1.Rapid Detection Test for Streptococcus pyogenes in Normal Carriers of Group A Beta-Hemolytic Streptococci.

Streptococcus pyogenes has been regarded very importantly because of its severe sequelae, namely rheumatic fever and acute glomerulonephropathy. Rapid tests to identify this organism have been studied widely in developed countries in 1980s. We proceeded the rapid test (A Strept ADTM, Denka Seiken Co, Japan) of S. pyogenes on 141 children who were Known to be carriers of this organism to evaluate whether the rapid test could be used at the pediatric office base. Throat cultures were taken simultaneously to evaluate the efficacy of this rapid test. The test shoewed very low sensitivity of 17% (13/14) and high specificity of 99% (65/66). The positive predictive value was.93% (13/14) and negative predictive value 51% (65/127) respectively. Most of the positive results of rapid test have been agreed with the actual number of colonies on the throat culture. The reasons for the low sensitivity and negative predictive value of the rapid test should be discovered and supplemented if it could be applicable to the pediatric office and accurate evaluation of the rapid test in patients with acute pharyngitis is needed.

2.Comparison of Two-Dimensional Doppler Echocardiography and Fick Method with Cardiac Catheterization of Qp, Qs and Qp/Qs Ratio in Ventricular Septal Defect in Children.

This study was performed to compare the amount of blood flow from left and right side heart using Doppler echocardiography and Fick method. The pulmonary blood flow (Qp), aortic blood flow (Qs) and Qp/Qs ratio were noninvasively evaluated by two dimensional Doppler echocardiography in 10 patients with ventricular septal defect in children, aged 2 years to 15 years (mean+/-SD=7.1+/-4.9 years). Cardiac catheterization was performed within 2 days after examination of Doppler echocardiography in all patients. Systemic and pulmonary blood flow was calculated by using Fick principle, based on measured oxygen consumption and measured blood oxygen saturation. The Doppler frequency shift was analyzed by computer system combined with Meridian Echocardiography System. Comparison of Qp, Qs and Qp/Qs ratio obtained by invasive methods and by two-dimensional pulsed wave Doppler echocardiography showed a correlation of 0.679 in Qp, 0.710 in Qs and 0.639 in Qp/Qs ratio (p<0.05).

3.Incontinentia Pigmenti Associated with Congenital Heart Diseases.

Incontinentia pigmenti(IP) is an X-linked dominantly inherited disorder with female predominance. Skin lesions are characterized by three or four stages; vesicobullous, verrucous, hyperpigmented and hypopigmented lesions. About 80% of patients with incontinentia pigmenti have one or more associated ectodermal or mesodermal anomalies involving teeth, nail, hair, eye, breast, bones and nervous system. A newborn girl had erythematous based vesicles and bullae on her trunk and extremities with peripheral eosinophilia. Within several days, she showed linear verrucous plaques. A skin biopsy specimen showed eosinophilic spongiosis in the epidermis and numerous eosinophils in the dermis. The diagnosis of IP was made. She was revealed to have some congenital heart anomalies; atrial septal defect (ASD) and patent ductus arteriosus(PDA). Cases of IP with congenital heart disease have been reported very rarely. Therefore, we report this unique case of IP associated with ASD and PDA.

4.Pulmonary artery sling: case report.

Aberrant left-sided pulmonary artery(pulmonary artery sling) is an uncommon anomaly, which may cause significant respiratory abnormality. We report a case of pulmonary artery sling which is combined with persistent left superior vena cava and dextrocardia. This case was identified by esophagogram and CT and confirmed by MRI and angiography. We consider that MRI is a valuable new method for the diagnosis of aberrant left-sided pulmonary artery.
