3.A Case of Ossification in the Phthisis Bulbi.

Phthisis bulbi, resulting from repeated ocular inflammation and infection or ocular trauma, causes various degeneration of ocular tissue as well as changes in ocular external shape. It shows the impression of scleral thickening with calcification of ocular tissue and an unusual ossification as well. Having observed 31-year-old patients of phthisis bulbi and band keratopathy caused by repeated operation after ocular trauma years ago, we extracted the tan brown colored shell-like firm tissue in posterior segment and report sclerotic impression of some of ocular tissue and histopathologic finding of the typical ossificationof ocular tissue during the course evisceration.

5.Clinical Evaluation of Ophthalmic Injury Associated with Head Injury.

Secondary eyeball injury can be caused by head injury as well as direct injury of the eyeball or the orbit. Authors of this report have researched on 117 patients who were applied for ophthalmic examination which in care of the neurosurgical department and have researched frequency of secondary eyeball injury, association between the eyeball injury and the head injury, and favourable frequency of the eyeball injury which were developed without direct injury of the eyeball. There were 21 eyes(9.0%) with external ocular abnormality in 18 patients which included lagophthalmos, ptosis, paralytic strabismus, and there were 14 eyes(6.0 %) with posterior segment abnormality which were vitreoretinal hemorrhage, option. injury and papilledema in 11 patients. In association between the head injury and the eyeball injury, there were many external paralytic injury in basal skull injury and many posterior segment injury in cerebral parenchymal injury. After their injury, follow-up was executed in 3 month interval for 12 month. 15 eyes with external ocular abnormality and 8 eyes with posterior segment abnormality showed favourable progression. Posterior segment abnormality improved statistically significant at 3 month after injury(p<0.05). Secondary ophthalmic injury can be caused by various head injuries. Based on periodic observation, external ocular abnormality improved frequently at more than 9 months after injury. In cases of posterior segment abnormality, there was statistically significant improvement at 3 month after injury(p<0.05). Thus careful observation and treatment should be made in its early stage.

8.A Case of Yellow Nail Syndrome.

We report a 58 year old man who complained that his nails didn't grow for a year and developed yellowish discoloration. On physical examination his nails showed yellow greenish discoloration, increased corvexity, loss of lunulae and cuticles. C]iest X ray revealed atelectasis on right lower lung field and bronchiectasis on left lower lung field. Mild restrictive pattern was observed in pulmoniry function test. We gave him 800IU of Vitamin E claily for 3 months, but rio remarkable changes have been observed as yet..

9.Dynamic MR Imaging in gastric Cancer: Comparison Between Precontrast and Postcontrast Images.

PURPOSE: To analyze contrast-enhancement pattern of stomach cancer on dynamic MRI and to verify the efficacy of intravenous contrast agent in the evaluation of stomach cancer. MATERIALS AND METHODS: Twelve patients with proven stomach cancer underwent dynamic MRI, By using 1.0T scanner, we obtained precontrast FLASH images, and 30, 60, 90 second delay FLASH images after intravenous contrast injection. All patients ingested on liter of water and had intramuscluar injection of Buscopan just before MR study. For quantitative analysis we measured signal to noise ratio (S/N) of stomach cancer on each image, and signal difference to noise ratio (SD/N) between cancer and intraluminal fluid, cancer and the pancreas. For qualitative analysis two radiologists evaluated lesion conspicuity on each image by grading system(grade 0, 1, 2, 3: poor, fair, good, excellent). RESULTS: S/N of stomach cancer increased gradually by time(precontrast, 30, 60, 90 second delay:38.7, 42.5, 57.4, 65.7). SD/N between cancer and intraluminal fluid significantly increased after contrast enhancement(1.24, 25.01, 39.30, 45.89). SD/Ns between cancer and the pancreas were 10.5, 9.33, 9.99, 10.66 respectively. In qualitative analysis, precontrast images were better than postcontrast images for delineation of stomach cancer, Postcontrast images showed clear endo-luminal side of stomach cancer, but outer margin of stomach cancer was more distinct on precontrast images. CONCLUSION: Precontrast MR images are better than postcontrast MR images in the depiction of stomach cancer. Intravenous contrast agent is not imperative in the evaluation of stomach cancer.

10.Reference Values for the Ambulatory Blood Pressure in Normotensive Korean Young Men.

BACKGROUND: To obtain reference values for the ambulatory blood pressure, 30 normotensive Korean young males, aged 22-28 years(mean age: 25 years) were investigated. METHODS: The ambulatory blood pressure was recorded with SpaceLabs Model 90207 monitor over 24 hours at 20 minutes intervals during day-time(6 am-10pm) and at 30 minutes intervals during night-time (10 pm-6 am). RESULTS: The ambulatory blood pressure of the 30 subjects averaged 119/74mmHg over 24 hours, 122/78 mmHg during day-time and 112/68 mmHg during night-time. The upper limit of normal ambulatory blood pressure was suggested as the value of mean+/-2SD which revealed 131/84mmHg for 24 hours, 134/90 mmHg for day-time and 126/80 mmHg for night-time. CONCLUSION: Reference values for the ambulatory blood pressure, in normotensive Korean young men, were obtained by SpaceLab model 90207 monitor.
