1.Clinical features of bee venom anaphylaxis.

Background and OBJECTIVE: There has been a few case reports of anaphylaxis due to honeybee in Korea. In order to observe the clinical feature of bee sting anaphylaxis. Moderials and methods: Six patients living in Kyunggi province area were referred under history of anaphylaxis after the bee sting. Atopy was defined by skin prick test result to common inhalant allergen. Serum specific IgE antibody to each bee antigen was detected by radioimmunoassay to identify the causative bee. RESULTS: All six cases were female. Three had atopy and four had combined allergic diseases such as allergic rhinitis, asthma, and urticaria. The etiologic bees consisted of yellow jacket (6 cases), paper wasp (4 cases), yellow hornet (3 cases), white faced hornet (1 case) and honey bee (1 case). Four cases had experienced anaphylaxis after ant bite and they showed positive result on specific IgE to imported fire ant. Specific immunotherapy against causative bee venom was begun using bee venom extracts from Bayer (USA) based upon results of specific IgE anti-body to bee venom. CONCLUSION: The yellow jacket is the most common cause of bee venom anaphylaxis in this area. Further studies will be needed to evaluate possible cross-reactivity between bee and ant venom.

4.A Case of Torsed Voluminous Parovarian Cyst.

Parovarian cysts constitute 10% of adnexal masses. They were found in broad ligament and mostly arise from mesothelium and less commonly from paramesonephric element and rarely from mesonephric element. In most cases parovarian cysts were asymptomatic. So, they were found incidentally at surgery for other conditions. Torsion is infrequent and it is difficult to distinguish it from torsion of other adnexal masses, appendicitis, etc. Recently, we experienced a voluminous parovarian cyst undergoing torsion, 25 cm in diameter. The case is presented with a review of literature.

6.M-VAC(Methotrexate, Vinblastine, Doxorubicin and Cisplatinum) Chemotherapy for Metastatic Transitional Cell Carcinoma.

We treated 23 patients with metastatic transitional cell carcinoma with methotrexate, vinblastine, doxorubicin and cisplatinum(M-VAC) chemotherapy from March 1989 to June 1994. The mean age of the patients was 60.0 years old and ranged from 49 years old to 75 years old. They were 18 in male and 5 in female. Complete clinical remission was observed in 5 of 23 patients(21.7%) and 6 patients(26.1%) had a partial remission. An overall objective response rate was 47.8%. The median survival was 12.4 months in all 23 patients, and 20.6 months in 5 complete responder, 8.9 months in non-responder. The duration of survival of the patients with complete remission was prolonged significantly compared to the patients with progression. The progression of disease was in 3 of 4 patients with metastatic bone lesion in effect of M-VAC chemotherapy, but complete clinical remission was 1 of 4 patients. After treatment, complete clinical remission was observed in 5 of 18 patients with metastatic lymph node and 5 patients had a partial remission. The progression of disease was in 1 patient with lung and liver metastatic lesions. Toxicity included moderate to severe myelosuppression that resulted in sepsis in 3 patients and mild to moderately anorexia, vomiting, alopecia and renal dysfunction. It was suggested that M-VAC chemotherapy may be effective as treatment for patients with metastatic transitional cell carcinoma.

7.Appropriate Rest Time after Repetitive Sleep Deprivation Suppresses Apoptosis and Cell Proliferation in the Hippocampus.

PURPOSE: Sleep deprivation may exert many negative effects on hippocampus-dependent cognitive function, such as learning and memory. The present study was conducted in order to investigate the effects of repetitive sleep deprivation on cognition, apoptotic neuronal cell death, and cell proliferation in the hippocampus, using mice. METHODS: To induce sleep deprivation, mice were placed in a water cage containing six platforms (3 cm in diameter), surrounded by water up to 1 cm beneath the surface of the platform for 24 h. Mice were randomly divided into four groups (n=20 in each group): control group, 24 h rest after 24 h sleep deprivation group, 48 h rest after 24 h sleep deprivation group, and 72 h rest after 24 h sleep deprivation group. This cycle was continued for 36 days. Novel objective recognition test and immunohistochemistry for 5-bromo-2'-deoxyuridine (BrdU), western blot for expression of Bax, Bcl-2, brain-derived neurotrophic factor (BDNF), and caspase-3 were performed. RESULTS: Results of the novel objective recognition test showed decreased cognition in the 24 h rest after 24 h sleep deprivation group, while a similar effect was observed in other groups, compared to the control group. Increased cell proliferation and enhanced expression of BDNF and Bax protein were observed in the 24 h rest after 24 h sleep deprivation group and the 48 h rest after 24 h sleep deprivation group, compared to the control group. Expression of Bcl-2 showed a decrease in the 24 h and 48 h rest groups, compared to the control group. Expression of caspase-3 in the dentate gyrus of the hippocampus showed a significant increase in the 24 h rest after 24 h sleep deprivation group and in the 48 h rest after 24 h sleep deprivation group, compared to the control group. CONCLUSION: Results of the present study indicate that insufficient rest after sleep deprivation may induce impairment of cognitive function. After sleep deprivation, at least 72 hr of rest time is needed for recovery.

9.Association of gingival biotype with the results of scaling and root planing.

PURPOSE: The concept of gingival biotype has been used as a predictor of periodontal therapy outcomes since the 1980s. In the present study, prospective and controlled experiments were performed to compare periodontal pocket depth (PPD) reduction and gingival shrinkage (GSH) after scaling and root planing (SRP) according to gingival biotype. METHODS: Twenty-five patients diagnosed with chronic periodontitis participated in the present study. The PPD and GSH of the labial side of the maxillary anterior teeth (from the right canine to the left canine) were evaluated at baseline and 3 months after SRP. Changes in the PPD following SRP were classified into 4 groups according to the gingival thickness and initial PPD. Two more groups representing normal gingival crevices were added in evaluation of the GSH. The results were statistically analyzed using the independent t-test. RESULTS: In the end, 16 patients participated in the present study. With regard to PPD reduction, there were no significant differences according to gingival biotype (P>0.05). Likewise, sites with a PPD of over 3 mm failed to show any significant differences in the GSH (P>0.05). However, among the sites with a PPD of under 3 mm, those with the thin gingival biotype showed more GSH (P<0.05). CONCLUSIONS: PPD changes after SRP were not affected by gingival biotype with either shallow or deep periodontal pockets. GSH also showed equal outcomes in all the groups without normal gingival crevices. The results of SRP seem not to differ according to gingival biotype.

10.Maxillary Sinus Mucocele Secondary to Organized Hematoma.

Excluding cases of maxillary sinus mucoceles developing 1-2 decades after Caldwell-Luc's operation, the primary mucoceles of the maxillary sinus are very rare and believed to result from chronic obstruction of sinus ostium. Organized hematoma represents a rare benign lesion with locally destructive behavior. After the first case reported in 1996, there have been only a few reports for organized hematoma of the maxillary sinus. To our knowledge, this is the first article describing a maxillary sinus mucocele resulting from obstruction of natural ostium by organized hematoma presenting with unilateral cheek hypesthesia and retro-orbital pain. We present this first case with a review of literature.
