1.The Effect of Immunotherapy Based on Interferon - alpha in Advanced Renal Cell Carcinoma.

PURPOSE: Recently in light of the development in immunology, interferon- e and inter- leukin-2 or combination therapy with anticancer drugs have been performed. This study aims to verify and compare the efficacy of therapies using interferon- a alone, interferon- a plus vinblastine, and interferon- a plus interleukin-2 plus 5-fluorouracil (5-FU) plus 13-cis retinoic acid (13cRA) in patients with advanced renal cell carcinoma. MATERIALS AND METHODS: A total of 29 patients were randomly assigned to receive treatment with either interferon- a alone or interferon- a plus vinblastine or interferon- a plus interleukin-2 plus 5-FU plus 13cRA from December 1989 to May 1998. The most frequent metastatic sites were the lung, lymph nodes, bone, liver, and brain. We studied the response rates, survival period, and complications of each regimen. RESULTS: Responses were achieved in 1 out of 1~5 patients (6.73?o) on interferon- a alone (partial responses lasting 13 months), 1 out of 9 patients (11.1%) on interferon- e plus vinblastine (partial responses lasting 25 months) and 1 out of 5 patients (20.0%) on interferon-a plus IL-2 plus 5-FU plus 13cRA regimen (partial responses lasting 14 months). The median durations of survival were 18, 33, and 23 months respectively. The overall response rate was 10.3% and overall median duration of survival was 19 months. The most common side effects were flu-like symptom such as fever, chills (93.1%), skin symptom such as erythema, pruritus (31.0%), G-I symptom such as nausea, vomiting (17.2%), netropenia (10.3%), abnormal LFT (10.3%), and thrombocytopenia (3.4%). CONCLUSIONS: This study confirms the manageability and tolerability of several regimen used. There is no significant differences in response rates and survival duration among the regimens used in this study. The effective immunotherapy in patients with metastatic RCC should be evaluated by further studies of larger patients groups even though a minority of patients responded.

2.A study on normal value of eyeball protrusion in Korean adult using CT

CT may be valuably used indetermination of proptosis. Normal value of eyeball protrusion was measured innormal Koreans using CT. Among 86 Koreans, 26 have normal both eyes, but 60 have one normal eye with one abnormaleye (chiefly, proptosis and and eyeball tumor) The results of 112 eyes are as follows; Absolute measurement of“a”, that is, the distance between cornea & IZL (interzygomatic line) is 16.0±3.4mm in male (for 63 eye),15.7±4.0 mm in female for 49 eyes. Ratio of “a” over “A” (A-P diameter of eyeball) is 57±12% is male (for 63eye), 57±14% in female (for 49 eye) Discrepancy of eyeball protrusion between both eyes is 0.67±1.5mm (for 26adults) Practically, normal range of eyeball prototrusion(“a”) is 12.6mm< male< 19.4mm, 11.7mm

3.Tumor Angiogenesis and Stage in Ovarian Carcinoma.

Tumor angiogenesis has been found to have prognostic significance in many tumor types for predicting an increased risk of metastasis. We assessed tumor vascularity in 28 cases of borderline malignancy and 71 cases of carcinoma of the ovary which had been resected and diagnosed, using the highly specific endothelial cell marker CD34. The numbers of microvessels were counted in 200 magnification in three highly vascularised areas. The numbers of microvessels in carcinomas were higher than that in the borderline malignancy of serous and mucinous tumors. The number of microvessels of mucinous carcinomas was significantly higher than that of serous carcinomas. There were neither significant differences in the number of microvessels according to histological tumor types (p=0.075) nor significant differences in the number of microvessels according to FIGO stages (p=0.072). But in serous carcinomas, the number of microvessels was higher in the FIGO III-IV stage than in the FIGO I-II stage (p=0.017). This study showed higher neovascularization in malignant tumor than borderline malignancy, and in the advanced stage (FIGO III-IV) than less advanced stage (FIGO I-II) of serous carcinomas.

5.Papular Tuberculid and Suppurative Lymphadenitis After BDG Vaccination.

Various dermatological complications have been reported following the BCG vaccination although this vaccination has been recommended to all healthy infants in our country. A 4-months-old boy, who had a BCG vaccination three weeks after birth, developed erythematous papules and pustules on the whole body, especially on the face and upper extremities. Two erythematous subcutaneous soft nodules were also noted on the left side of the neck. A biopsy specimen from the right forearm revealed caseous necrosis surrounded by a granulomatous inflammation consisting of epithelioid cells and lymphocytes. It was centered along the axis of the damaged pilosebaceous follicle. No acid-fast bacilli could be demonstrated by AFB stain. A marked improvement of his skin eruptions was observed after anti-tuberculosis medication. However, nod-ules in the cervical area perforated spontaneously and healed leaving scars several months after the cutaneous eruptions subsided. The clinical and histologic findings and a good response to anti-tuberculous therapy support the diagnosis of papular tuberculid and suppurative lymphadenitis after BCG vaccination.

6.Immunopathological Studies in Immune Vasculitides (II).

Direct immunofluorescent (DIF) starining was performed on biopsy specimens from four patients with pityraasis lichenoides chronica. Deposits of IgM and C3 were foundin the dermal vessel walls of the lesional skin in two patients with pityriasis lichenoides chronic. Deposition of C3 and cytoid bodies were detected along the dermo-epidermal junction in one patient. This supports the view that deposition of immune complexes may play a part in the pathogenesis of pityriasis lichenoides chronica.

7.Statistical Analysis and Comparative Study of Breast Volume and Measurements in Korean Females.

A study of the volume and linear measurements of the breast to determine new nipple positions for 142 subjects was undertaken. Volume determination was done by using the Grossman-Roudner device. The size and shape of the female breast may differ significantly, and such size and linear measurements have to be measured accurately prior to breast aesthetic surgery. Statistical analysis of these results are closely related to physical constitution(height, weight, chest circumference). There have been increasing changes in linear measurement and decreasing changes in breast volume during the last 10 years, The results were as follows: 1) the mean distance from the midclavicular point to the nipple was 19.5cm, the distance from the sternal notch to the nipple was 19.4cm, the distance between nipples was 18.6cm, and the areolar diameter was 3.3cm, 2) the regression equation for the midclavicular point to the nipple was 7.236+ 0.00077x height(cm)+ 0.233 x weight(kg), for the sternal notch to the nipple was 8.845-0.0039 x height(cm)+ 0.214x weight(kg), for the distance between nipples was 11.192 - 0.0033xheight(cm)+ 0.151x weight(kg), and for the areolar diameter was 4.031+0.02xheight(cm)-0.048 x weight(kg),3) the mean value of breast volume of the left side was 209.6cc, and that of the right side was 211.8cc, 4) the regression equation for the mean breast volume was -137.673 + 6.668 x weight(kg), which was equal to -528.414 + 9.115x chest circumference under the axilla(cm) and -468.420 + 8.298 x chest circumference across the nipple (cm). In conclusion, these data could be useful for numerous aspects of breast surgery, including augmentation mammoplasty, mastopexy, reduction mammoplasty, and correction of asymmetrical breasts as the standard for Korean females, if we know the body constituents (weight, chest circumference) before surgery.

8.Esophageal Actinomycosis after Insertion of Esophageal Stent: A Case of Surgical Experience.

Actinomycosis is an indolent, suppurative infection caused by an anaerobic gram-positive organism (usually actinomyces israelii) which usually causes infection in the face, mediastitum, lung, and abdomen. Primary esophageal actinomycosis which is not related with pulmonary or mediastinal actinomycosis, is very rare, especially in immunocompetent host. A 58-year-old woman has been suffered from dysphagia, odynophagia, and chest pain after insertion of esophageal stent in esophageal acid stricture. She underwent a esophagectomy with esophagogastrostomy for above mentioned symptoms. Pathologic diagnosis was a esophageal actinomycosis.

9.The Effect of Hemostatic Solution on Dentin Permeability.

Before impression making in the fixed restorations or other prosthesis, hemostatic solutions are used for hemostasis and moisture control. Hemostatic solutions effectively control bleeding but their major ingredients, acid removes smear layers which are formed in the tooth preparation, exposes the dentinal tubular orifices which are occluded by smear layers, makes dentinal tubular fluid displace more easily to the various external stimulus, and according to the hydrodynamic theory, consequently causes dentin hypersensitivity. To know the effect of hemostatic solutions on dentin permeability, coronal dentin discs, 1 mm in thickness, were prepared from extracted third molars free from decay and wear, and a spilt chamber device was used. Hydraulic conductance values and SEMs, which were measured before and after treatment with Astringedentr , Altract and Epri-dent , were compared and analyzed. The following conclusion were drawn : 1. Hydraulic conductance values which wee measured after the treatment of hemostatic solutions were increased in all groups(p<0.05). 2. %change values of hydraulic conductance were compared but no significant difference was found among the three hemostatic solutions(p<0.05). 3. on SEM observations of all groups, after treatment smear layers were removed and dentinal tubular orifices were partially exposed. on the basis of these conclusions, the reckless use of hemostatic solutions should be restricted, and when in use, various methods should be considered to protect dentin.

10.An Experimental Study of Osteoporosis Produced by Oophorectomy and /or Immobilization in Rats

Previous studies implied association of osteoporosis with estrogen deficiency, immobilization and low calcium absorption only through epidemiological studies. There have been only a few experimental studies verifying the etiologic factors of osteoporosis in vivo condition. Authors conducted an experimental study using white rats(Sprague Dawley) in order to find out what the endocrinological and biochemical changes of experimentally induced osteoporosis are and how they behave with or against each other. White rats, eighty five in number of 3 to 6 months females and weighing 220±12.7gm were divided into four groups. They consisted of Group I(n=10) for control, Group II(n=25) for bilateral oophorectomy, Group III(n=25) for bilateral division of sciatic nerve and hip spica cast immobilization, and Group IV(n=25) for bilateral oophorectomy and bilateral division of sciatic nerve plus hip spica cast immobilization. Blood samples were taken preoperatively and postoperatively at six weeks to check Estradiol and Osteocalcin levels there of. And then, rats were sacrified immediately after the second sampling to retrieve femora for bone mineral density measurement and torsional stress test. Estradiol levels before operation were 21.4±13.3pg/ml for Group I, 31.6±3.1pg/ml for Group II, 25.6±4.5pg/ml for Group III and 33.7±4.5pg/ml for Group IV, respectively. There were no significant differences observable among the groups. Estradiol levels at six weeks postoperatively were 42.3±18.8pg/ml for Group I, 5.4±2.7pg/ml for Group II, 40.8±5pg/ml for Group III and 6.2±2.3 pg/ml for Group IV, respectively. Apparent reductions in group II and IV were proved of statistical significance. Osteocalcin levels preoperatively were 1.2±0.6ng/ml for Group I, 1.7±0.4ng/ml for Group II, 1.5±0.1lng/ml for Group III and 1.5±0.1.lng/ml for Group IV, respectively. At six weeks postoperatively they were 1.6±0.1lng/ml for Group I, 1.7±0.3ng/ml for Group II, 1.8±0.3ng/ml for Group III and 1.2±0.1lng/ml for Group IV, respectively. The differences and changes among the groups and measurements were not of statistical significance. Bone mineral contents at six weeks postoperatively were 0.248±0.03g for Group I, 0.177±0.03g for Group II, 0.226±0.04g for Group III and 0.092±0.01g in Group IV, respectively. Low values of Group II and IV compared to those of Group I and III were of statistical significance.(P=0.0001) Torsional strength of bones at six weeks postoperatively were 4.0±0.2N/m for Group I, 1.5±0.1N/m for Group II, 1.5±0.1N/m for Group III and 1.4±0.1N/m for Group IV, respectively. Decreases of experimental groups(II, III, IV) compared to that of control group(I) were of statistical significance, but differences among the experimental groups were not of significance(p>0.05). For above observations, it was possible to conclude that osteoporosis measurable by bone mineral content and torsional stress test was caused by oophorectomy and immobilization in vivo, the former of which was more rapid and profound than the latter during early phase. When both factors, i.e., oophorectomy and immobilization are exerated simultaneously. the resultant osteoporosis was found in higher degree than either factor only, but not at incremental degree as one may expect.
