1.Osteochondritis of the Both Patella: One Case Report

Osteochondritis usually occurs in the epiphyseal center of children. Common sites of involvement of osteochondritis are the femoral head, tarsal navicular, talus, lunate, but the patellar involvement is rare. The exact cause of osteochondritis is unknown but it is believed to be traumatic or nontraumatic (idiopathic) in origin. This disease was first described by Kohler in 1908 and characterized by radiographic finding of increased density in the patella We have experienced here one case which is believed to be osteochondritis of the patella.

2.CT Findings of Intrathoricic Neoplasm Associated with Hypertrophic Osteoarthropathy.

PURPOSE: Hypertrophic osteoarthropathy(HOA) is a clinical syndrome consisting of clubbing, periostitis and synovitis. Most frequent causes of hypertrophic osteoarthropathy are intrathoracic neoplasms, among which the bronchogenic carcinoma ranks the highest. But computed tomographic evaluation of intrathoracic neoplasm associated with HOA has been seldom reported. The purpose of this study is to evaluate CT findings of intrathoracic neoplasm associated with HOA, and to infer possible mechanism. MATERIALS AND METHODS: Seven cases of intrathoracic neoplasm associated with HOA were included in our study. Diagnoses of HOA were made by Tc99m bone scintigraphy or plain radiography. The findings of chest CT scans were reviewed retrospectively, with main interests on their size, location and internal characteristics, ect. RESULTS: Seven cases of intrathoracic neoplasm consisted of five bronchogenic carcinomas and two thymic tumors. The size of intrathoracic tumors were relativelY, large ranging from 6cm to 13cm(average 8.0cm). All thoracic neoplasms showed wide pleural contact, and one of them invaded thoracic wall. The range of length of pleural contact was 5-18cm(average 9.9cm). All of seven patients had internal necrosis, and one of them showed cavitation in thoracic mass. CONCLUSION: lntrathoracic neoplasms associated with HOA had a tendency to be large, to contain internal necrosis, and to widely abut the thoracic pleura.

5.Two cases report of bronchial carcinoid tumors.

Bronchial carcinoid tumor was a low grade malignant and it was regarded as predictable clinical course and good survivality after surgical resection. But despite of its low grade malignant potentiality, bronchial carcinoid tumor was clearly capable of metastasizing and causing death. We present 2 cases of bronchial carcinoid tumors. One of them was typical carcinoid tumor in 44 year-old female and another was atypical carcinoid tumor in 53 year-old male patient. Currative therapeutic procedure was performed by lobectomy and wedge resection.

6.Cognitive Impairment and Peripheral Neuropathy by Mixed Organic Solvents in Spray Painters Working in a Shipbuilding Industry.

BACKGROUND: Exposure to organic solvents becomes a common problem to workers of heavy industries in Korea. A number of volatile organic solvents which are frequently used in painting can cause various derangements of the nervous system, especially cognitive impairments and peripheral neuropathy. METHODS: This study was carried out on 190 workers as a control group who had never been exposed to organic solvents and on 674 spray painters with long-term exposure to organic solvent mixtures. The major components of organic solvent mixtures were determined. All subjects underwent neurological examination as well as routine physical check-up after completing questionnaires on general, musculoskeletal, neuropsychiatric and neurological systems. Subjects with abnormal findings on neurological examination related with the exposure of organic solvent mixtures took further neuropsychological and neurophysiological tests. RESULTS: The prevalence rates of cognitive impairments and of peripheral neuropathy in the exposed group were significantly higher than the control group (9.5% vs 2.1% and 2.1% vs 0%, respectively). High exposure group (more than 2.64 of cumulative exposure index, CEI) showed also higher prevalence of cognitive impairments and of peripheral neuropathy than low exposure group (cognitive impairments; 12.3% vs 6.4%, peripheral neuropathy; 2.3% vs 1.8%). Most common cognitive impairments were attentional deficit, and abnormal sense on the extremities or face was most common symptom of the peripheral nervous system. CONCLUSION: This study is the first large-scale, case-control study extensively evaluating cognitive impairments and peripheral neuropathy related with volatile organic solvent mixtures in spray painters working in a shipbuilding industry in Korea. The results show that the prevalence rates of cognitive impairments and of peripheral neuropathy are relatively low, but clearly related with the exposure extent of organic solvents.

7.Relationship of Renal Echogenicity with Renal Pathology and Function.

PURPOSE: Renal ultrasonography has been widely used in children with renal disease. However, the relationship of renal echogenicity with renal pathology and function in children is not well known. METHODS: Ultrasound examination was performed in 75 patients undergoing renal biopsy for suspected renal disease in Konkuk University Medical Center from August 2005 to November 2015. We compared renal echogenicity to pathologic findings and renal function. Renal echogenicity was scored as 0 to 2 by comparing adjacent liver echogenicity. Three histologic characteristics were evaluated: glomerular changes, interstitial infiltration or fibrosis, and tubular atrophy. These were graded as 0 to 3, according to increasing severity. Laboratory results included urine albumin excretion and estimated glomerular filtration rate (eGFR). RESULTS: Among pathologic findings, renal echogenicity revealed a positive correlation with interstitial infiltration or fibrosis (r=0.259, P=0.025), and with tubular atrophy (r=0.268, P=0.02). Renal echogenicity and glomerular changes were not correlated. Renal echogenicity showed a positive correlation with microalbuminuria (r=0.283, P=0.014), but a negative correlation with eGFR (r=-0.352, P=0.002). CONCLUSION: Increased renal echogenicity suggested severe interstitial infiltration or fibrosis and tubular atrophy among the pathologic findings. Moreover, increased echogenicity is correlated with increased urine albumin excretion and decreased eGFR. Echogenicity on ultrasonography is useful for determining the status of renal pathology and function.

9.Effect of fast track on prognosis in patients with common bile duct obstruction with cholangitis in emergency department

Objective:
Biliary decompression through bile drainage is a key treatment for common bile duct obstruction with cholangitis. However, the effectiveness of early interventions has not been studied sufficiently in Korea. This study investigated the effectiveness of fast-track biliary decompression.
Methods:
A group of patients diagnosed with common bile duct obstruction with cholangitis between January 1, 2014, and December 31, 2019, was reviewed retrospectively. We divided them into two groups: before and after the implementation of fast-track biliary decompression. The following items were analyzed in the two groups: time to intervention, number of hospital days, length of stay in the emergency department, and intensive care unit (ICU) admission.
Results:
Between January 1, 2014, and December 31, 2019, 418 patients were admitted for common bile duct obstruction, and a total of 369 patients were included in this study. Of these, 168 patients visited the hospital prior to implementation of the treatment, and 201 patients visited after implementation. The time to intervention was 6.1 (4.2-11.0) hours in the fast-track group, which was about 9 hours shorter than the other group (P<0.001). There was no statistical difference in the number of hospital days, emergency department length of stay, and ICU admissions (P=0.535, P=0.034, P=0.322).
Conclusion
The time to intervention was shortened significantly in the fast-track group. However, we did not observe a significant improvement in patient prognosis. It may be possible that the procedure time may need to be shortened for a better prognosis. This should be investigated in future studies.
10.A case of angiomyolipoma of the liver.

Angiomyolipoma occurs most commonly in the kidneys; the liver is the second most frequent site of involvement. Hepatic angiomyolipoma is a rare, benign, mesenchymal neoplasm, composed mainly of blood vessels, smooth muscle cells, fat, and myelocomponents. Radiologic findings are non-specific because the various elements of these neoplasms vary in their proportion and distribution within the tumor. Thus, data obtained by imaging technologies such as computed tomography, ultrasonography, or magnetic resonance imaging tend to be merely suggestive; definitive diagnosis usually requires histologic confirmation. We report here a case of angiomyolipoma in an incidental tumor of the liver of a 53-year-old female. Tissue was removed from the tumor by ultrasonography-guided gun biopsy and subjected to immunohistochemical analysis. Data showed that tumor cells were positive for HMB-45 and SMA, but negative for cytokeratin, anti-hepatocyte antigen, and alpha-fetoprotein. The patient did not receive any treatment and is being followed up.
