3.Prognostic Factors for Non-lesional Temporal Lobe Epilepsy.

PURPOSE: Temporal lobe epilepsy in infants and children have been the focus of many clinical investigations and observations. Several prognostic aspects of nonlesional temporal lobe epilepsy (NLTLE) in childhood remain unclear or controversial. This study was aimed to evaluate the clinical characteristics of NLTLE influencing short-term remission and whether the short-term seizure outcome has any impact on long-term prognosis. METHODS: The study was performed between June 1994 and August 1997. There were 32 newly-referred patients who had diagnosed of NLTLE from the data registry of Pediatric Epilepsy Clinic of Ajou University Medical Center. The patients identification was based on the careful review of hospital records, EEGs, and brain MRI. We have evaluated 6 months terminal remission rate (6M-TR) at one year of continuous antiepileptic drug (AED) treatment as short-term outcome. The predictive value of clinical parameters of NLTLE was comparatively analyzed between the patient who attained 6M-TR and who did not. To identify the long-term prognosis in NLTLE, we analyzed continuous seizure free rate during the next one year and compared between the patient who attained 6M-TR and who did not. RESULTS: 1) Among total 32 NLTLE patients, 18 (56.2%) patients attained 6M-TR at one year of AED treatment. 2) We cannot find any statistically significant clinical parameters influencing the short-term outcome between the two groups : age of onset (P=0.467), duration of illness (P=0.408), seizure type (P=0.725), abnormality of EEG (P=0.473), MRI findings (hippocampal sclerosis or temporal neocortical atrophy) (P=0.685). However, a previous history of perinatal asphyxia (P=0.367) and febrile seizure (P=0.253) were not statistically significant but those clinical parameters suggest clinical significance of influencing short-term outcome of NLTLE. 3) Patients with 6M-TR have showed the next one year remission in significantly higher proportion (77.8%) than those without 6M-TR (28.6%) (P=0.001). CONCLUSION: Early short-term outcome of NLTLE is relatively good and significant proportion of patients with early 6M-TR enters the next one year remission period. So we conclude that short-term remission of NLTLE may be an important determinant in predicting long-term prognosis.

4.Expression of bcl-2 , p53 Protein and Aggressiveness in Thymic Epithelial Tumor.

BACKGROUND: The distinction between non-invasive and invasive or thymic carcinoma has been severely compromised by lack of objective morphological criteria. A reliable biological marker of tumor aggressiveness is, therefore, mandatory for predicting tumor behavior. MATERIAL AND METHOD: Thirty thymic epithelial tumors, including 7 non-invasive thymoma, 10 invasive thymoma, and 13 thymic carcinoma of the Rosai's classification; and 5 stage I, 7 stage II, 2 stage III, and 3 stage IVa of the Masaoka stage of thymoma were investigated for expression of bcl-2 and p53 proteins by immunohistochemistry. RESULT: The thymic epithelial cells showed positive immunostain for bcl-2 in 0 (0%), 3 (30%), 8 (61.5%) of categories in the Rosai's classification respectively and in 0 (0%), 1 (14.3%), 2 (100%), 0 (0%) of stage I, II, III, IVa of the Masaoka stage respectively. Thymic carcinoma, and high stage thymoma had significantly higher proportion of bcl-2 expression than thymoma (p=0.021) and low stage thymoma (p=0.011). However, p53 showed no correlation with the histological subtypes nor with clinical aggressiveness. Bcl-2 expression appeared to be positively correlated with p53 immunoactivity (p=0.007, kappa=0.525). CONCLUSION: These date indicate that bcl-2 expression correlates with aggressiveness in thymic epithelial tumors, but further studies on mutation of p53 protein is necessary because bcl-2 expression appeared to be positively correlated with p53 immunoactivity.

PURPOSE: To assess the results of an arthroscopic repair for traumatic peripheral tears of triangular fibrocartilage complex (TFCC, Palmer type Ib). MATERIALS AND METHODS: 10 patients with traumatic peripheral TFCC tear were treated with outside-in technique with arthroscope and evaluated with an average follow-up of 19 months (range, 15 to 28 months). The clinical outcomes were assessed with investigation of pain, range of motion, grip strength, return to job and patient's satisfaction. RESULTS: The arthroscopic repair of traumatic peripheral TFCC tear resulted in significant pain relief and increase in functional ability of wrist, that is, 8 excellent, 1 good and 1 fair results. At last follow-up, the average of flexion was 79° (range 76~86°), average of extension was 78° (range 70~84°), average pronation was 85° (range 75~91°) and average supination was 87° (range 79~92°). Nine patients except one were back to their original job. CONCLUSION: Arthroscopic repair of traumatic peripheral TFCC tear could be used for pain relief and increase in functional ability of wrist.

8.Tumoral Calcinosis in The Knee Joint Causing Acute Symptoms : A Case Report.

A sixty-eight-year-old female presented with suddenly developed pain in the left knee joint. On the first visit, there was severe tenderness on the anterolateral aspect of the knee joint with active flexion and extension limited by pain. Radiographs revealed multiple round calcific masses in the anterolateral aspect of the knee joint. Under the impression of intraarticular lesions, we performed arthroscopic examination of the knee joint, which revealed no intraarticular pathology. The masses were found in the extracapsular soft tissue and we performed excisional biopsy of the masses completely. The masses were confirmed as tumoral calcinosis by histopathologic examination. It rarely occurs around the knee joint and is usually asymptomatic. Idiopathic tumoral calcinosis with acute symptoms must be differentiated from other pathologic conditions such as gout or infection. The clinical manifestation of this case was opposed to the previous reports. So we report a rare case of idiopathic tumoral calcinosis of the knee joint with acute symptoms.

9.Effect of beta-adrenergics on the Survival and Production of Nitric Oxide in the Cultured Trabecular Meshwork Cells.

PURPOSE: To investigate the effect of beta-adrenergics on the production of nitric oxide (NO) in cultured trabecular meshwork cells. METHODS: Primarily cultured porcine trabecular meshwork cells were exposed to timolol and to propranolol at various concentrations. Cellular survival was assessed by MTT assay and the production of nitrite was assessed by Griess reaction after 24 hours and 3 days respectively. Also investigated was the effect of co-administraton of timolol and isoproterenol. RESULTS: After exposure for 24 hours, neither drug affected the cellular survival. Timolol and propranolol inhibited the production of NO significantly (p<0.05). Isoproterenol abolished timolol-induced inhibition of NO production after 24 hours. These results were similar after exposure for 3 days. CONCLUSIONS: Both timolol and propranolol inhibit the production of NO in trabecular meshwork cells and isoproterenol abolished this effect. These results suggest that beta-adrenerics involves in the production of NO in trabecular meshwork cells.
