1.Intramedullary Nailing in Osteogenesis Imperfecta

Osteogenesis imperfecta is a heterogenous group of inherited disorder, which has abnormalities in the synthesis of collagen. Among variable clinical manifestations, orthopaedic clinical features are laxity of ligament and deformities arising from frequent fractures or angulation due to skeletal fragility. We had managed 4 cases of osteogensis imperfecta, which were belonged to type IV-B in two cases, type I-A in one, and type I-B in remained one by Sillence classification and they had suffered from recent fractures or deformities due to previous fractures in the long bones of the lower extremities. These patients had been treated with various types of intramedullary nails, such as Bailey-Dubow extensile rod. Rush rod and Kuntcher nail, and the result of intramedullary nailing lead to improvement of walking ability.

3.Cutaneous Bronchogenic Cyst.

A 45 year old male patient complained of mucous fluid drainage in the vicinity af the manubrium sterni for 5 years. Skin around the lesion had normal appearance except pinpoint sized sinus opening. H.istopathology showed a cyst in the upper and mid-dermis which consisted of pseudoetratified columnar epithelium with interspersed goblet cells. Goblet; cells were confirmed by positive stain to D-PAS and alcian blue and by metachromatic stain to toluidine blue. VFe present here a case of cutaneous bronchogenic cyst clinically and histopathologically which is thought to be the first cace in Korea.

4.A Clinical Study on Baseball Pitcher's Elbow

History taking, physical examination and x-ray study of both elbows were made on 55 senior high school, college and vocational baseball pitchers who visited the Department of Orthopedic Surgery of Hanyang University Hospital from March 1, 1980 to April 30, 1980. The results of this study were as follows: 1. Average age of 13 high school pitchers was 17.2 years, of 20 college pitchers was 20.1 years, and of 22 vocational pitchers was 23.4 years and average duration of pitching of high school pitchers was 3.8 years, college pitchers was 7.4 years, and vocational pitchers was 9.8 years. 2. Overhand type pitchers were 39(70.9%) and it was the most common pitching style. 3. 45(81.8%) complained elbow pain and the order of frequency was medial, lateral, and posterior. 4. 45(81.8%) showed hpertrophy of forearm and 38(69.1%) showed hypertrophy of arm. 5. Valgus deformity of the elbow joint was observed In 34(61.8%) and flexion contracture of the elbow joint was in 41 (74.5%). 6. Changes on x-ray were ossicle, traction spur, humeroulnar hypertrophy, narrowing of joint space, calciflcation of soft tissue and others. 7. Changes on x-ray were in proportion to duration of pitching and especially combined Iesions were observed more often ln overhand type pitchers.

5.Surface Replacement of The Hip with the THARIES System

The aims of the Tharies replacement are to preserve bone stock and provide a more physiological and anatomical replacement using eccentic shells with the hope that long term durability will be improved. 8 cases of THARIES replacement performed between 1981 and 1983 at Hanyang UniversityHospital were reviewed and analyzed by radiography and U.C.L.A. hip rating system. Follow-up ranged from 34 to 54 months and averaged 43.3 months. The result was relatively good in 6 cases except two cases of aseptic loosening without further progression.

6.Colonna Capsular Arthroplasty for Congenital Dislocation of the Hip: A Report of Five Cases

Colonna capsular arthroplasty is a two-stage procedure for congenital dislocation of the hip, but is not suitable for congenital subluxation. The bone architecture of the femoral head should approach the nearly normal and the child preferably should be more than three years old and less than eight if the best functional result is to be obtained. These brief case reports of five patients who could be followed for from one year and one month to more than seventeen years document the fact that the procedure may produce a hip with an excellent roentgenographic appearance and enduring satisfactory function.

7.Gluteal Fibrosis in Adult

Gluteal fibrosis is rare in Kores. We thought the probable primary etiology of gluteal fibrosis might be repeated intramuscular injection into the buttock, Inability to adduct the legs in the sitting position and flex the hips in neutral position had been observed. Four cases(3 persons) of gluteal fibrosis in adult from October 1986 to February 1987 are presented with much improvement of squatting and flexion of hip in neutral position after operation.

The recent development of endoscopic retrograde cholangiopancreatography (ERCP) now permits the collection of pure human pancreatic secretions via duodenoscopic cannulation. Study objectives are evaluation of exocrine pancreatic function and analysis of pure pancreatic juice collected by use of ERCP technique in subjects without pancreatic disease. 10.A Clinical Study On Gouty Arthritis.

OBJECTIVE: The gout characterized by hyperuricemia is a disorder of purine metabolism and clinical manifestations were seen such as arthritis and tophi in the subcutaneous fitssues. But clinical symptoms were not correlated with the serum level of uric acid. So we analysis of clinical orthopaeclic manifestations and associated factors in gout. METHODS: We have reviewed the medical records, radiography and clinical results of forty six patients admitted at out department between july 1981 and July 1990. RESULTS AND SUMMARY: 1) OF the forty-six patient, 44 cases(95.6%)were male and two female. The age at initial attack was 47.7 year-old in average, from 13 to 66 year-old. 2) Hyperuricemia was associated with obesity in 17 cases(40%), and hyper-tension in 16 cases(35%). 3) The level of uric acid in serum was 9.7mg/% in average on admission and 5.9mg/% on discharge after treated with allopurinol about 2 weeks. 4) The first metatarsophalangeal joint was involved in 28 cases(60.9%) and the ankle joint in 12 cases. Topbus was observed in 14 cases(30.4%) and they were surgically treated in 10 cases.
