5.Solitary Bone Cyst Patella

The common sites of solitary bone cyst(SBC) are humerus, femur and less common sites are tibia, calcaneus, ilium, fibula, radius and ulna. Ischium rib, scapula, spine, clavicle and patella are rare sites of solitary bone cyst. As mentioned above, SBC of patella is very rare and on view of literatures we could find only one reported case which was treated by patellectomy. We also experienced one case of SBC located at patella, so we report this case with review of literatures.

The authors report one-case of hyperthyroidism that occurred in a 56-year-old woman with bipolar disorder after 3 years of chronic lithium treatment. The high level in thyroid fuction test returned to normal after discontinuation of lithium. This case is shows that lithium-induced hyperthyroidism can be reversible.

7.Correlation between Chromosome Abnormalities and Genomic Imprinting in Developing Human 2) Independent Expression of Imprinted Genes, H19 and Insulin-like growth factor II (IGF2), in Androgenetic Hydatidiform Moles.

Human uniparental gestations such as androgenetic hydatidiform moles provide a model to evaluate the integrity of parent-specific gene expression,-i.e, genomic imprinting,- in the absence of a complementary parental genetic contribution. Several imprinted genes are characterized so far including the insulin-like growth factor-2 gene (IGF2) coding for a fetal growth factor and the Hl9 gene whose normal function is unknown but which is likely to act as an untranslated mRNA for its tumor-suppressing function. IGF2 is expressed exclusively from the paternal allele while Hl9 from the maternal allele. Such an alternate expression is quite interesting because both Hl9 and IGF2 genes are located close to each other on chromosome 11p15.5. An in situ hybridization analysis has shown strong expression of Hl9 and IGF2 alleles in nine hydatidiform moles. Especially, a prominent expression of Hl9 and IGF2 was detected in cytotrophoblast and the cellular localization was almost paralleled in Hl9 and IGF2 transcripts . Hl9 and IGF2 genes could be expressed either biallelically or monoallelically in the moles. However, IGF2 biallelic expression did not affect allele-specificity of Hl9 expression.. These results suggest that both H19 and IGF2 transcripts are expressed in the same cells and that the functional imprinting of H19 and IGF2 genes in hydatidiform moles can be controlled individually and independently of each other.

9.Urinary enzyme levels after ESWL for renal stone.

ESWL is a safe and effective treatment for renal calculi. However, on destruction of the stone, the pressure of shock waves produces renal microdamage. Urinary enzyme testing has been used to diagnosis and monitor various types of renal injured. Three urinary enzymes, N-acetyl-beta- glucosaminidase, beta-galactosidase, gamma-glutamyl transferase in 24 hour urine were monitored in 20 patients before and after ESWL and in 5 controls. The ESWL, a single session treatment, was performed using EDAP LT-01. The amount of the shock wave treated was same regardless of stone size and location. The results were obtained as follows. 1. Post-ESWL N-acetyl-beta-glucosaminidase level was higher than that of control on day 1 and showed a rapid decrease on day 3, and returned to normal by day 28. 2. Post-ESWL beta-galactosidase level was higher than that of control on day 1 and was still higher on day 7, and returned to normal by day 28. 3. Post-ESWL gamma-glutamyl transferase level reached to a peak on day 1 and showed a rapid decrease on day 3, and returned to normal by day 28. In summary, ESWL for renal stones elevates urinary enzymes transiently, which was on peak one day after the treatment and returned to nearly normal level by 1 month after the treatment. Therefore, urinary enzymes seem to be useful in evaluation of the functional recovery of the kidney after ESWL.

10.The changes of CT number in various organs according to hematocrite level.

The computed tomography (CT) number is closely related to hematocrit level according to many of the previous reports. We measured the CT number in various organs, such as the frontal white matter, basal ganglia, occipital white matter, cerebrospinal fluid, liver, abdominal aorta, spleen, kidney, and psoas muscle. We correlated the CT numbers of the organs with hematocrit levels which were graded into 10% increments (20.0-29.9%, 30.0-39.9%, 40.0-49.9%). Thus the change of CT numbers in various organs according to the hematocrit level was analyzed. The increased CT numbers according to the 10% increment of hematocrit in the frontal white matter, basal ganglia, occipital white matter, liver, abdominal aorta, spleen, and psoas muscle were 1.3, 1.5, 1.6, 3.3, 5.3, 3.8, 2.4 respectively. Even though the CT numbers of the cerebrospinal fluid and kidney were not influenced by hematocrit level the CT numbers in most of the there organs postitively correlated with hematocrit level. Therefore, it was concluded that in the differential diagnosis using CT numbers, the hematocrit level of patient must be taken into consideration.
