associated with angioid streaks in both eyes were presented and the literatures were reviewed. The affected sisters were 15 and 18 years female among the five siblings. They had pin head sized yellowish maculopapular eruptions on the sides of the neck and axillae, and also visual distubance of the both eyes. They seemed to be autosomal recessive inberitance in their family background. There were no other complications.

5.Studies on Serum Lipids Level in Normal Korean Adults and Vegetarians.

The serum lipids level of healthy Koreans, including 306 normal adults, 15 overweight persons, 25 faculty members and 40 vegetarians (monks) were studied in 1973. The following results were obtained. 1. The mean serum liplids values of normal Korean adults were 188.7+/-33.7mg% for total cholesterol, 176.9+/-47.0mg% for phospholipid and 78.0+/-28.9mg% for triglyceride. 2. It was observed that the serum level of normal Koreans were influenced only by age and were not influenced statistically by sex, weight and socioeconomic state. 3. The mean serum lipids level of Korean vegetarians (monks) were 188.0+/-44.0mg% for total cholesterol, 158.5+/-35.6mg% for phospholipid and 64.8+/-21.4mg% for triglyceride and not influenced by sex and duration of altered eating habit. 4. The serum lipids level of Korean vegetarian (monks) were significantly low in phospholipid and triglyceride but not in cholesterol. 5. No significant difference in serum total cholesterol level between normal and vegetarian were observed all the role of diet of general population were discussed.

6.Clinical Study on Congestive Heart Failure.

Our experience with 150 patients with congestive heart failure has been reviewed with particular reference to physical and laboratory findings on admission, precipitating factors and diagnostic clues. Congestive heart failure has been defined as that heart failure due to myocardial failure (primary or secondary). (1) Congestive heart failure was found in 3.65% of total medical admissions and in 24.0% of total cardiovascular patients. There was no difference between two sexes and incidence was peak in the 6th decade. (2) Rheumatic heart disease was the most common cause of congestive heart failure (41.0%). Hypertensive and/or atherosclerotic heart disease (36.5%), cor pulmonary (6.6%), postpartum heart failure (5.3%), pericardial disease (3.6%), thyrotoxic heart disease (2.0%) and congenital heart disease (0.6%) were the etiologies of the remainder. In male hypertensive and/or atherosclerotic heart disease was most common cause of congestive heart failure. (3) The precipitating factors of congestive heart failure were found in 31% and infection, pregnancy, discontinuation of digitalis and physical exertion were major causes. (4) Dyspnea, cervical vein distension, pulmonary rale, hepatomegaly and cardiac murmur were important symptoms and signs on admission. (5) Peripheral venous pressure was elevated in 883.2% and circulation time was prolonged in 90.9%. (6) Leucocytosis (39.8%) albuminuria (60.2%), elevated GOT (serum glntamin oxaloacetic transaminase) (12.0%) and azotemia (50%) were observed. (7) Chest X-ray on admission revealed increased vascularity including pulmonary edema (88.8%), cardiomegaly (83.3%) and hydrothorax (27.1%). (8) Electrocardiographic findings was abnormal in 99.4% and left ventricular hypertnophy (45.7%), atrial fibrillation (27.3%), Right ventricular hypertrophy and low voltage were frequently observed.

7.The Advantages of Computed Tomogram in Diagnosis of Pelvic Fracture

Computed tomography is helpful in delineating the extent and configuration of pelvic fracture. These injuries are frequently complex, and the precise pathologic anatomy is not easily demonstrated by conventional radiographs. In some case CT will reveal a fracture which may not be seen on the radiograph. In addition, the extent of soft tissue damage and joint involvement is precisely demonstrated with CT. The twenty-three patients admitted to our Inchon Christian Hospital with pelvic fracture with or without hip dislocation from October 1981 to October 1984. We could assess the applicability of C-T scan in pelvic fracture and the following conclusions were obtained. 1. The axial plane of CT was shown to be the most suitable for evaluation of pelvic fracture. 2. C-T could detect the pattern of hip fracture including degree of fracture fragment displacement, and rotation, hip joint stability, intra-articular osseous fragment and interposed soft tissues in hip joint. 2. C-T permitted better evaluation of associated injuries in soft tissue and viscera on the pelvic and retroperitoneal cavity, and also gave information about other associate fractures. 4. C-T was more sensitive than plain radiography in detecting fracture involving the sacrum, quadrilateral surface, acetabular roof, and posterior acetabular hip.

9.Treatment of Tibial Pilon Fracture Applied by Biologic Principle.

Fractures significantly involving the weight-bearing articular surface and the overlying metaphysis of the distal tibia have deserved notoriety as a severe and challenging subject of ankle injuries. It was not until 1963 that the AO group developed principles for open reduction and internal fixation of pilon fracture. But recently, limited internal fixation of the joint surface combined with external fixation may avoid the soft tissue complications associated with formal open reduction and internal fixation. We have treated 22 cases of tibial pilon fracture with biologic principle. The mean follow-up period was two years ranging from one to three years. According to the Riiedi and Allgower classification, three cases were type I, five cases were type II, and fourteen cases were type III Biologic treatment includes combination of internal and external fixation, external fixation alone, or plating was done with careful soft tissue dissection, limited stripping of fracture fragments, and indirect reduction technique. Radiographic results were assessed by Burwell and Charnley criteria. Three cases of type I, three cases of type II and seven cases of type III pilon fractures obtained good anatomic radiographic results. We also assessed the functional results by Mast and Teipner criteria. Three cases of type I, three cases of type II, six cases of type III pilon fractures obtained good functional outcome. We believed that combination of internal and external fixation provides the patient who has a markedly displaced pilon fracture and diaphyseal comminution with the best chance of good clinical result and minimal risks. As such, we believe biologic treatment is excellent for these injuries.
