2.Histological Changes in the Heart-Lung Preparation by the Autoperfusion.

The experimental study for extracorporeal preservation of the autoperfused working heart-lung preparation was performed in 5 dogs. Their weights were between 14??6kg, average of 14.8kg. The preparations were observed from 6 to 8 hours under autoperfusion, average of 5.2 hours. Hemodynamic and hematologic variables were measured during preharvest and autoperfusion. The experimental results during the autoperfusion were as follow : The arterial blood pH revealed severe respiratory alkalosis due to very low PaCO2. PaO2, 130~150mmHg remained constant; Heart rates remained constant at 110/min and the mean aortic pressures were maintained at 75mmHg; Serum Na and Ca showed mild decrease and serum K, moderate increase; Hemoglobulin and white cell count decreased progressively. The lung showed grossly focal congestion and atelectasis following 3~4 hours of autoperfusion and both findings worsened progressively, and little abnormality appeared in the heart. According to the light microscopic findings in the lung, congestion increased 3~4hours following autoperfusion and worsened between the 6th and 8th hours of autoperfusion. Atelectasis, inflammatory infiltration, edema, and hemorrhage worsened after the 4th hour. Emphysematous changes were initially severe to decreasel later on. Hyaline membrane developed after 6 to 8 hours. Hyaline thrombi appeared after 2 hours. The heart was examined by and electron microscope. Vacuolization increased mildly at the 6th to 7th hour. Mitochondrial swelling and cellular swelling showed focally near the end of the perfusion. Glycogen amount was rich throughout the perfusion. It was concluded that the heart preservation was good but the preparation of the lung seemed to be inadequate after 3~4 hours by this autoperfusion technique.

4.Cutaneous Metastasis from Prostatic Carcinoma.

Prostatic carcinoma is one of the most common malignancies in males of western countries but it is a rare malignancy in Korea. If it occurs, it commonly metastasizes to other organs. In-spite of its high incidence of distant metastasis, the cutaneous metastasis of the prostatic carcinoma is very rare and seems to have a poor prognosis. A 67-year-old man presented multiple nontender nodules on the skin of the right thigh and had episodes of multiple metastases from prostatic carcinoma to the lungs and bones. Histopathologic findings of the skin showed atypical cells infiltration at the dermis. Immunohistochemical staining for prostate specific antigen showed a positive reaction, therefore, we reached the diagnosis of cutaneous metastasis from prostatic carcinoma.

5.Case-Control Study on Some Risk Factors of Cerebrovascular Diseases in Rural Community: Matched Analysis with One Control per Case.

The main objectives of this study were to know the prevalence of cerebrovascular disease, to find the important risk factors of cerebrovascular diseases. This study is a part of Eumseong Community Health Project supportes by GTZ(West German Government). 116 perceived cases of cerebrovascular disease were first screened by health interview and examinations and 80 cases were diagnosed as a cerebrovascular group. For comparison, 80 cases were matched with their neighbor controls of the same sex and the similar age. 1. The prevalence rate for cerebrovascular diseases was 476.3 per 100,000 population which is considered to be very high compared with that of other countries. Age adjusted rate for cerebrovascular diseases was 261.6 per 100,000. 2. Among the risk factors examined for the association with cerebrovascular diseases by case-control study, and analyzed by paired marginal test(McNemar's chi-square-test) and odds ratio, only hypertension showed high significant statistical association.

6.Toxic Epodrmal Necrolysis: Report of a Case.

A case of toxic epidermal necrolysis(TEN) in 20 years old female was presente d. The characteristic skin lesions of toxic epidermal necrolyais developed after oral administration of Aspirin for common cold. The cause of the disease. was probably due to Aspirin, but on the microscopic pathologic findings intraepithelial and subcorneal bullae which is characteristic in the staphylococcal induced TEN were noticed. She was treated with a massive systemic corticosteroids(Dexamethasone 12mg/d), antibiotics(Lincocin l.5grn/d), Fluids with electrolytes, sedatives and topical measures. So she was discharged with excellent results in a week of admission.

7.Changes of Bone Mineral Density and IL-6 Levels after Bilateral Ovariectomies in Rats.

OBJECTIVE: Estrogen is a major regulator/modulator of bone metabolism, and bone loss in estrogen deficiency is associated with increased bone turnover, But the mechanism for estrogen action on bone metabolism is still unknown. Recent studies have suggested that the increase in bone loss induced by estrogen deficiency is mediated by increased paracrine production of bone resorbing cytokines. Among cytokines, 1nterleukin-6(IL-6) is released from osteoblasts in estrogen deficiency and increases bone resorption by stimulation of osteoclastic activities and recruitment. Thus we performed this study to evaluate the effect of ovariectomies on bone mineral density and IL-6 in cultured monocytes of peripheral blood and bone marrow. METHODS: The experimental animals were 13 female Sprague-Dawley rats that were 8 weeks of age and weighed an average of 188.5 gram at the beginning of the study. Bilateral ovariectomies were performed in all rats from a ventral approach. Bone mineral density(BMD) of the total body, spine and level of IL-6 of cultured monocytes of peripheral blood and bone marrow were measured before and 8 weeks after ovariectomy. RESULTS: 1) BMD of total body and spine were lower after ovariectomy(0.257+/-0.069g/cm2, 0,208+/-0.005g/cm2) than before ovariectomy (0.276+/-0.005g/cm2, 0.229+/-0.011g/cm2), respectively (P<0.01). 2) Although IL-6 level of cultured monocytes in peripheral blood tended to be higher after ovariectomy than before ovariectomy, this difference was not statistically significant (P>0.05). 3) IL-6 level of cultured monocytes in bone marrow was higher after ovariectomy(82.78+/-4.99pg/ml) than before ovariectomy(48.85+/-2.42pg/ml)(P<0.05). CONCLUSION: It is possible that increased production of IL-6 in estrogen deficiency induced by ovariectomy occurs in the local environment of bone or bone marrow rather than in the pheripheral blood and stimulates bone resorption.

8.Cystic Lesions in Os Calcis

The authors experienced six patients who were admitted in Department of Orthopedic Surgery at Seoul National University Hospital during the period of March 1966 to February 1976 with cystic lesions in calcaneus by radiographic findings. The following results were obtained. Among the six patients, four were osteomyelitis, one was eosinophilic granuloma and the other was fibromyxoid sarcoma. They were all similar in radiographic findings, but were different each other in pathologic findings. 2. Diagnosis is very difficult only with the patients history, hematologic and radiographic findings and the pathologic findings is the most important in the final confirmative diagnosis from benign cystic lesions to malignant one. 3. We describe about the kinds and points of the differential diagnosis. of the other discases from the reported six cases.

9.Diastematomyelia associated with clubfoot: A Case Report

Diastematomyelia, a form of spinal dysraphism, is characterized by a division of the spinal cord or canda equina resulting the form a bony or cartilaginous spur which transfixes the neural elements and dura. The presence of a structure of neural crest origin in the subarachnoid space connecting the spicules suggested that the anomaly resulted from delayed closure of the neural tube and its investments. The cinical finding are presumed due to its restriction, tethering of the normal apward migration of the spinal cord. Congenital anomalies of one or both lower extremities and spine are frequently found with this condition. The surgical removal is recommended as a prophylactic measure against further progressive neurological damage. The purpose of this paper is to presented becanse it was associated with clubfoot and adds to understanding of teratogenesis of diastematomyelia.

10.The t (15;17) Breakpoint of the PML Gene in Acute Promyelocytic Leukemia.

BACKGROUND: The characteristic t(15; 17) of acute promyelocytic leukemia (APL) fuses the retinoic acid receptor alpha (RARA) gene on chromosome 17 to the PML gene on chromosome 15. The test of PML-RARA rearrangement is essential for diagnosis and therapy of APL. We analyzed breakpoints of the PML gene as a basic study for PML-RARA rearrangement test. METHODS: PML-RARA rearrangements, breakpoints of the PML gene and junction sequences were analyzed in 41 patients with APL using RT-PCR and direct sequencing. RESULTS: Forty out of 41 cases revealed PML-RARA rearrangement, of which results coincided with cytogenetic data. Breakpoint distribution was 26 cases in burl (65%), one in bcr2 (2.5%), and 13 in bcr3 (32.5%). Sequencing data showed invariable joining of exon 3 of the RARA gene and exon 6 (bcrl type) or exon 3 (bcr3 type) of the PML gene. One case with bcr2 type had breakpoint in exon 6 of the PML gene with 57 bp deletion. CONCLUSIONS: Bcrl Is the most common breakpoint site of APL in Koreans, and bcy1+2/bcr3 ratio is approximately 2.1. PML-RARA junctions were continuous and joined by a correct splicing event. Breakpoint analysis would be useful in quality control of PML-RARA rearrangement test and the fused protein analysis.
