1.Factors Influencing Corneal Involvement in Leprosy.

Recently, in leprosy patients the frequency of the blindness due to corneal disorder has been increased. We evaluated factors influencing corneal involvement in leprosy patients for the purpose of early detection and treatment of the preventable causes of the blindness. In 100 eyes of 100 leprosy patients and 50 eyes of 50 control who had no abnormality in ocular examination. Each patient was examined with corneal sensitivity test, Schirmer test(Type I) and was graded according to severity of lagophthalmos. The results were as follows: 1. 28 eyes(28%) of the leprosy patients and 3 eyes(3%) of the control patients showed less than 5mm wetting of fiter paper respectively. 2. Corneal sensation was decreased moderately or markedly in 42 eyes(42%) of the leprosy patients and 7 eyes(14%) of the control patients respectively. 3. The values of Schirmer test and corneal sensation were decreased with increasing duration of the disease. 4. In leprosy patients, 32 patients(32%) were showed lagophthalmos. The grade of lagophthalmos was increased with increasmg duration of the disease. These results suggest that leprosy patients should be followed closely for factors(corneal sensation, decreased lacrimation, lagophthalmos) affecting cornea and that treatment directed towards this problem should be initiated early.
2.Prevalence of Antibody to Hepatitis C Virus among the Employees and their Partners in Korea.

OBJECTIVE: This study was performed to determine the prevalence and age-adjusted prevalence of anti-HCV antibody among the employees and their partners in Korea, and whether prevalence varies with job type. METHOD: The blood serum of 29, 278 people, employees and their partners aged between 20 and 60 residing in Seoul or Kyung-gee province, were tested with Immunoradiometric Assay (IRMA) method using third generation anti-HCV serum. RESULTS: Twenty-five among the 29, 278 people tested positive; a prevalence rate of 0.9 per 1, 000 (95% CI 0.6-1.3). The prevalences of different age groups were 0, 0.5, 1.3, and 4.2 per 1, 000 among the subjects in their 20s, 30s, 40s and 50s respectively, which shows the increasing rate of prevalence with age. Age adjusted prevalence among employees was 1.3 per 1, 000. The prevalences among different kinds of job were 1.9 per 1, 000 (95% CI: 0.5~7.0) for construction, 1.7 per 1, 000 (95% CI: 0.7~3.9) for finance, and 1.2 per 1, 000 (95% CI: 0.2~6.6) for telecommunication. No significant statistical difference was found in the prevalence according to job type(p>0.05). CONCLUSIONS: Employees in Korea showed a lower prevalence rate of anti-HCV antibody than that of blood donors, health screening examinees and the general population. This result is in line with that of other studies abroad and is considered to be because employees may have lower rates of Hepatitis C risk activities, such as needle sharing, than unemployed.

3.A Case of Acquired Digital Fibrokeratoma: Immunohistochemical Stain with Anti - factor XIIIa antibody.

In normal hurnan dermis, factor XIIIa positive dermal dendrocyte are located in the papillary areas closely associated with blood vessels and the upper reticular dem These cells represent a specific type of bone marrow derived dermal cells, distinct from Langerhans cells having some features in common with rnonocyte/macrophage lineage and with potential antier presenting activity. Although the significance of these cells has not yet been fully established, it been suggested that they play a major role in skin immune iesponses, in collagen synsthesis regultic and in wound repair. We report a case of acaqired fibrokeratoma which is studiec conventional histopathology and immunohistochemistry. Histopathologic findings of this case showed ovascular proliferation and the increased presence of fibroblast like cells as a common fe;ture of these benign tumors. Immunohistochemical staining with anti factor XIIIa antibody deiaoi strates increased numbera of positive dendritic cells in the upper dermis. There finding supports the fat that some fibroblagt like cells in the upper dermis of acquireid fibrokeratoma may be factor XIIIa positive dermal dendritic cells.

5.Extracorporeal Membrane Oxygenation for the Support of a Potential Organ Donor with a Fatal Brain Injury before Brain Death Determination.

The shortage of available organ donors is a significant problem and various efforts have been made to avoid the loss of organ donors. Among these, extracorporeal membrane oxygenation (ECMO) has been introduced to help support and manage potential donors. Many traumatic brain injury patients have healthy organs that might be eligible for donation for transplantation. However, the condition of a donor with a fatal brain injury may rapidly deteriorate prior to brain death determination; this frequently results in the loss of eligible donors. Here, we report the use of venoarterial ECMO to support a potential donor with a fatal brain injury before brain death determination, and thereby preserve donor organs. The patient successfully donated his liver and kidneys after brain death determination.

6.Substance P and VIP = induced Flare and Wheal Responses in Normal Healthy Persons.

BACKGROUND: Despite concern about information of neuropeptide, the has been no baseline study of neuropeptide in Koreans. OBJECTIVE: The purpose of the study was to investigate the skin sinsitivity of substance P and VIP in normal healthy persoas. METHODS: We prepared 1000pM, 100pM, 10pM solution of substan P 1-11, substnace P 1-7, substnace P 7-11, and VIP. We injected intradermally 50ul of the br ve solutions on 12 sites of both forearms in addition plaebo. We measured the size of the area of flare and wheal along time. We repeated the same test after antihistamine intake. RESULTS: Flare and wheal respinses were dose dependent. Injection of substance P 1-7 did not evoke wheal responses and injection of substance P 7-11 did not wake flare responses. Flare responses of substance P 1-11, ubstance P 7-11, VIP were inhibiteb antihistamine and wheal responses of VIP were inhibitedly antihistamine. CONCLUSION: N-terminal of subtance P is responsible for flarers onses and C-terminal of substnace P is responsible for wieal responses. Flare responses of sisance P were mediated by histamine but wheal responses osubstance P were direct effect on postcapillary venule. Flare and wheal responses of VIF were mediated by histamine.

7.Immunohistochemical study on granulomatous skin deseases.

BACKGROUND: A definition of granuloma is a focal chronic inflammatory response to tissue injury evolved by a poorly soluble substwice characterized by the accumulation and proliferation of the mono-nuclear histiocytic cells. The accuracy with which rnononuclear cells may be identified in skir. is much improved by the use of both heteroantisera and monoclonal antibodies directed against selected cellular antigens, OBJECTIVE: Our purpose was to examine the staining patterns of anti-lysozyme, anti-a-1-antitrypsin, anti-S-100 protein antibodies, and MAC-387 monoclonal anibody in granulomatous skin diseases. METHOD: We performed imminoperoxidase staining(the labelled str prvidin-biotin peroxidase complex method on the formalin-fixed, paraffin-embedded tissue specimens of granulomatous skin diseases. RESULTS: S-100 protein positive dendritic cells were demonstrated in the granulomatous infiltrates as scattered pattern and MAC-387 positive cells were predominantly found in the center of granulomas, The staining pattern and percentage of positively stained cells of a--antitrypsin were similar to those of lysozyme. A1Pha-1-antitrypsin and lysozyme positive cells w re present in the center as well as lymphohistiocytic infiltrates of granulomas. CONCLUSION: These data sugget that histiocytes are composed of heter igeneous groups of cells such as the mononuclear-phagocyte system and dendritic cell system.

10.ATP-induced focal adhesion kinase activity is negatively modulated by phospholipase D2 in PC12 cells.

Extracellular ATP has been known to modulate various cellular responses including mitogenesis, secretion and morphogenic activity in neuronal cells. In the ATP-induced morphogenic activity, focal adhesion kinase(s) such as Fak have been suggested to play a critical role. Binding of ATP to its specific cell surface receptor in PC12 cells induces phospholipase D (PLD) activity. However, the role of PLD on ATP-induced Fak activation in PC12 cells remains unclear. In this study, we investigated the role of PLD on the ATP-induced Fak activation and paxillin phosphorylation using two established cell lines: wild type PLD2- and lipase-inactive mutant PLD2-inducible PC12 cells. Stimulation of cells with ATP caused PLD2 activation via classical protein kinase C activation. ATP also induced Fak activation, and paxillin phosphorylation, and were dramatically reduced by wild type PLD2 overexpression but not by lipase-inactive mutant PLD2 overexpression. When the PC12 cells were pretreated with propranolol, a specific inhibitor for phosphatidic acid phosphohydrolase resulting in the accumulation of PA, ATP-induced Fak activation and paxillin phosphorylation were also reduced. We found that inhibition of tyrosine phosphatases by pervanadate completely blocked PLD2-dependent Fak and paxillin dephosphorylation. Taken together, we suggest that PLD2 activity might play a negative role in ATP-induced Fak and paxillin phosphorylation possibly through tyrosine phosphatases.
