2.Degenerative Cervical Myelopathy: Pathophysiology and Current Treatment Strategies

Asian Spine Journal 2020;14(5):710-720
Chronic compression or ischemia of the spinal cord in the cervical spine causes a clinical syndrome known as cervical myelopathy. Recently, a new term “degenerative cervical myelopathy (DCM)” was introduced. DCM encompasses spondylosis, intervertebral disk herniation, facet arthrosis, ligamentous hypertrophy, calcification, and ossification. The pathophysiology of DCM includes structural and functional abnormalities of the spinal cord caused by static and dynamic factors. In nonoperative patients, cervical myelopathy has a poor prognosis. Surgical treatments, such as anterior or posterior decompression accompanying arthrodesis, arthroplasty, or laminoplasty, should be considered for patients with chronic progressive cervical myelopathy. Surgical decompression can prevent the progression of myelopathy and improve the neurologic status, functional outcomes, and quality of life, irrespective of differences in medical systems and sociocultural determinants of health. The anterior surgical approach to the cervical spine has the advantage of removing or floating the intervertebral disk, osteophytes, and ossification of the posterior longitudinal ligament that compress the spinal cord directly. The posterior surgical approach to the cervical spine is mainly used for multisegment spinal cord compression in patients with cervical lordosis. In this review article, we addressed the pathophysiology, clinical manifestations, differential diagnosis, and treatment options for DCM.

Korean Journal of Dermatology 1984;22(2):119-126
The present study evaluates the type, frequency and location of basal cell epithelioma, as well as the age and sex distribution of patients and the recurrence rate of basal cell epithelioma. In this study 116 patients, who visited Severance hospital during the last 15 years from Jan. 1, 1967 to Dec. 31, 1981 and were diagnosed histopathologically as having basal cell epithelioma, were analysed. Journal of the Korean Radiological Society 1994;30(1):53-56
PURPOSE: We studied to evaluate the effect of percutaneous catheter drainage (PCD) in liver abscess. MATERIALS AND METHODS: We retrospectively analyzed clinical data (presence of fever, WBC count), radiological findings (computed tomography, ultrasonography and abscessogram) and correlated them with treatment period. Percutaneous drainage of liver abscess were performed in 19 patients under fluoroscopy guide during recent three years. RESULTS: Eighteen patients (95%) were cured, and one patient underwent surgery because of peritonitis caused by PCD procedure. Average treatment period was 17 days. CONCLUSION: In the treatment of liver abscess, PCD is thought to be safe and effective.

Korean Journal of Urology 1998;39(2):138-140
PURPOSE: Lower caliceal stones treated with ESWL do fail to pass more frequently than middle or upper caliceal stones. We analyzed the results of ESWL of renal caliceal stones according to the location of calix. MATERIALS AND METHODS: We reviewed the stone-free rate in 119 caliceal stone patients treated with 3rd generation lithotriptor, EDAP LT-02 according to the caliceal location of the stone. Location of the stones were upper calix in 30, middle calix in 30, and lower calix in 59 patients. RESULTS: Stone-free rate was 90.0%(27/30 patients) in middle caliceal stones, 74.6%(44/59 patients) in lower caliceal stones, and 66.7%(20/30 patients) in upper caliceal stones. CONCLUSIONS: Stone-free rate of lower caliceal stones is higher than upper caliceal stones.

The Journal of the Korean Orthopaedic Association 1989;24(1):103-108
Severe injuries of hands frequently expose the bone and joint due to the defect of crushing of soft tissues. Although the several methods are introduced, the treatment of bone and joint exposure of finger and hand is very difficult to control satisfactorily. From October 1986 to February 1988, we performed the pedicled skin flap in 11 cases; 9 abdominal skin flaps and 2 pectoral skin flaps. Follow up stuides showed good results. The successful surviving of flaps was accomplished in all cases and the dead bones were regenerated by the creeping substitution. Therefore this pedicled skin flap operation can be considered to be a simple and effective method in treating the bone and joint exposure of fingers.

Korean Journal of Dermatology 1984;22(3):327-329
Penile Horn occurs as a single or multiple lesions. It usually starts as warty growth that later becomes hyperkeratotic and assumes the appearance of a horn. A 22-year-old male patient was presented with a slow growing, asymptomatic, dark brownish colored, corn-shaped, protruding mass on prepuce of the penis for 5 months. Histopathologic finding showed verruca vulgaris. We think that penile horn in this case may be come from verruca vulgaris.

Journal of the Korean Radiological Society 1986;22(3):323-331
Cerebral CT findings were described in twenty-six cases with the late sequelae of acute carbon monoxidepoisoning and were compared with the neurological symptoms and signs. The CT findins include symmetricalperiventricular white matter low density in five cases, globus pallidus low denstiy in six cases, ventriculardilatation in seven cases, ventircular dilatation and sulci widening in three cases, and normal findings in tencases. Only one case showed low densities in both periventricular white matter and globus pallidus, Late sequelaeof the interval form of carbon monoxide poisoning were clinically categorized as cortical dysfunction, parkinsonian feature, and cerebellar dysfunction. The severity of the clinical symptoms and sings of neurologialsequelae is generally correlated with presence and multiplicity of abnormal brain CT findings. But CT. Anothercase showing small unilateral low density of globus pallidus had no parkinsonian feature but showed mild corticaldysfunction.
