1.Serodiagnosis of Syphilis.

The serodiagnostic test of syphilis employed with the greatest freguency in this country is the VDRL procedure which is one of the flocculation test utilizing cardiolipin-lecithin as an antigen. As well known, the immunologically nonspecific nature of this test relegates it only to screening test, so that all the sera displaying weakly reactive or reactive VDRL in the absence of definite evidence of present or past syphilis should be confirmed by using the specific treponemal antigen tests. However, there are only lirnited numbers of institutions at which the specific treponemal antigen tests are carried out, because the tests usually need quite coaiplicatnl technology and specifically trained personnel. The Treponema. pallidum hema.gglutination assay (TPHA) which was first described by Rathlev in 1965 and established as more improved and standardized procedure in serodia,gnosis of syphilis by Tornizaxva and Kasamatsu (1966) has been found to h as sensitive and specific as the technically more complicated Fluorescent treponemaI antibody absorption (FTA-ABS) or Treponema pallidum immobilization (TPI) po cedure by rnany investigators from different parts of the world during these 10 years. The TPHA has also been found to have definite advantage over the other tests because it is easy to perform and economic. The present authors felt. that the TPHA meight be the most reasonable method to be used in this country and decided to evaluate it in the serodiagnosis of problem cases. Sera from 516 presumed normal persons, 686 pregnant women ancl 1345 patients with various diseases other than syphilis were screened with the standard VDRL procedure as described and recommanded by USPHS. Out of these, 475 of t.he presumed normal persons, 646 of the pregnant women and 1243 of the patient group wit;h various diseases other than syphilis v;ere serologically nonreactive and 10, 12 and Rl sera from each of the above groups, respectively, were found to be true syphilitc with definite evidence of clinical signs or history of syphilis. Thirty-one out of the presumed normal persons, 28 out of the pregnant women and 71 out of the patient group with various diseases other than syphilis showed either weakly reactive or reactive VDRI, in undiluted sera without definite evidence of syphilis and these 130 sera from 130 persons were the subject of the present study. The TPHA was carried out on these 130 sera. according to the ma.nual MEA-TP instructed by National Institute of Health, Japan. The reagents used in this study rvere rnanufactured and supplied hy the Fujizoki Pharmaceutical Co. Ten (32.3%) out of the R1 sera from preaumed normal persons, 11 (39.4%) out of the 28 sera from pregna.nt women a.nd 28 (69.4%) out of the patient group with various disea ses other then syphilis showed reactive TPHA. Eighty-seven of the total 130 sera showed weakly reactive VDRL so called rough result and 19 (21.8%) Of these 87 sera showed reactive TPHA, while 30(69.8%) of the 43 sera which showed reactive VDRL were found to be TPHA reactive. These results arc in general agreement with those of other investigators confirmed by the more complicated FTA-ABS or TPI. The overall incidence of syphilis was 3.87%, 3.35% and 4.38% in presumed normal persons, in pregnant women and in patient group with various diseases other than syphilis respectively. Considering the relative lack of study concerning to the false positive reactions in this country, these results may be helpful to the clinician to rnake a cert.ain diagnostic decision when they meet such a patient whose VDRL is weakly reactive or reactive in the absence of definite evidence of syphilis. The authors concluded that: the MHA-TP technique is easy in performance, economic and highly specific in serodiagnosis and that this technique is highly recommanded in this country. * The authors thank Fujizoki Pharrnaceutical Company, Tokyo, Japan which provided the TPHA reagents used in this study.

2.Hamartoma of the Breast A report of two cases.

Hamartoma of the breast is a rare benign lesion presenting as palpable mass. They are, however, sometimes misdiagnosed by pathologist due to their resemblance to other benign or physiologic conditions of the breast. We report two cases of hamartomas of the breast. The clinical diagnosis was fibroadenoma in both cases. One patient was a 49-year-old woman presented with palpable mass for the duration of 5 months. Mammography showed a well demarcated round mass with homogeneous density which is slightly greater than the surrounding glandular tissue. Grossly it was a 3.5 3.0 2.0 cm sized round firm mass covered by thin fibrous tissue. Microscopically it was a well defined mass composed of variable amount of breast lobules, dilated ducts and adipose tissue surrounded by dense hyalinized connective tissue. The adipose tissue accounted for 30% of the entire lesion. Cellular atypism was not found. The other patient was a 18-year-old woman presented with palpable mass of the duration of 3 years, which was increased in size recently. Mammography demonstratesd a well delimited lobulated mass with lucent halo. Ultrasonography showed a well encapsulated low echoic mass admixed with high echoic areas. Grossly, it was a 4.0 4.0 2.0 cm sized well demarcated oval-shaped firm mass with translucent thin fibrous capsule.. Microscopic findings were similar to the former. The amount of adipose tissue was 20%.

4.A Case of Pulmonary Blastoma.

Pulmonary blastoma is a rare lung tumor resembling fetal lung tissue. Pathologically the tumor can be classified to 2 groups, well-differentiated fetal adenocarcinoma(WDFA) and biphasic blastoma. WDFA has more favorable prognosis with fewer metastasis at initial presentation and fewer recurrence after treatment. We experienced a case of pulmonary blastoma in 32-year-old female patient. The patient was referred to our hospital because of abnormal mass shadow in right middle lobe. The diagnosis of pulmonary blastoma(WDFA type, Stage I T2NOMO) was confirmed after right middle lobectomy. We followed up 22 months without an evidence of recurrence.

5.Clinical Features and Prognosis of HLA-B27 Positive and Negative Anterior Uveitis in a Korean Population.

Clinical features and prognosis of HLA-B27 positive anterior uveitis (AU) were assessed compared with HLA-B27 negative AU in a Korean population, based on the medical records of AU patients seen at a university hospital. Twenty-seven HLA-B27 negative, idiopathic AU patients (group I) and 55 HLA-B27 positive AU patients (group II) were studied. HLA-B27 positive group was further divided into 29 with associated systemic disease (seronegative spondyloarthropathy) (group IIA) and 26 without associated systemic disease (group IIB). Significantly more severe anterior chamber inflammation in terms of anterior chamber cells (P=0.006) and hypopyon formation (P=0.034) was observed with higher frequency of AU attacks (P=0.007) in the HLA-B27 positive group than in the HLA-B27 negative group. Systemic/periocular steroids were required in significantly more patients in the HLA-B27 positive group than in the HLA-B27 negative group (P=0.015). However, no significant differences were observed for final ocular and visual outcomes between these two groups. Associated systemic disease made no significant difference in the clinical features and prognosis in the HLA-B27 positive AU patients. In conclusion, despite more severe inflammation and a higher recurrence rate, HLA-B27 positive AU shows similar good final ocular and visual outcomes compared to HLA-B27 negative, idiopathic AU in a Korean population.

6.Dose Dependent Effects of Intravitreal Triamcinolone Acetonide on Diffuse Diabetic Macular Edema.

PURPOSE: To evaluate the effect of different doses of intravitreal triamcinolone acetonide on diffuse diabetic macular edema. METHODS: In a retrospective study, 44 eyes with diffuse diabetic macular edema were treated with an intravitreal injection of 4 mg (n=12 eyes), 8 mg (n=17) or 25 mg (n=15) of triamcinolone acetonide (TA). Optical coherence tomography, best-corrected logMAR visual acuity and Goldmann tonometry were performed at baseline, 1 week, and 1, 3, 6, 9 and 12 months after treatment. Mean follow-up was 9.8 months (standard deviation=2.3) with a range of 5-12 months. RESULTS: The duration of intravitreal TA effects on macular thickness and visual acuity increased with increasing dosage. An observed increase in intraocular pressure induced by TA was not significantly associated with dosage. CONCLUSIONS: In patients with diffuse diabetic macular edema who receive intravitreal TA, effects may last longer after a dosage of 25 mg, than after lower doses of 8 mg or 4 mg.

7.Effects of Maeil Human Milk Fortifier on Growth and Bone Mineralization in Preterm Infants.

PURPOSE: A prospective, controlled trial was conducted to evaluate growth, bone mineralization, and nutritional status receiving preterm human milk supplemented with a newly formulated Maeil human milk fortifier. METHODS: Twenty five fortified human milk-fed and preterm formula-fed infants with a birth weight < 1, 800 g and gestational age <35 weeks, who were born at Ajou University Hospital from March, 2003 through August, 2004 were studied. Growth, biochemical indices of bone mineralization, feeding tolerance, morbidity and wrist X-ray were assessed serially. Total body bone mineral density was measured by dual energy X-ray absorptiometry at 2 and 5months of age. RESULTS: There were no differences in growth, including weight, height and head circumference, between two groups. Serum Ca, P, ALP and other biochemical indices were similar. Although low grade rickets (grade I and II) were occasionally found on wrist X-ray, the rate of occurrence and severity were similar. The bone mineral densities of both group showed no difference. CONCLUSION: The fortified human milk-fed infants and preterm formula-fed infants showed no difference in growth, and bone mineralization. This newly formulated Maeil human milk fortifier can be safely used in preterm infants.

8.An Immunohistochemical Study on the Expression of SUMO-2/3 in the Colorectal Carcinoma.

OBJECTIVE: The incidence of colorectal carcinomas continues to rise in Korea due to the westernized life style. However, the precise colorectal carcinogenic mechanisms remain to be elucidated. The protein products of oncogenes and cancer suppressor genes play important roles in the carcinogenesis. The effects of the proteins are influenced by post-translational modifications as phosphorylation, acetylation, methylation, and ubiquitination. The aberrant sumoylation plays some roles in carcinogenesis. However, the expression pattern of small ubiquitin-related modifier (SUMO)-2/3 in the colorectal cancer has not been reported. We assessed the expression of SUMO-2/3 and evaluated the expression pattern in colorectal cancer. METHODS: The SUMO-2/3 expression was tested in one normal colon mucosal cell line and 5 colorectal cancer cell lines by Western blot. We collected 322 cases of colorectal cancer operated from January 2000 to December 2010 at Soonchunhyang University Cheonan Hospital. We fabricated the tissue microarray and the expression of SUMO-2/3 was evaluated by immunohistochemistry. The results were analyzed with clinicopathologic parameters. RESULTS: The SUMO-2/3 was not expressed in the normal colon mucosal cell line. However, it was expressed highly in all the 5 colorectal cancer cell lines as the beta-actin. The SUMO-2/3 was expressed in 68.3% of the colorectal cancers and its expression was correlated with the pathological tumor stage stage (odds ratio, 2.89; 95% confidence interval, 1.10 to 7.55; P=0.031). CONCLUSION: The SUMO-2/3 plays some roles in carcinogenesis and progression of the colorectal cancer.

9.Malignant Peripheral Nerve Sheath Tumor in Descending Colon: A Case Report.

We report a unique case of malignant peripheral nerve sheath tumor (MPNST) of colon, not associated with neurofibromatosis or parasite infection. The tumor presented as an encircling mass in descending colon causing obstruction with nuberous metastatic lesions in a 43-year-old man. The tumor was largely composed of spindle cells which showed strong positivity for vimemtin, S-100 protein and Leu-7. The tumor often exhibited epithelioid feature where tumor cells were weakly positive for cytokeratin.

10.The Effect of Component Transfusion on Perioperative Change of Hemoglobin , Hematocrit and Platelet Count.

The progress of anesthetic and surgical techniques has resulted in an increase in the number of operations requiring transfusions over the past 10 years. Thus, the incidence of complications from shortage transfusions increases daily and a blood supply shortage occurred recently. blood component therapy is now believed to be an answer to these problems. To assess the effect of blood component therapy, especially packed RBC transfusion compared with routine whole blood transfusion, the author compared the preoperative and postoperative hemoglobin, hematocrit and platelet count values. The conclusions are as follows: 1) There were no significant changes in hemoglobin and hematocrit values of the whole population studied, from preoprative values of 12.6+/-0.2g% and 37.6+/-0.6% to postoperative values of 12.7+/-0.1g% and 38.2+/-0.4%, respectively. The platelet count significantly decreased (p<0.05) from 305200+/-9500/mm(3) preoperatively to 249600+/-8800/mm(3) postoperatively in the whole population studied. 2) There were no significant changes in hemoglobin and hematocrit levels according to age distribution, type of transfusion, amount of transfusion, surgical department and operative site, except in the Department of General Surgery, in abdominal surgery, and tumor group surgery in which preoperative hemoglobin and hematocrit values were slightly increased postoperatively. 3) The platelet count significantly decreased postoperatively in the whole population studied except in transfusion amounts under 1 unit, in the age distributions of 20~29, 60~69 and above 70 years, in the Departments of Neurosurgery, Urology, Plastic Surgery, Dental Surgery, ENT, and in head and neck surgery. However, it did not decrease to the level of abnormal bleeding(50.000/mm(3). From the above results, blood component therapy, especially packed RBC transfusion, made no difference in maintaining postoperative hemoglobin and hematocrit levels compared with routine whole blood transfusion.
