2.Mechanism of Impaired Growth Hormone Response in Children with Simple Obesity.

Obesity is associated with an impairment of the growth hormone secretion elicited by all stimuli known to date, but the basic mechanism of the alteration is unknown to date, but the basic mechanism of this alteration is unknown. To determine whether obesity is associated with a chronic state of tonic somatostatin secretion, several tests with growth hormone stimuli such as GHRH(1 microgram/kg), clonidine(150 microgram/m(2))and Regular insulin (0.1U/kg, subcutaneously), to obese subjects and normal control with or without pyridostigmine were undertaken, and the Somatomedin-c levels were measured in both obese subjects and matched controls. 1) The peak GH levels and AUC-GH after administration of GHRH, Clonidine or Regular insulin in obese group is less than those in control group. 2) Pretreatment with pyridostigmine increased the peak GH levels and AUC-GH significantly in obese group but the absolute values are less than those of normal group. 3) The Somatomedin-c levels are significantly higher in obese group than control. Our results lend support to the view that chronically high level of somatostatin decreases the responsiveness to GHRH and secretion itself.

4.The Relationship between Socioeconomic Status and Metabolic Syndrome, Using Structural Equation Modelling.

BACKGROUND: This study aimed to investigate the complexity of the relationships between socioeconomic status, health behaviors, stress and risks of metabolic syndrome. By applying structural equational modelling, modified generalized conceptual model had described the associations and interactions among them. METHODS: 24,210 participants (8,242 men, 15,968 women) registered with the Korean Health Examinee Cohort (KOEX) between 2004 to 2008. This study collected data on the socioeconomic status, health behaviors, and stress through individual interviews. Socioeconomic status (education, house income, occupation), health behaviors (diet, physical activity, smoking, alcohol consumption) and stress level were defined as exogenous factors. Endogenous variables were risks of metabolic syndrome based on modified National Cholesterol Education Program Adult Treatment Panel-lll (NCEP ATP-lll, 2006). RESULTS: According to model fitness test, these proposed model were acceptable in men, comparative fit index=0.877, incremental fit Index=0.877, Tucker-Lewis index=0.757, root-mean-squared error associated=0.055. These were compatible also in women as comparative fit index=0.924, incremental fit index=0.924, Tucker-Lewis index=0.851, root-mean-squared error associated=0.050. There were sex difference related to risks of metabolic syndrome: in men socioeconomic status (β=-0.08), health behaviors (β=-0.25) and stress (β=-0.25) were relevant, but in women only socioeconomic status (β=-0.25) was relevant (P<0.05). CONCLUSIONS: A conceptual model properly explains how Socioeconomic status may influence on health behaviors, stress, and risks of metabolic syndrome. Health behaviors in men and socioeconomic status in women had the strongest associations with risks of metabolic syndrome. In addition, socioeconomic status was strongly associated with health behaviors, stress and age.

5.Estimation of Long-term Care among in-patients at a Veterans Hospital.

BACKGROUND: This study was done to estimate the size of long-term care in-patients in one Veterans Hospital. Using KADL and KIADL (developed in 2002, verified validity & reliability), we evaluated the activities of daily living among in-patients in Daegu Veterans Hospital. METHODS: During the two months in June and July 2003, interviews were conducted by two interviewers. Daegu Veterans Hospital is 300-bed hospital and 257 in-patients were interviewed. Information from patients, care-givers, nurses and others were obtained. We classified a patient as severely disabled reguiring long-term care if one's total-KADL score over 16. RESULTS: The patients studied were representative of in-patients of Daegu Veterans Hospital. Their characteristics were males, old aged, slightly lower education but with high income compared to community based people. Among the total, 34% were classified as severe disabled (total-KALD score over 16) needing long-term care. The distribution of total-KADL was bi-modal (both the independent group and the dependent group occupied a high percentage). But the distribution of total-KIADL was more skewed to the independent group and we were able to estimate that the real long-term care need might be over 34%. Average admission period during the recent six months was 83.3 days in all-covered patients and 55.3 days in partial-covered patients. There was no significant correlation between the total-KADL/KIADL score and the admission period. It was suggested that Daegu Veterans Hospital was utiliged as a long-term care hospital. CONCLUSION: Among the total, 34% of admitted patients was classified as a long-term care group. Considering KIADL, the percentage of patients needing long-term care may be greater than those studied.

6.A Case of Severe Serotonin Syndrome Induced by Fluoxetine and Sertraline.

A 54-year-old male patient who was suffering from bipolar I disorder for 19 years and was admitted to the National Bugok Mental Hospital due to a depressive episode, was referred to the Kosin University Gospel Hospital. On arrival at the emergency room, he had confused mentality with disorientation, memory impairment, hypomania, marked anxiety and hyperirritability. The change of neuromuscular activity such as ataxia, gait disturbance, tremor, shivering, myoclonus and epileptic seizures was also shown. In addition, the symptoms and signs of autonomic instability including diaphoresis, tachycardia, hypotension, fever and facial flushing were noticed. The above symptoms developed after the administration of sertraline successive to the discontinuation of fluoxetine without any washout period. The degree of severity seemed to be severe because he had epileptic seizures, fever and hypotension. He was recovered from the severe serotonin syndrome by the supportive symptomatic treatment with sodium valproate, clonazepam, lorazepam and cyproheptadine after cessation of the selective serotonin reuptake inhibitors during hospitalization. Therefore, this rare case of severe serotonin syndrome was reported and related literatures were also reviewed.

7.Comparison and Analysis for CT Findings of Gastric Adenocarcinoma and Gastric Lymphoma.

PURPOSE: The purpose of this study is to analyze and compare the CT findings of gastric adenocarcmoma and gastric lymphoma. MATERIALS AND METHODS: We retrospectively analyzed and compared the CT findings in 46 cases of adenocarcinoma and 12 cases of lymphoma, which were pathologically confirmed by operation and endoscopic biopsy. RESULTS: Antral involvement was more common in adenocarcinoma(71.7%) than in lymphoma(41.7%). Localized involvement was more common in adenocarcinoma(83.7%), while diffuse involvement was more common in lymphoma(55.6%). Adenocarcinoma tended to have smooth inner margin(83.8%), while lymphoma had nodular or irregular inner margin(36.4%). Mean gastric wall thickness of adenocarcinoma was 1.7cm, while mean thickeness of lymphoma was 2.7cm. The involved wall of adenocarcinoma tended to be highly enhanced (59.5%), while lymphoma was moderately enhanced(90.1%). Perigastric lymphadenopathy was present in 40% (group I) and 42%(group II) of adenocarcinoma and 33% of lymphoma(group I and II). Three cases of lymphoma had lymphadenopathy below renal vascular pedicles with no or minimal perigastric lymphadenopathy. When only the distal portion of the stomach was involved, disturbance in passage developed in six adenocarcinoma (12%), while none developed in the lymphoma. CONCLUSION: CT features help to differentiate between gastric adenocarcinoma and gastric lymphoma

9.A Case of Wegener's Granulomatosis with Acute Renal Failure and Pneumothorax Developed during Treatment with Immunosuppressive Agent.

Wegener's granulomatosis is characterized by necrotizing granulomatosis lesion of the respiratory tract, glomerulonephritis and frequently vasculitis involving other organs. The basic pathophysiologic mechanism of Wegener's granulomatosis is not defined yet. However, it may be suspected an autoimmune disease. We experienced a case of Wegener's granulomatosis which are associated with acute renal failure and pneumothorax. The patient suffered from hemoptysis, fever and cough. Despite antibiotic therapy, symtoms did not improved and multiple varying sized nodules were aggravated on chest roentogenogram and serum creatinine elevated 3.4mg/dl. After diagnosis using video associated thoracoscopic surgery, the patient was treated with cyclophsphamide, glucocorticoid and sulfamethoxasole-trimethoprime. With the combination therapy, the patient felt completely well and chest roentogenogram showed lungs were improved and serum creatinine was normal. The patient was readmitted because of right pleuritic pain and dyspnea 15 day after discharge. The patient was developed a right pneumothorax. The lung was easily expanded by intercostal tube drainage with a one way valve. The patient has been treated as an out- patient with immunosuppressive agents continously.
