2.Three Cases of Familial Occurrence of Crouzon's Disease (Cranlofaeial Dysostosis).

Craniofacial dysostosis, a well defined. rare syndrome first described by Crouzon in 1912, characteristically shows frontal bosses, prognathism, exophthalmos, exotropia, optic nerveatrophy and maxillary hypoplasia. Three cases of familial occurrence are presented, and according to their history, 11 of 13 members in 5 generations of their family are suspected to have been afflicted. A brief review of related literature is described.

3.A Case ef Orbital Cellulitis Subdural Subdural Abscess.

Acute orbital cellulitis is most commonly caused by sinusitis. This complication is based on the anatomy of sinuses, orbit, and their venous connections. We have experienced a case of acute orbital cellulitis combined Subdural abscess due to maxillay sinusitis in a 14-year-old girl. Rapid Surgical drainage in the both upper lid and forehead and craniectomy resulted in complete recovery.

4.A Clinical Study on the Traumatic Hyphema.

Authors have experienced 30 cases of traumatic hyphema for the recent 3 years. We treated all the patients by absolute bed rest. binocular patches, enzyme and osmotherapy, and in some cases, anterior chamber irrigation was performed. The following conclusions were obtained; 1) It took 2~3 1/2 days to be absorbed in the cases of hyphema less than one-quarter volume of anterior chamber. 2) It took 4-16 days in the cases of total hyphema. 3) In most cases of hyphema more than three-quarters volume of anterior chamber, they were associated with other ocular injuries and also shawed poar visual prognosis.

5.2 Cases of Nevus of Ota Associated with Nevus of Ito.

The nevus of Ota is a distinctive, mesodermal pigmentation of the facial skin commonly combined with pigmentation of the eye, usually congenital but capable of benign progression. It is usually unilateral, but in the 1st of these 2 cases, pigmentation of the facial skin and the eye was bilateral and associated with pigmentation of the left shoulder and the anterior cervical area(nevus of Ita). In the 2nd of these 2 cases, pigmentation of the eye and the shoulder was bilateral. Pigmentation of the eye accompanies that of sclera in every case, that of conjunctiva, iris and choroid in some case. It accompanies that of optic disc rarely, but in the 1st of these 2 cases, the pigmentation of optic disc was found in both eyes, in the 2nd, in the right eye. We have experienced the above-mentioned 2 cases of nevus of Ota associated with nevus of Ito which accompanied the pigmentation of optic disc, that are rare. We briefly reviewed the literature relating to nevus of Ota and Ito.

6.Clinical Observation of Congenital Syphilis.

The incidence of congenital syphilis was decreased after penicillin was introduced in the treatment of the disease in 1943. However, occurrence of the disease is rapidly increasing in recent years. The authors observed 46 cases of congenital syphilis in the newborn and infants, in which 28 cases born at this hospital and 18 cases admitted from O.P.D., treated at Han-Gang Sung-Shim Hospital from January 1973 to 1978. The results were as follows : 1) The ratio of the cases delivered at this hospital was 0.35% and that of the cases admitted via O.P.D. was 0.23%. 2) The incidence of congenital syphilis in the newborn infants who were delivered at this hospital was 0.25% in fullterm babies and 0.10% in premature babies. 3) Birth weights of the cases born in this hospital were as follows : Fullterm babies, 71.5% : 2,000~2,300Gm, 14.3% : 1,501~2,000Gm, 14.3% : and no cases below 1,500Gm. 4) Cliniccl manifestations were developed at neonatal period in 77.8% and at infancy in 22.2%. 5) Common clinical manifestations were skin manifestation (73.9%), hepatomegaly (71.7%), splenomegaly (63.1%), and anemia (60.8%). 6) Peripheral blood findings of patients born at this hospital showed mild anemia (hemoglobin level below 15 Gm/dl) of 60.7%, and severe anemia (hemoglobin level below 10Gm/dl) of 25%. 7) VDRL titer of all cases were above 1 : 2. and those of mothers were above 1 : 2 except 1 case. 8) 83.3% of affected cases revealed periostitis and osteochondritis. Most common sites of involvement were tibia (75%) and ulnar (66.6%). 9) Three out of the 46 patients had complications during the treatment : one was nephrot ic syndrome, second was acute pyelonephritis, and third was pneumonia with sepsis.

7.The level of buccal gingival margin around single and two adjacent implant restorations: a preliminary result.

STATEMENT OF PROBLEM: Little information is available about the buccal gingival level of multiple implant restorations. PURPOSE: This study was aimed to evaluate the relationship between width and height of buccal soft tissue around single and 2 adjacent implant restorations. MATERIAL AND METHODS: Four implant restoration groups (first and second molars, single second molars, posterior single restorations between teeth, and anterior single restorations between teeth) were randomly chosen from one dental institute. Each group comprised of 6 patients. After 6 months of function, silicone impressions were taken and stone models were fabricated for each restoration group. The stone models were cut in bucco-lingual direction at the most apical point of buccal gingival margin. The height and width of buccal supra-implant soft tissue were measured. One way ANOVA and Tukey HSD post hoc tests were performed to analyze the data obtained (P < .05). RESULTS: The most unfavorable width-height ratio was noted for the group, which was comprised of the second molar in the multiple adjacent (first and second molar) implant-supported restorations. The group also resulted in the shorter height of buccal supra-implant mucosa rather than that of anterior single implant restorations between natural teeth. CONCLUSION: To achieve a favorable level of buccal gingival margin, greater thickness of buccal supra-implant mucosa is required for the implant restorations without a neighboring natural tooth compared to the implant restorations next to a natural tooth.

8.Biological Effects of bioactive glass and natural coral on periodontal ligament fibroblast-like cell behavior.

The purpose of this study was to evaluate the effects of bioactive glass and natural coral on the human periodontal ligament fibroblast(HPLF) behaviors during the regeneration process of peridontium. To determine the cellular events occuring in the presence of the particles of bioactive glass and natural coral, HPLF were isolated from healthy premolar teeth extracted for orthodontic treatment. Cells were cultured in alphaMEM at 37degrees C, 5% CO2, 95% humidity incubator. Bioactive glass and natural coral were powdered, and each particled(<40micrometer) were placed on the cultured cells at the concentration of 0.3mg/ml, and l,0mg/ml for experimental group. In control group no particles were added. And each group was evaluated by examining the cell morphology under phase-contrast micrograph at 4 day and transmission electron micrograph(TEM) and scanning electron micrograph(SEM) at 14 day, alkaline phosphatase activity at 5 and 9 day, protain synthesis at 4 day, DNA synthesis at 1, 2, 3 and 4 day, cell proliferation at 1, 3, 5,7 and 9 day and the formation of bone nodule at 30 day after culturing all groups in mineralizing supplemented mediun. No significant changes in cell morphology by adding these two matirials were found under phase contrast microscopy and TEM, HPLF phagocytocized each particles suggesting that HPLF is involved in the process of resorbing each particles and that bioactive glass were more biocompatible than natural coral. The ALPase activity of bioactive glass 0.3 mg/ml was similar with control groups and all the rests of control groups were significantly low(P<0.01) indicating a transient dedifferentiation of HPLF in the presence of bioactive glass and natural coral particles. There were no significant differences of protein synthesis between all groups. The DNA synthesis in experimental groups were significantly lower than control groups at 1, 2 and 3 day (P<0.01) but became similar to control groups at 4 day. Between control groups, the DNA synthesis in bioactive glass 0.3mg/ml group was significantly higher than other groups(P (0.01). Cell proliferation in natural coral 1.0mg/ml and bioactive glass l.0mg/ml groups were significantly lower than control group at 3 day(P(0.05) and there were no differences at 5, 7, 9 day. There were more bone nodule formation in experimental groups than in control groups. In conclusion, these results indicated that bioactive glass and natural coral have some effects of a transient dedifferentiation on HPLF and regeneration of periodontal tissues, however any significant cytotoxic effect on HPLF by these two particles were not found.

9.Concomitant Boost Radiotherapy for Stage 3 Non - Small Cell Lung Cancer.

PURPOSE: This study was undertaken to evaluate the treatment outcome and side effects of accelerated radiotherapy (RT) using concomitant boost for stage III non-small cell lung cancer (NSCLC). METHODS: Between April 1991 and December 1994, 102 patients with stage III NSCLC who had the favorable prognostic factors by CALGB criteria, were treated with concomitant boost radiotherapy. Patients were treated with standard large fields to 54 Gy in 6 weeks. The boost treatment was administered concomitantly during the last 2 weeks with a dose of 13 Gy in 10 fractions. The interfraction interval was at least 6 hours. The total tumor dose was 66-70 Gy, given over 6 weeks. RESULTS: With 30 months median follow-up period for survivors, median survival was 15 months with 2 and 3-year overall survival rates of 34% and 19%, respectively. Thirty patients (29%) who had achieved complete remission after RT showed significantly better 2-year survival rates than those without complete remission (58% vs 22%, p 0.001). Local failure and distant metastases as the first or only failure occurred in 40 (44%) and 13 (14%), respectively, and ultimate local and distant failure rates were 45% and 29%, respectively. Although Grade IV esophageal complication of T-E fistula was observed in one patient, most patients with pulmonary complication showed mild, transient radiation pneumonitis. CONCLUSION: This result suggests that the treatrnent of stage III NSCLC with concomitant boost RT may improve survival rates without enhanced radiation induced toxicity compared with conventional RT. Further investigation of dose escalation by conformal radiotherapy of combining chemotherapy and accelerated RT is warranted.
