1.A clinical and histopathological study of pityriasis lichenoides et varioliformis acuta(PLEVA).

We reviewed the clinical features and the skin biopsy slides of 27 patients who were diagnosed as PLEVA clinically or histapathologically in order to evaluate the clinical characteristics and the specific histopathological findings of PLEVA. The results were as follows : l. 13 of these 27 cases(48%) were confirmed as PLEVA. 2. PLEVA was more frequent in males(85% ), most of whom were young adults. 3. In most cases of PLEVA, the predilection site was on the trunk and extremities, and mild pruritus was a characteristic complaint. 4. Most cases of PLEVA were treated with tetracycline or PUVA therapy with temporary remission, but this therapy did not decrease the rate of recurrence. 5. Some histopathological findings previously described as typical in PLEVA(eg, trapped RBC in the epidermis, exocytosis of lymphocyte, dermal hernorrhage and lymphocytic perivasculitis) seemed not to be specific and were often seen in a variety of other dermatoses. 6. The histopathological findings of linear parakeratosis(85%) and dyskeratotic cells in the middle and upper epidermis(92% ) seemed to be significant findings for PLEUA. 7. The above mentioned histopat,hological findings for PLEVA were frequently found from the vesicular lesions or necrotic hemorrhagic papules.

2.High-Grade Myxofibrosarcoma Showing Pleomorphic Hyalinizing Angiectatic Tumor-like Appearance: A Case Report.

Myxofibrosarcomas (MFSs), which consist of multiple nodules with a variable cellular population in a myxoid matrix, are primarily located in subcutaneous tissue. Pleomorphic hyalinizing angiectatic tumors (PHATs) are rare soft-tissue tumors characterized by a proliferation of highly pleomorphic spindle or polygonal cells and abundant ectatic blood vessels in cellular or myxoid stroma. We present here an unusual case of a high-grade MFS with a PHAT-like appearance. A 67-year-old man presented with an asymptomatic subcutaneous mass in the right forearm. The tumor had myxoid, hypo-, and hypercellular areas with highly pleomorphic spindle or polygonal tumor cells that showed frequent mitoses and nuclear pseudoinclusions. Foci of punctuate necrosis and inflammatory infiltration were present throughout the tumor, and abundant ectatic, thick-walled vessels containing blood clots were noted. The tumor cells were immunohistochemically positive for vimentin but negative for CD34, S-100 protein, smooth muscle actin, desmin, and bcl-2.

3.Two Cases of Pseudoepitheliomatous , Keratotic and Micaceous Balanitis.

Pseudoepitheliomatous, keratotic and micaceous balanitis is a rare distictive clinical entity that represents a histologic spectrum ranging from hypertrophic hyperpalstic penile dystrophy to verrucous carcinoma. This condition is thought to be a malignant growth potential by resistance to treatment and its tendency toward local recurrence. We report two cases with similar clinical presentation of hyperkeratotic plaque and micaceous scaly patches on the glans penis that were compatible with pseudoepitheliomatous, keratotic and micaceous balanitis. Histopathologically, case 1, 48 year-old male was progressed to squamous cell carcinoma and case 2, 78 year-old male, was shown pseudoepitheliomatous hyperplasia without malignant changes.

4.Effective Exchanging Method of Ureteral Stent in Patient with Severe Ureteral Stricture.

PURPOSE: The ureteral stent is very effective for the drainage of urine and maintenance of renal function in patients with ureteral strictures. Retrograde ureteral stunting is a common procedure. However, there is a need for antegrade ureteral stunting with percutaneous nephrostomy in cases of severe ureteral strictures. In these cases, retrograde exchange of ureteral stent is very difficult. Hence, we tried an easier and more efficient exchanging method available in these cases and also assessed its clinical results. MATERIALS AND METHODS: We performed retrograde ureteral scent exchange In the patients using the following procedure. First, the distal tip of the previously Inserted ureteral stunt was pulled out 2cm beyond the external meatus. Second, a guide-wire was inserted into the scent through the hole upto the kidney and then the stunt was completely removed. Third, a cystoscopic sheath without obturator was introduced into the bladder passing over the guide-wire. Fourth, a new ureteral stent was introduced into the ureter over the guide-wire with pusher. RESULTS: We experienced successful exchange of ureteral steno 95 times on 9 ureters in 8 patients with severe ureteral strictures who had antegrade ureteral stenting performed. CONCLUSIONS: From these results, we conclude that this exchanging method is very safe and effective.

5.Observations of Arrhythmias by 24 Hour Ambulatory ECG Monitoring in Early Convalescent Phase of Acute Myocardial Infarction.

In order to observe the arrhythmia which occured in early convalescent phase of acute myocardial infarction, a 24 hour ambulartory ECG monitiring was performed in 15 patients who is admitted at dept of Int Medicine, Ewha Womans University hospital from Aug. 1985 to Sept. 1986. during 7.5+/-3.4 days after admission. In all cases one or more of the following arrhythmias were observed;atrial premature beat, ventricular premature beat(VPB), transient paroxysmal ventricular thchycardia, atrial fibrillation, sinus arrest and junctional beat. Of 15 cases, 11(77.3%) had VPB's:Simple VPB's in 4 cases(26.7%) and complex VPB's(6.52+/-2.52)(P<0.05). But there were no difference of premature and abberancy index between complex VPB's and simple VPB's(P>0.1). In addition, we compaired the arrhythmias in CCU with those of early convalescent phase of acute myocardial infarction. But there was no correlation between them.

6.A Case of Factitial Panniculitis.

We report a case of factitial panniculitis in a 50-year-old female patient who had multiple variable sized subcutaneous nodules on area accessible to the hands and disappeared by only prohibition of habitual skin massage. Histologic findings showed mostly lobular panniculitis with hemosiderin deposits, many foamy histiocytes and pseudomicrocysts.

7.Periodontal Management strategies for the future in Korea.

In periodontics, much progress was made in the understanding of periodontal disease from 1960s to 1980s and in prevention and management of periodontal disease since the end of 1980s. This presentation will discuss about the prevalence of periodontal disease, treatment need, and provision of periodontal treatment in Korea, and how we could manage the periodontal disease efficiently in the future. According to an epidemiological study in Korea, periodontal disease(including gingivitis) was present in 82% of general population and periodontitis in 30-40% in adult population over 30y and juvenile periodontitis in 0.1% of adolescents. If we consider that at least 17% of these patients may have recurrent or refractory forms, there is obviously an abundance of disease that needs treatment. As a result of increase in life expectancy, senile population over 65 y will be increased from 6% in 1996 to 6.9% in 2000, and tooth retention rate and periodontal treatment need are expected to increase. Periodontists need all the help they can get from the general dentists to control periodontal disease. As for provision, postgraduate course in periodontics started in 1957 in Korea and produced over 700 specialized dentists in periodontics. One report indicated that the periodontists as well as general practitioners did periodontal therapy on only a few periodontal patients, because of specific control by current medical insurance system in Korea. Comprehensive periodontal examination is rarely done in local dental clinic. Therefore, enhancement of periodontal care in medical insurance system and education of simplified periodontal examination such as Periodontal Screening & Recording will make dentists diagnose and manage the management of adult patients is based on the recognition that there are multiple diseases, including gingivitis, chronic adlt periodontitis, and other more aggressive forms of periodontitis, and requires the earliest possible recognition of these three disease categories. In this presentation, we discuss practical approach using PSR to diagnose, manage and refer the patients, to facilitate the separation of the simple from the complex and the predictable from the unpredictable form of periodontal diseases and to integrate diagnostic and therapeutic techniques into private practice today.

8.Obesity in elementary school children.

Obesity has been implicated as a significant risk factor for multiple medical condition including hypertention, glucose intolerance, hyperlipidemia, hyperinsulinemia and fatty liver. Furthermore, obesity in childhood is believed to be a reliable predictor of a similar condition in adulthood. During the recent years, there has been a rapid rise in the incidence of childhood obesity in Korea due to increased dietary intake rich in caloric content. This study was undertaken to investigate the epidemiology and potential health complications of obesity observed in elmentary school students. A total of 3,103 children (1630 boys and 1,473 girls), ages ranging from 6 to 12, were studied with personal interview, physical examination, and laboratory evaluation. The results were as follows: 1) The prevalence of obesity was 13.44% with approximately equal distribution among boys (14.42%) and girls (12.36%). 2) The prevalence of childhood obesity in Seoul was significantly higher (18.62%) than that in Suwon (7.85%) and in Hongchon (6.02%). 3) A higher incidence of hypertension was dectected in obese subjects and this was more obvious in girls. 4) Hyperlipidemia and elevated Atherogenic Index were more frequently observed in obese group. 5) although fasting blood glucose levels were similar in both groups. AST and ALT abnormalities were much more prevalent in obese group. It is concluded that hypertension and other metabolic abnormalities are much more frequently observed in obese children and that our effects should be focused on the earliest dection, prevention and management of childhood obesity.

10.Nutritional Status of Preschool Children Residing in Low Income Urban Area: Biochemical Parameters of Protein Nutrition and Immunity.

The purpose of this study was to determine biochemical parameters related to protein and immunity. Subjects were 125 preschool children(M:69, F:56) residing in low income area of Seoul. Mean serum total protein of the children aged 6 was 7.3g/dl which was significantly higher than 6.6g/dl of the group aged 3. The mean serum albumin was 4.7g/dl for 3, 4, 5 age group, and 4.9g/dl for 6 age group and there was no significant difference. Serum retinol binding protein(RBP) is used as a sensitive indicatior of protein, becase it tends to fall rapidly in response to protein status and respond to quickly dietary treatment. Mean RBP for each group(3, 4, 5 and 6 age group) were 2.5 microgram/dl was 15.9%, 19.2%, 24.3% and 16.7%, respectively. The 24-hour urinary excretion of hydroxyproline was 7.9 mg, 14.6 mg, 11.7 mg and 11.8 mg for each group and the mean excretion of all children was 12.2 mg/day. Children aged 3 were excreting significantly lower amount of hydroxprolinc per day than the children aged 4. The mean hydroxyproline index were 2.18, 2.39, 2.52, 2.80 for each age group and the mean of a group aged 6 was significantly higher than that of the group aged 3. The proportion of children assessed as malnourished and impaired growth(hydroxyproline index<2.0) was 18.8%, 4.9%, 2.5% and 4.3%, respectively. The nutrients which showed significant relationship with protein and immunity parameters were niacin, vitamin C and calcium. Vitamin C showed significant positive relationship(p<0.05-p<0.01) with serum RBP, total protein and globulin. The triceps skinfold thickness was significantly and positively correlated with serum globulin. Serum IgG showed significant positive relationship with height, weight, girth of chest and midarm circumference(p<0.05-p<0.01).
