4.Regional disparities in the availability of cancer clinical trials in Korea

OBJECTIVES:
Unequal access to cancer clinical trials is an important issue, given the potential benefits of participation for cancer patients. We evaluated regional disparities in access to cancer clinical trials in Korea.
METHODS:
From the Ministry of Food and Drug Safety database, we extracted 2,465 records of all cancer clinical trials approved between January 2012 and April 2023. To measure disparities in cancer clinical trial access, we calculated the ratio of clinical trials open to non-capital areas relative to those open to capital areas. We then analyzed temporal trends in this ratio, which we termed the trial geographical equity index (TGEI).
RESULTS:
Disparities in access to cancer clinical trials, as indicated by the TGEI, did not significantly improve during the study period (regression coefficient, 0.002; p=0.59). However, for phase II/III trials sponsored by global pharmaceutical companies, the TGEI improved significantly (regression coefficient, 0.021; p<0.01). In contrast, the TGEI deteriorated for trials initiated by investigators or those testing domestically developed therapeutics (regression coefficient, -0.015; p=0.05). Furthermore, the increasing trend of TGEI for phase II/III trials sponsored by global companies began to reverse after 2019, coinciding with the outbreak of coronavirus disease 2019 (COVID-19).
CONCLUSIONS
Over the past decade, access to cancer clinical trials has improved in Korea, particularly for phase II/III trials evaluating therapeutics from global companies. However, this increase in accessibility has not extended to trials initiated by investigators or those assessing domestically developed therapeutics. Additionally, the impact of COVID-19 on disparities in clinical trial access should be closely monitored.
5.A Case of Type 1 Pachyonychia Congenita (Jadassohn-Lewandowsky Syndrome).

Pachyonychia congenita (PC) is characterized by hypertrophic nail dystrophy, focal palmoplantar keratoderma and blistering, oral leukokeratosis, cyst formation, palmoplantar hyperhydrosis, and follicular keratoses on the trunk and extremities. PC is diagnosed by clinical findings and molecular genetic studies. A 26-year-old man presented with hypertrophic nail dystrophy and subungual debris of all 20 nails, hyperkeratotic plaques on the heels of both soles, and oral leukokeratosis. He had no family history of similar clinical findings. The patient's clinical presentation and history were consistent with PC. Herein we report on a rare case of pachyonychia congenita with a review of literature.

6.A Case of Keratosis Follicularis Squamosa (Dohi) Treated with Topical 1% Pimecrolimus.

Keratosis follicularis squamosa (Dohi) is a keratinizing disorder which is a separate clinical entity from keratosis follicularis (Darier's disease). It occurs predominantly in Japanese and appears clinically as scaly patches of 3~10 mm in diameter with brownish follicular plugs in the center, which are symmetrically scattered on the trunks and thighs. Scales on the margin, slightly detached from the underlying epidermis, look like 'lotus leaves on the water'. The efficacy of minocycline has already been confirmed in many cases. In this report, we describe a case of using 1% of pimecrolimus cream for keratosis follicularis squamosa (Dohi) instead of minocycline.

7.Vulvar Skin Metastasis of Lung Squamous Cell Carcinoma.

In this study, we examined a case where lung cancer metastasized to the vulvar area. A 79-year-old woman that was a heavy smoker was referred to our department with a short (two-week) history of dyspnea, and with a painful nodule on her right labium majora, which she had noticed almost three years earlier. Histopathologically, the specimen appeared as a poorly differentiated squamous cell carcinoma. The chest X-ray, chest computerized tomography scan, and percutaneous transthoracic needle biopsy via bronchoscopy revealed squamous cell carcinoma in the right upper lobe of the lung. Based on these clinical and histopathological findings, we concluded that her condition was skin metastasis at the right labium majora from the lung cancer, which occurs very rarely.

Hemangiopericytoma is a relatively uncommon vascular tumor. It was first described by Stout and Murray in 1942. Here, we report a case of a primary prostatic hemangiopericytoma, which has been reported approximately <10 cases. This may be the first case of a prostatic hemangiopericytoma in Korea. A 35-year-old male patient presented with an episode of dysuria and perineal discomfort over the past 3 months. The radiological and histological features indicated a malignant hemangiopericytoma. Systemic chemotherapy was selected as the primary treatment. After 5 cycles of chemotherapy, the size of masses of the prostate and lung decreased. Four months after then, the patient died from hemoptysis.

10.Neurosurgical Management of Cerebral Cysticercosis.

Cerebral cysticercosis presents a quite variable clinical picture in its manifestation and course. This clinical polymorphism produces principal difficulties in its diagnosis and in planning therapeutic strategies as well as in evaluating therapeutic results. The author reviewed his experience with 52 cases of cerebral cysticercosis, surgically treated from 1977 to 1987 in Department of Neurosurgery, Seoul National University Hospital. The age of patients at time of diagnosis varied between 7 and 66 years(mean 43.4 years), with 75% from the fourth to the seventh decade. There was a 1.6:1 male to female ratio. The two most common residency of the patients, next to the Seoul area, were Cheju Provience(21%) and Chollanam Province(15%). The chief symptoms and signs were those of increased ICP(94%), focal neurological deficits(69%), seizure(40%), mental change(33%) and meningismus(21%). We classified 52 cases into the following groups based on the location of lesion in the brain CT scans:a) Parenchymatous(34%), b) Intraventricular(25%), c) Meningocisternal(18%), and d) Mixed type(33%). ELISA was highly reliable in making the diagnosis, this test, however, should not be regarded as a quantitative clue to evaluate the degree of curability. In cases no improvenent was achived clinically after praziquantel therapy and/or surgical treatment disappeared cysts in follow-up brain CT, a further diagnostic work-up to search for a intraventricular or mixed type is recommended. In 2 cases with a single, superficially located parenchymatous cyst, one burr hole trephination followed by aspiration using a brain needle was proved to be very effective. Surgical treatment in combination with a praziquantel therapy produced a marked improvement in 47 cases(90.4%) out of 52 cases. In the mixed type, the surgical outcome was unfavorable compared with other types.
