1.Inhibition of Contact Hypersensitivity by PUVA Treatment.

Normal C3WHeN strain mice exposed to topical 8inethoxypsomlen plus long wave ultraviolet (PUVA) showed a reduction in contact hypersensitivity, (CH) which was localized to the skin in the area of PUVA treatment (local suppression), whereas systemic PUVA treatment caused diffuse suppression of CH reaction, regardless of the application site of 2,4-dinitro-1-fluorobenzene (DNFB). There seem to be two different mechanisms responsible for CH reduction by PUVA. Local suppression by topical PUVA treatment was thought to be a result of blocking the afferent phase of immune response, it was associated with a lack of CH effector cells in the peripheral lymph nodes and could not be reversed by indomethacin treatment. Diffuse suppression induced by systemic PUVA treatment seemed to be associated with blocking of egress of effector cells from the regional lymph nodes, this depressed CH response was prevented when indomethacin was administered before PUVA treatment.

2.Prognostic Studies on Acute Myocardial Infarction.

A retrospective clinical observation was done in 90 cases of acute myocadial infarction admitted to Hanyang University Hospital from July 1972 to Dec. 1980. The following results were obtained. 1) The ratio of male to female was 2.3:1. Most patients(63.2%) were in the age groups between the 6th and 7th decades. 2) The main symptoms of acute myocardial infarction were chest pain(76.6%), dyspnea (64.4%), radiating pain(27.7%), epigastric pain(18.8%) and palpitation(15.5%). The painless infarction accounted for 11.1% of all cases. 3) The most common preceding disease was hypertension(38.5%) and other associated diseases were diabetes mellitus(13.2%), C.V.A.(8.8%), angina pectoris(8.8%), previous myocardial infarction(6.6%) and drug intoxication(3.3%). No preceding diseases were found in 12.0% of cases. 4) The anterior wall infarction was 45.5% of 90 cases, inferior wall infarction 22.2%, antero-inferior wall infarction 11.2%, subendocardial infarction 7.7%, posterolateral infarction 2% on ECG. 5) The mortality rate of patients according to the Killip class was 4.2% in class I, 14.3% in class II, 50% in class III and 76.9% in class IV. Group of high Killip class was associated with high mortality. 6) In long term prognosis according to Norris' coronary prognostic index, the highest value was 11.72 with average value of 7.02+/-2.65 in survival group, whereas in death group, the lowest value was 5.08, the highest 16.88 and average value was 10.4+/-3.86. 7) High risk subgroup who complicated within the first 4 admission days, occupied 21.1% and low risk subgroup without complication occupied 46.7%. After the 5th admission days, 2.2% of high risk subgroup was expired, whereas there was no death cases in low risk subgroup. 8) Average duration of hospitalization was 22.4+/-9.5 days in high risk subgroup and 17.3+/-6.8 days in low risk subgroup. In low risk subgroup, 10 cases were discharged within the 7th day of admission and 30 cases after the 8th day of admission. 9) 89.5% of total death occured within the 4th hospitalized day, and 66.7% of cases under systolic BP of 84mmHg were expired. Definite cardiomegaly on chest X-ray and past history of myocardial infarction were associated with high mortality. Half of cases with pulmonary edema were died.

3.The Effect of Pantethine(Pantomin(R)) on Serum Cholesterol and Triglyceride in Hyperlipidemic Patients.

The incidence of atherosclerosis in Korea seems to be much increased due to diet change after national development. The pathogenesis of atherosclerosis is not clarified and there are many hypothesis but the most recent and reliable hypothesis is the ratio of HDL-cholesterol per total cholesterol. Under the basis of this hypothesis there have been much trials to administer the agents which has effect on lipid metabolism, so we tried pantethine on 30 patients who visited Han-Yang University Hospital Internal medicine Department, and the result as follows; 1) The mean age of study population was 50.1 years of age, mean body weight was 62kg, and mean height was 160.8cm. male was 17, and female 10. 2) The associated disease of study population was <19 with> cardiovascular disease, in 19, gastrointestinal disease 3, obesity in 1 and others in 4. 3) The undesirable effect of the drug was found on 3 patients; that is, constipation on 1 patient, dizziness on 1 patient, and skin eruption on 1 patient. 4) The mean serum lipids before and after pantethine administration(levels) are as following table. In conclusion, it seems that the effect of the drug which decreases the serum lipids is mild at initial but more increasing as the time goes by and constant, and we experience little side reaction except mild dizziness, constipation and skin eruption.

5.A Case of Papillary Eccrine Adenoma.

Papillary eccrine adenoma is a rare sweat gland tumor with a characteristic histopathological appearance and has a benign clinical course. Five cases were reported in Korea since a report by Song et al. in 1988. We report a case of papillary eccrine adenoma in a 38-year-old male who had a firm, dark brown nodule on the medial surface of the left thigh. Microscopically, the tumor was composed of multiple dilated ducts lined by two or more layers of epithelial cells and the luminal cells showed papillary projections into the lumen in some tubules. Immunoperoxidase staining was positive for CEA, S-100, and EMA. We excised the lesion completely and no evidence of recurrence was observed for 6 months.

6.Prognostic Significance of Histologic Features, DNA Content, Expression of Proliferating Cell Nuclear Antigen (PCNA), c-fos Protein and Transforming Growth Factor (TGF)-alpha and -beta in Giant Cell Tumor of Bone.

PURPOSE: This study was attempted to investigate the prevalence of the expression of c-fos protein, TGF-alpha and -beta, PCNA , DNA ploidy pattern and histopathological parameters of giant cell tumor (GCT) of bone and to correlate with prognosis and to extend our understanding on tumorigenesis of GCT. MATERIALS AND METHODS: Twenty eight cases of paraffin-embedded tissue were studied, classified as recurrent (5 cases) and non-recurrent group (12cases) within the limits of the cases which afforded surgical material on first operation. RESULTS: No significant difference was observed in cellularity of stromal cells, atypia of stromal and giant cells, presence of hemorrhage and necrosis between recurrent and non-recurrent group. However, presence of more than 10 mitotic figures in 10 high power fields in recurrent group was significantly higher than non-recurrent group (p<0.05). The immunoreactivity for PCNA was seen only in nuclei of stromal cells, whereas nuclei of giant cells showed negative staining. The positivity of PCNA revealed no significant difference between non-recurrent (mean; 40.9%) and recurrent group (34.4%). The expression of c-fos oncogene was seen in 5 cases (100%) in recurrent group, and 8 cases (66.7%) in non-recurrent group, and no significant difference was seen. No significant difference of expression of TGF-alpha was seen in 5 cases (100%) in recurrent group and in 11 cases (91.7%) in non-recurrent group. The expression of TGF-beta in stromal cells was significantly higher in non-recurrent group (80%) compared to recurrent group (100%) (p<0.05). In DNA analysis out of 18 cases, 4 cases (22.2%) were aneuploidy and 14 cases (77.8%) were diploidy. Among 4 aneuploidy cases, 3 cases (75%) had no recurrence, and 1 case (25%) had metastasis to lung and expired. No significant difference of DNA ploidy pattern was seen between the recurrent and non-recurrent group. CONCLUSION: Presence of more than 10 mitotic figures in 10 high power fields and less expression of TGF-beta are related to higher possibility of recurrence and it is suggested that the number of mitotic figure (more than 10/10HPF) and expression of TGF-beta could be helpful parameters in predicting recurrence of GCT.

7.Neurologic Complications of Infective Endocarditis:Retrospective Review of 100 Cases.

BACKGROUND: Neurological syndromes often complicate the management of infective endocarditis. METHODS: We reviewed the neurological complications in 100 patients with infective endocarditis in a tertiary care hospital. RESULTS: Neurological complications occurred in 25 patients (25%). Ischemic infarctions occurred in 15 patients, hemorrhagic infarctions in 5, intracerebral hemorrhages in 12, subarachnoid hemorrhages in 4, and subdural hemorrhages in 2. Brain abscess was detected in 4 and seizures in 2 patients. Staphylococcus aureus endocarditis (p=0.01) and mitral valvular heart disease (p=0.015) correlated statistically with the development of neurological complications. The mortality rate was significantly higher in patients with neurological complications than those without neurological complications (p=0.00). Age, sex, and type of valve (native versus prosthetic) were not related to the complication rate. CONCLUSIONS: We concluded that (1) hemorrhagic complications occurred at a higher rate in this study than previous reports; (2) S.aureus infection and mitral valvular disease predicted the occurrence of neurological complications; and (3) neurological complications were related to mortality rates. 8.Clinical Study of 25 Cases of Erythema Dyschromicum Perstans.

We studied 25 cases of clinically diagnosed erythema dyschrornicum perstans (EDP) who were seen in our clinic within 11-year period, from 1975 through 1985, and compared light and electron microscopic findings, and direct immunoflunrescence findings. The results are summarized as follows: 1. There were 16 males(64%) and 9 females(36%). The peak age incidence was in the second decade. 2. The mean age of onset was 18 years(M: 16. 6, F: 19. 4) and the rnean duration was 1.8 years(M: 2.6, F: 1.0). 3. The annual incidence increased from 1981 and was peaked in 1985. 4. The most commonly involvement site was the trunk(92%), the least common involvement(4%) was one of the face, neck, and shoulder combined. 5. The histopathologic findings in most patients shows focal vacuolization of the basal cell layer, pigment incontinence, dermal melanophages. 6. Electron microscopic findings of five patients consist of vauoles with abundant melanosomes in the basal cells, widened intercellular spaces and dermal melanophages.

9.A study on the fracture strength of collarless metal-ceramic fixed partial dentures.

PURPOSE: The objective of this study was to evaluate fracture strength of collarless metal-ceramic FPDs according to their metal coping designs. MATERIALS AND METHODS: Four different facial margin design groups were investigated. Group A was a coping with a thin facial metal collar, group B was a collarless coping with its facial metal to the shoulder, group C was a collarless coping with its facial metal 1 mm short of the shoulder, and group D was a collarless coping with its facial metal 2 mm short of the shoulder. Fifteen 3-unit collarless metal-ceramic FPDs were fabricated in each group. Finished FPDs were cemented to PBT (Polybutylene terephthalate) dies with resin cement. The fracture strength test was carried out using universal testing machine (Instron 4465, Instron Co., Norwood MA, USA) at a cross head speed of 0.5 mm/min. Aluminum foil folded to about 1 mm of thickness was inserted between the plunger tip and the incisal edge of the pontic. Vertical load was applied until catastrophic porcelain fracture occurred. RESULTS: The greater the bulk of unsupported facial shoulder porcelain was, the lower the fracture strength became. However, there were no significant differences between experimental groups (P > .05). CONCLUSION: All groups of collarless metal-ceramic FPDs had higher fracture strength than maximum incisive biting force. Modified collarless metal-ceramic FPD can be an alternative to all-ceramic FPDs in clinical situations.

10.A Statistical Study of Dermatoses in the Gangnam Area of Seoul.

BACKGROUND: There has been no statistical study of dermatoses in the Ganganm area of Seoul which has been rapidly developing in the last 10 years. OBJECTIVE: We evaluated the recent distributions of dermatoses in the Gangam area of Seoul and compared them with the previous reports in Seoul and other provinces of Korea. METHODS: 47,015 new outpatients who visited the Yongdong Severance Hospital from 1983 to 1992 were analyzed statistically RESULTS: The study results are summarized as follows: 1. Among the 47,015 outpatients, the total number of number of male patients were 20,180(42.9%) and female patients 26,835(57.1%). 2. In age distribution, the most frequent age groups were the 3rd decade(21.3%), the lst decade(19.5%), and the 4th decade(19.2%) amounting to 60% of the total outpatients. 3. The most common 15 dermatoses were dermatophytosis, atopic dermatitis, urticaria, irritant contact dermatitis, acne vulgaris, allergic contact dermatitis, seborrheic dermatitis, other eczemas, warts, vitiligo, drug eruption, herpes zoster, psoriasis, bacterial infection, and syphilis. 4. Those dermatoses which showed an increasing tendency annually were seborrheic dermatitis, warts, vitiligo, and syphiiis. Dermatophytosis and bacterial infection showed a decreasing tendency. 5. Seasonal distribution showed that those diseases occurring most frequently during the summer were dermatophytosis, urticaria, allergic contact dermatitis, vitiligo, herpes zoster, and bacterial infection. The incidence of atopic dermatitis and psoriasis increased during the winter season. 6. In sexual distribution, acne, allergic contact dermatitis, and irritant contact dermatitis were more frequent in females, and the remainding common dermatoses did no show any difference. 7. In age distribution, atopic dermatitis and bacterial infection were most frequent in the lst decade. Urticaria, allergic contact dermatitis, acne, drung eruption, psoriasis, and syphilis were most frequent in the 3rd decade, with warts and vitiligo in the 2nd decade, and herpes zoster in the 6th decaded. 8. The distributions of dermotoses as disease groups were eczema(24.9%), urticaria-drug eruption (13.2%), dermatophytosis(10.7%), skin appendage disorder(10.1%), viral infection(10.1%), pigmentary disorders(4.5%), and papulosquamous diseases(4.0%) which constituted 78% of the total. 9. In patients dwelling in apartments, atopic dermatitis and molluscum contagiosumn showed a more frequent tendency in comparison to those living in house. 10. The most common 10 dermatoses among children were atopic dermatitis, urticaria, contact dermatitis, molluscum contagiosum, bacterial infection, dermatophytosis, vitiligo, other viral infections, and nevo-cellular nevi. On the other hand, the most common 10 dermatoses among the elderly were drmatophytosis, other eczemas, urticaria, herpes zoster, pruritus, contact dermatitis, drug eruption, lichen simplex chronicus, seborrheic dermatitis, and vitiligo. CONCLUSION: In contrast to earlier published report from Seoul, the infectous dermatoses, especially parasitic infestation, showed a decreasing tendency. The distribution pattern of the skin diseases did not differ fro other provinces of Korea in general.
