1.Comparative Measurement of Skin Surface Hydration Using a Hydrometer and Corneometer.

BACKGROUND: Hydration, the water content of the stratum corneum, is an important factor in both the appearance and the barrier function of the skin. Many non-invasive methods have been developed in order to assesa the hydration state of the skin and the effect of various moisturizers. The hydrometer and carneometer are commercially available instruments which are widely used in dermatologic research. OBJECTIVE: Our purpose was to compare the measuring usefulnes of both instruments on normal subjects with a wide range of hydration states of the hornylayer. METHODS: We measured the conductance with the hydrometer and the capacitance with the corneometer in various hydrating eonditions and compared both insruments. RESULTS: Conductance had a non linear correlation with capacita ice. Capacitance measured with the corneometer had a high sensitivity to moisture in a wide range of hydration levels. On the other hand, conductance guaged with the hydrometer showed a few sensitivity to moisture at a low level. When the hydretion level was measured after a moiturizer was added, 10 % urea cream showed a sharp increase by 256% in terms of conductanae, and H-base by 97.8%. Mineral oil decreased by 41.5% and petrolatum by 62% respectively. On the other hand, when measured in terms of capacitance under the same conditions as above, 38.9 increase in 10% urea cream, 19.3% increase in H-base, 20% decrease in mineral oil, and 43.5 % decrease in petrolatum were reported. Conductance showed a higher hygroscopicity and a lower water holding capacity than capacitance did. When the measurement of the moisture level iri virious parts of the human body is performed, the corneometer provided us with more stable alies than the hydrometer. While the hydrometer can not the available and appropriate under the condition of insulation, the corneometer can measure moisture levels even down at the depth of 250mm. When measured after the oily surface of the forehead was cleansed with soft tissue, conductance and capacitance revealed higher values of moisture levels than before the eleanin. However, there is no statistical significance in these values. CONCLUSION: The hydrometer is a more appropriate tool for the measurement of moisture at a high level, and the corneometer at a low moisture level. The hydrcmeter is a more sensitive method of measuring the increase and the decrease in the moisture level.

3.Atypical Autoerotic Death Due to Insertion of a Foreign Body into the Urethra and Rectum

Autoerotic death is an accidental death during sexual pleasure, usually caused by a device or equipment used to enhance pleasure. Autoerotic deaths can be classified as typical or atypical. Typical autoerotic deaths occur due to asphyxiation by mechanical compression, while atypical autoerotic deaths occur by means other than asphyxiation. Narcissism-associated accidental deaths during tube and wire insertion into the urethra and rectum are extremely rare. The victim, a man in his 40s, was found dead at his home. Several tools for masturbation and wires for urethral and rectal insertion were observed around the victim and many pornographic videos were also identified. Family accounts and medical records indicated that he masturbated often by inserting plastic tubes and wires into the urethra and rectum. Moreover, the victim had a history of surgeries to remove foreign objects from the urethra and rectum as well as a history of psychiatric treatment. The autopsy results revealed no specific findings indicative of the cause of death. In conclusion, the investigators determined the cause of death as an atypical autoerotic death based on the death scene investigation, autoerotic sexual behavior, personal history, and autopsy results.

Bizarre parosteal osteochondromatous proliferation (Nora's lesion) is a rare benign tumor and known to be primarily occur in the small tubular bone of the hands and feet. However, it is very unusual to be reported that it occurs in metatarsal bone in Korea. Thus, we report this tumor of metatarsal bone including the literature review because we have experienced this example.

7.Clinical Observation on Infective Endocarditis.

A retrospective review of 36 patients with infective endocarditis by strict case definition was performed from 1972 through April of 1984. Male to female ratio was 1:1.25, with mean age of 19.1. Thirty of the 36 patients(83.3%) had the history of predisposing heart diseases: congenital heart disease was most frequent one below the age of 20, rheumatic heart disease between the age of 20-39 and degenerative heart disease above the age of 40. The commonest presenting symptoms were fever, dyspnea, and malaise, and the commonest signs were fever, murmur, and hepatomegaly. Average hemoglobinand hematocrit were 10.1g/dl and 30.8% respectively, showing moderate degree of anemia. White cell count was 11,600+/-5400/mm3 and erythrocyte sedimentation rate was 46+/-18mm/hr. Over four fifths of the patients demonstrated proteinuria and elevated C-reactive protein. Two thirds of the patients showed microscopic hematuria and one third showed positive rheumatoid factor. Eight patients(22.2%) showed arrhythmia and 4(11.1%) showed conduction abnormalities on electrocardiography. The commonest etiologic microorganisms were alpha-hemolytic streptococcus(30.6%) and staphylococcus aureus(22.2%). Abacteremic cases were found in 38.9% of the patients. To compared with alpha-hemolytic streptoccus endocarditis the cases caused by staphylococcus aureus showed the shorter duration of symptoms before admission, the less frequent complication, the higher fever, and the more elevated white cell counts. To penicillin, alpha-hemolytic streptococcus was sensitive in most cultures and staphylococcus arueus was resistant in half of them, and to aminoglycosides vice versa. To cefazolin, both microorganisms were sensitive in most cultures. In 18 of 24(75%) patients vegetation was visualized by echocardiography. The most frequent site of involvement was the mitral valve. And in vegetation detected group complication rate by embolism was found to be higher. The common complications were congestive heart gailure in 24(66.75%), embolism in 8(22.2%), and metastatic infection in 6(16.7%). The mortality rate of infective endocarditis was 25%, and the causes of death were cerebral embolism in 3(8.3%), septic septic shock in 3(8.3%), congestive heart failure in 2(5.6%), and mycotic aneurysmal rupture in 1(2.8%). Conclusively in infective endocarditis the author observed the increasing prevalence of degenerative heart disease as a prdisposing heart disease and increasing incidence of staphylococcus aureus as a causative microorganism. Adn the differences of clinical characteristics between staphylococcus ureus and alpha-hemolytic streptococcus endocarditis would be helpful in diagnosis and treatment of infective endocarditis as well as recognition of prognosis.

8.Running A Closed User Group for an Academic Society: Experience with The Korean Society of Clinical Pathologist' Information Network.

Korean Society of Clinical Pathologists' Information Network(KSCPIN) began its service as a Closed User Group (CUG) In HiTEL. one of the major information providers in Korea, on March 2nd, 1995. The KSCPIN is the first and the only society which has its own information network among the Regular Member Society of Korean Medical Association. As the name CUG implies. this network is opened only for the members of the society. The main concept of KSCPCIN is to offer various informations and opinion exchange sites only to the society members. For providing various laboratory related informations to every one, we accepted non-society members as KSCPIN members to make KSCPIN as an opened CUG. KSCPIN now offers various bulletins and library not only for the society members, except for some limitations. KSCPIN consists of 115 clinical pathologists, 60 clinical pathology residents, 34 business men, 26 physicians, and 15 others making the total members of 262 at present. With new definition of members and new menu, KSCPIN was self-estimated as a successful CUG, and we recommend the utilization of CUG for all academic societies. In near future, we are going to move the CUG on the internet. which is our finalgoal.

9.A case of autoimmune hemolytic anemia due to autoanti-Ce.

A 12-year-old female with histiocytosis X accompanied by autoimmune hemolytic anemia. During the episode of hemolysis, the hemoglobin level fell to 5.2 g/dL. The direct antiglobulin test was weakly positive. The anti-C and anti-e were identified in her serum. The Rh subgroup of her family(father, mother and brother) including the patient, were all same as DCe. The antibodies which showed anti-C and anti-e specificity were confirmed autoanti-Ce(non-separable) using the serum absorbed with various known Rh phenotyped RBCs. Two packed RBCs phenotyped as DeE were transfused for correction of anemia. Acute and delayed hemolytic transfusion reactions were not noted after transfusion. Identification of blood group specific autoantibodies may be benificial in such case for blood transfusion.
