1.A STUDY THE SHADE CHANGE OF SEVERAL KINDS OF ALL CERAMIC CROWNS USED FOR METAL DOWEL CORE.

The purpose of this study is to compared and analyze the shade changes(deltaE)about In-Ceram(R),IPS-Empress(R),OPC(R) by using of the spectrophotometer arising from initial status and before and after cementation of the resin cement mounted on the metal core. We used a couple of statistics such as 'One-Way ANOVA'and Multiple Range Test. We could be able to verify significantly what is being discussed here up to 95%. The results drawn from our research are as follows: 1. At the time of our experiments regarding the initial shapes of all-ceramic and mounting status of all-ceramic on the metal crown(deltaE1), and a comparison of mounting of all ceramic on the metal crown with all-ceramic cemented on the metal core(deltaE2), at the time of shade change of initial shapes and after we cemented on the metal core. (1) no significant difference among all-ceramics was found. (2) no particular difference was found regarding the In-Ceram(R) (3) a significant difference between the deltaE1 and deltaE2 regarding the IPSS-Empress(R) was found (P<0.05). (4) a significant difference between the deltaE1 and deltaE2 regarding the OPC? was found (P<0.05). 2. When we compared the shade changes (deltaE) resulted from before and after the cementation on each of the parts involved of some all-ceramic . we could be able to find shape change increase from incisal third, middle third, and cervical third in that order in In-Cream(R)(spinell), IPS Empress(R) and OPC(R) all. In addition , we could be able to find a siginificant difference between cervical third and incisal third, middle third. (P<0.05) From what we have just seen, we might conclude that there is a significant shade change difference before and after the cementation with respect to IPS-Empress(R)and OPC(R). In addition, we could also be able to find more shade change difference at the cervical third rather than incisal third and middle third depending on the parts involved.

2.Clinical Significance of Proteinuria in Pregnancy-Induced Hypertension.

OBJECTIVE: Proteinuria is a major clinical manifestation as well as hypertension and generalized edema in pregnancy-induced hypertension(PIH) and it should be considered an important marker of perinatal outcome. We studied the impact of proteinuria on maternal and perinatal outcome according to the degree of urine protein in PIH. METHODS: Maternal urine protein and serum albumin levels were studied in 64 cases of PIH who were admitted to Department of Obstetrics and Gynecology, Pil-Dong Hospital, College of Medicine, Chung-Ang University for the period of 10 years from January 1, 1989 to December 31, 1998. RESULTS: Serum albumin level was 2.9+/-0.7g/dL in mild preeclampsia group and 2.7+/-0.7g/dL in severe peeclampsia group and there was a significant difference between them. Birth weight of infant was 3001+/-659g in mild preeclampsia group and 2446+/-878g in severe peeclampsia group, and there was a significant difference between them. 1 minute Apgar score was 8.4+/-2.4 and 7.6+/-3.4, respectively and there was a significant difference between them. 5 minute Apgar score was 9.5+/-1.8 and 8.4+/-2.9, respectively and there was a significant difference between them. There were several maternal complications above 2(+) urine protein and no maternal complications below 1(+) urine protein. Small for gestational age infant was more common above 2(+) urine protein than below 1(+) urine protein and fetal death was more common in 3(+) urine protein. CONCLUSIONS: There was a decreasing trend in gestational weeks at delivery as proteinuria become more severe and also in birth weight, 1 minute Apgar score and 5 minute Apgar score. Maternal and fetal complications were more common as proteinuria become more severe.

3.Three cases of earty dyskeratoma.

Warty dyskeratoma is a rare, well-circumscribed epidermal tumor that frequently aries as a single lesion on the skin of the head or neck of middle aged or older persons. Its histopathological features include : suprabasilar clefting with villous proliferations of the connective tissue papillae into the clefts ; acantholytic and dyskeratoic epithelial cells,basilar hyperplasia, and hyperleratosis. We report three cases of warty dyskeratoma describing clinical and histochemical fea tures.

4.Marfan's Syndromne.

Five cases of Marfan's Syndrome were presented. The syndrome first described by Marfan (1896) is not a common condition. We experienced recently 5 cases of it. 3 cases of these occured in a family, and the others were different. The youngest in this series was 11 year-old and oldest 56 years. Four were male and one was female. In all cases, the characteristics are the lens dislocation, visual disturbances and an elongation of the long bones. The cases developed in a family, rarely reported in the literature, so we reported it.

5.Does Painful Heels in Ankylosing Spondylitis Demonstrate Distinctive Features on Plain Radiographs: A Study of 104 Cases.

OBJECTIVE: To investigate simple radiographic findings on painful heels in ankylosing spondylitis (AS). Heel radiography in most studies was from AS patients' non-painful heel. METHODS: Seventy AS patients (34 bilateral cases) with heel pain at the time digital radiographs were taken were studied. Standing lateral views (104 radiographs) of the heel were reviewed. Associations between radiologic abnormalities and disease duration and among various abnormal findings were analyzed. RESULTS: Ninety-six (93.4%) had radiographic abnormalities (82.7% in soft tissues/61.5% in bone). Abnormalities of bone only were observed in 9.6%, of the soft tissues only in 30.8%, and of both were 51.9%. These included Kager's triangle's blurring (77.9%), posterior soft tissue swellings near the Achilles tendon insertion (65.4%), obliterations of the retrocalcaneal recess (65.4%), erosions of the superior pole of the posterior calcaneus (31.7%), subplantar irregular spurs (20.2%), posterior traction spurs (16.3%), subplantar erosions (14.4%) and cortical thickenings of the inferior calcaneal body (5.8%). There was a significant association between swelling in the posterior soft tissue and obliteration of the retrocalcaneal recess (p<0.001). CONCLUSION: Digital radiography in AS is useful for observing not only bony lesions but also soft tissue abnormalities of the heel, particularly of the posterior heel. For assessing the symptomatic enthesitis of the Achilles, this simple and quick diagnostic tool is valuable when examining for soft tissues' alterations of the posterior heel.

7.Pylephlebitis as a Complication of Acute Appendicitis.

Pylephlebitis is defined as septic thrombophlebitis of the portal vein or one of its tributaries, and is usually secondary to an infection in the region drained by the portal venous system. Pylephlebitis is extremely rare today but high mortality still persists. It is a severe complication of acute appendicitis and other intraabdominal and pelvic infections. The patient presents with high fever, chills, leukocytosis, altered liver function, and sometimes jaundice. We describes two cases of pylephlebitis with acute appendicitis. One patient had pylephlebitis with multiple liver abscess and the other had superior mesnteric vein thrombi without liver abscesses. Both the patients had favorable outcomes with medical and surgical therapy. The early diagnosis and treatment with the timely administration of antibiotics and surgical therapy is most important for pylephlebitis.

8.Expression of Angiopoietin-1 and Angiopoietin-2 mRNA in Eutopic and Ectopic Endometrium.

OBJECTIVE: To investigate the mRNA expression of Angiopoietin (Ang)-1 and Ang-2 in ectopic and eutopic endometrium from women with or without endometriosis throughout the menstrual cycle. METHODS: Total RNA was extracted and reverse transcribed into cDNA. The expression of Ang-1 and Ang-2 was examined by quantative competitive PCR (QC PCR) and each result was analyzed by Kruskal- Wallis analysis and Mann-Whitney U-test. RESULTS: The expression of Ang-1 mRNA was statistically significantly higher in endometrium of follicular phase from women with endometriosis compared to that from normal ones in the follicular phase. Endometriotic tissues expressed significantly lower Ang-2 mRNA compared to endometrium from women without endometriosis. CONCLUSION: We suggested that the expression of Ang-1 mRNA could be associated with the pathogenesis of endometriosis and might have a role as neovascularizaton and survival of ectopic endometrium.

9.Radiation Dose Accuracy at the Isocenter: Standard Stereotactic Radiosurgery Technique Developed at Seoul National University Hospital .

PURPOSE: To confirm the accuracy of the radiation dose at the isocenter by the standard linear accelerator-based stereotactic radiosurgery technique which was developed at Seoul National University Hospital. MATERIALS AND METHODS: Radiation dosimetry was undertaken during standard 5-arc radiosurgery using 6 MV X-ray beam from CL2100C linac. The treatment head was attached with circular tertiary collimators of 10 and 20 mm diameter. We measured the absorbed dose at the isocenter of a multi-purpose phantom using two kinds of detector : a 0.125 cc ionization chamber and a silicon diode detector. RESULTS: The dose differences at each arc plane between the planned dose and the measured dose at the isocenter raged from -0.73% to -2.69% with the 0.125 cc ion chamber, and from -1.29% to -2.91% with the diode detector during radiosurgery with the tertiary collimator of 20 mm diameter. Those with the 10-mm tertiary collimator ranged from -2.39% to -4.25% with the diode. CONCLUSION: The dose accuracy at the isocenter was +/-3%. Therefore, further efforts such as modification in processing of the archived image through DICOM3.0 format are required to lessen the dose difference.

10.Confirmation of the Dose Distribution by Stereotactic Radiosurgery Technique with a Multi-purpose Phantom.

PURPOSE: For the purpose of quality assurance of self-developed stereotactic radiosurgery system, a multi-purpose phantom was fabricated, and accuracy of radiation dose distribution during radiosurgery was measured using this phantom. MATERIALS AND METHODS: A farmer chamber, a 0.125 cc ion chamber and a diode detector were used for the dosimetry. Six MV x-ray from a linear accelerator (CL2100C, Varian) with stereotactic radiosurgery technique (Green Knife) was used, and multi-purpose phantom was attached to a stereotactic frame (Fisher type). Dosimetry was done by combinations of locations of the detectors in the phantom, fixed or arc beams, gantry angles (20 ~100 ), and size of the circular tertiary collimators (inner diameters of 10 degrees~40 degrees mm). RESULTS: The measurement error was less than 0.5% by Farmer chamber, 0.5% for 0.125 cc ion chamber, and less than 2% for diode detector for the fixed beam, single arc beam, and 5-arc beam setup. CONCLUSION: We confirmed the accuracy of dose distribution with the radiosurgery system developed in our institute and the data from this study would be able to be effectively used for the improvement of quality assurance of stereotactic radiosurgery or fractionated stereotactic radiotherapy system.
