BACKGROUND: To investigate the possibie difference, if any, between office blood pressure(BP) and home BP may be important in the diagnosis and treatment of hypersensive patients. This report deails the difference between the two BP's and the usefulness of digital electronic sphygmomanometer(DES) for self-monitoring of home BP. METHODS: The BP's of 14 patients with essential hypertension were measured with mercury sphygmomanometers at outpatient department by physician and with DES at home(twice a day) by the patients. Patients were followed up every 2 weeks for 4 weeks and previous 2 weeks' average home BP's were compared with the office BP's of each 2 weeks' end. RESULTS: There was a significant difference between office and home BP(both systolic and diastolic) ; office average BP(151/95mmHg) was higher than home average BP(136/86mmHg). CONCLUSION: Caution may be needed in the interpretation of office BP unless it is measured several times after adequate rest.

2.A clinical comparison of the urine acid-fast bacilli culture results in renal tuberculosis.

A clinical comparison of the urine acid-fast bacilli culture results with 82 cases of renal tuberculosis patients was made who were admitted to the Department of Urology, Wonju Christian Hospital during the period from January, 1982 to December, 1989. The urine acid-fast bacilli culture positive rate was 37.8%. The glucose and protein contents in urine of culture positive group were 47.6 mg% and 56. 8mg% and they were higher than those of culture negative group's 29.4 mg% and 27.7 mg%. The most frequent symptom in the urine acid-fast bacilli culture positive group was voiding symptoms (48%), and flank pain was the most frequent symptom(48%) in the urine acid-fast bacilli culture negative group. Non visualized kidney or delayed visualization was the main excretory urography findings in 41% of the culture positive and 74% of the culture negative group. In early renal tuberculosis the radiologic changes are minimaland the culture positive rate is high and the main complaints of patients are bladder irritation symptoms, later the disease progresses and the obstructive symptom appear more frequently and the culture positive rate is low. We concluded that the culture positive rate, excretory urography findings and main symptoms are related with the drainage of urine.

3.Site-specific effect of testosterone on bone mineral density in male hypogonadism.

To assess the correlation between the remaining serum testosterone and bone mineral density(BMD), and to determine the effect of exogenous testosterone on BMD in subjects with male hypogonadism, we evaluated the serum testosterone levels and BMDs of the femur neck, Ward's triangle and the spine(L1-4) in 20 subjects with Klinefelter's syndrome and 7 with hypogonadotropic hypogonadism before and after testosterone replacement. BMDs of the femur neck, Ward's triangle and the spine were below the age-matched normal mean at 77.8%(21/20), 74.1%(20/27) and 88.9%(24/27), respectively. There were significant differences in serum testosterone levels and the spinal BMD between the two groups and the BMD of the spine closely correlated with the serum testosterone level (R = 0.63, p < 0.001). Following a mean 11.8 +/- 4.9 months of testosterone replacement, the BMD at all sites increased significantly and the pretreatment difference in spinal BMD between the two groups disappeared. We conclude that, although testosterone may increases the bone density, it has a site-specific effect of maintaining and increasing the bone mass especially at the spine in male hypogonadism.

4.A Study on the Effect of Waste Anestheic Gases on the Sister Chromatid Exchanges of Peripheral Lymphocytes in Operating Room Personnel.

The authors examined the induntivity of sister chromatid exchanges (SCE) in peripheral lum-phocytes on the personnel working in the operating room and analyzed the factors associated with it to obtain data for their monitoring health status. The results obtained were as follows; 1) Average inductivity of SCE in peripheral lymphocytes of operating room perssonnel exposed to anesthetic gases was 7.85 per cell; it was significantly higher than in those who were not exposed (p< 0.05). 2) The inductivity of SCE in peripheral lymphocytes by age group and exposure duration was not statically significant (p<0.05). 3) The inductivity of SCE in the subjects with a smoking habit was significantly higher than those without a smoking habit (p<0.05). 4) There was no significant difference in the inductivity of SCE between the subjects with a drinking habit and those without it (p>0.05). 5) It is possible that the high inductivity of SCE in operating room personnel who both with drink and smoke was due to more the influence of their smoking than their drinking.

6.Clinical Analysis of the 784 Traffic Accident Victims

A retrospective study has been made of 784 cases of road traffic casualties who were treated at the Soon-Chun-Hyang Hospital between July 1980 and December 1981. This study was conducted to find out the nature and pattern of the traffic accident from the clinical and epidemiological standpoints. In additions, the mode of injury from the road accidents were persued which might help to reduce the misdiagnosis rate. The results obtained were as follows: l. In this survey, male dominated in numbers (casualty rate, 70%). The highest incidence was in the age group between 31–40 year (26.5%). The predominance was found in younger casualties (ages between 21–50 years, 72.1% of all casualties). 2. The large number of casualties followed the rush hours. There was a peak between 10PM to midnight. The explanation for this high incidence during the hours was the hurry-up life due to midnight crufew, pedestrian's carelessness and hurry-scurry, and the negligence of traffic rules, the poor labor conditions and weariness due to drivers long continuous working hours, especially in taxi drivers. The highest seasonal incidence was in October. 3. The high incidences were in drivers and students; the drivers were the commonest victims in car passenger accidents and students were injured by pedestrians accidents respectively. The causes of high risk of injury in drivers were intimately related with their vocational aptitudes, the employment status, the driving attitudes and car maintainances; and in students, their carelessness and emotional instability, unsolved mass transportation system and their negligence of traffic rules were listed as the causative factors. 4. Over 51.8% of all casualties were injured by passenger car accidents, and 21.5% were by truck and other types of special vehicles. 5. Approximately 60% of victims were injuried in the road as pedestrian and other 40% were in the car as passengers. In general, the pedestrian injuries were more serious than those sustained in the car as passengers. 6. About 40% of total pedestrian casualties sustained injury at pedestrian's cross road. 7. About 80%of injured drivers were in the drunken state on arrival, and 20.4% of pedestrians were also heavily drunken. 8. The trauma risk was about twice higher in the front-seated passengers than the rear-seated ones in the car. In buses, the highest incidence was in the passengers seated in the front 1/3 of the seats. 9. Orthopaedic (40.5%) and head injuries (38.2%) were the commonest types of the injuries among the hospitalized casualties; about 35% of total patients had the tibial fractures. 10. Of the 784 cases, 479(61.6%) victims arrived hospital within 30 minutes after accident, and among them only 94 cases (12%) were transported by ambulance from the accident site to the hospital.

7.Familial Bilateral Acoustic Neuroma.

The familial occurrence of bilateral acoustic neuroma has rarely been reported in the literatures, and its inheritance is known to be an autosomal dominant trait in association with or without von Recklinghausen's disease. Recently, we experienced an unusual case of bilateral acoustic neuroma, which was familially occurred in mother and her son. A 18-year-old Korean boy was referred to us due to bilateral hearing disturbance and staggering gait of one year duration. There were no stigmata of von Recklinghausen's disease. Neurological examination, simple skull films and vertebral angiograms revealed various evidences of bilateral cerebellopontine angle tumors. At operation, a hen egg-sized firm mass was subtotally removed at the left cerebellopontine angle region and a peanut-sized mass was totally removed at the right cerebellopontine angle region via suboccipital craniectomy. The histological diagnosis was neurofibroma. In family history, 13 years ago his mother was operated on C6-7 neurofibroma at her age of 28 years, and again operated on bilateral acoustic tumors 2 years later. The histological diagnosis was also neurofibroma.

8.MR Imaging of the Prostate and Seminal Tract Using an Endorectal Surface Coil.

A total of 15 adult male (2 cases of normal, 3 cases of benign prostatic hyperplasia, 3 cases of Prostatic cancer, 4 cases of obstructive azoospermia and 3 cases of hemospermia) were evaluated of their prostates and seminal tracts with magnetic resonance(MR) imaging using an endorecta1 surface coi1. On T1W1, the prostatic capsule, periprostatic venous plexus and neurovascular bundle were clearly defined. The zonal anatomy and internal structure of the prostate were clearly visualized on T2W1. The invasion of cancer into the prostatic capsule or the neurovascular bundle could be defined. More of these anatomical depiction, we found some other possible pathophysiologic process such as inhomogeneous signal intensity of seminal plasma in hemospermia and wolffian remnants in vasal agenesis Our results show the value of MR imaging with endorectal surface coil in the evaluation of morphology and pathologic process of the prostate and seminal tract.

9.A case of Large Aneurysm of the Anterior Ethmoidal Artery Associated with an Anatomically Related Arteriovenous Malformation.

The authors report a case of ruptured large saccular aneurysm of anterior ethmoidal artery, ethmoidal artery associated with anatomically related arteriovenous malformation(AVM). The patient was a 49-year-male who presented with severe headache followed by deteriorated consciousness. Computerized tomography and Magnetic resonance imaging revealed intracerebral hemorrhage in left the frontal lobe and intraventricular hemorrhage and contrast enhancing ring-like mass in the frontobasal area. The cerbral angiography showed anterior ethmoidal artery aneurysm with abnormal vascularity in arterial phase. The patient was treated successfully by clipping of the aneurysm and removing of AVM, Among the reference, I have reviewed this paper is the first case report on the anterior ethmoidal artery aneurysm associated with AVM.
