1.Development of a novel antigen capture-ELISA using IgY against porcine interleukin-6 and its application.

Interleukin-6 (IL-6) is introduced as a marker of disease. At present, a variety of method may be used to quantify expression of this protein. Antigen capture-ELISA is a sensitive and accurate quantification method previously used with ovine, rat, and human IL-6 proteins. However, it has never been reported to quantify porcine IL-6 protein using capture ELISA. In this study, we generated and characterized a set of IgY and mono-specific polyclonal antibodies to recombinant porcine IL-6 (rpIL-6), and combining these with a sensitive and specific capture-ELISA for a diagnostic purpose. cDNA encoding the mature protein coding region of porcine IL-6 was cloned and expressed with pQE-30UA expression vector. rpIL-6 was then expressed and purified by using Ni-NTA resin. Protein mass of 24 kDa was found with SDS-PAGE and the identity of the protein was confirmed by Western-blot. Production of polyclonal antibodies against rpIL-6 was performed using the purified rpIL-6 in mice and hens. An antigen capture-ELISA was developed with the antibodies after their extraction. To compare the IL-6 level in the different sanitary state of farms, pig sera were randomly collected and concentration of IL-6 in the sera was measured with the antigen capture-ELISA. The capture-ELISA with the optimal concentration of antibodies, in this study, was able to detect about 10 ng/ml of rpIL-6. IL-6 levels determined with the capture-ELISA in pig sera showed positive correlation with the sanitary states of the farms. These results suggested that the developed antigen capture-ELISA could be a good tool for the screening of microbial infection in pig farms.

2.A comparison study on shear bond strength of 3D printed resin and conventional heat-cured denture base resin to denture relining materials

Purpose:
The purpose of this study was to evaluate the shear bond strength of various 3D printed denture base resins and the conventional denture base resin to various denture relining materials.
Materials and Methods:
For denture base materials, a heatcured (Vertex RS) and two types of 3D printed DENTCA Denture base II, NextDent TM Base) were used. And 4 types denture relining materials (Tokuyama Rebase II fast, Kooliner, Denture Liner, Denture Liner, Lang Jet Denture Repair Kit) with different components were used. It was classified into 12 groups. Adhesion was performed between the resin base and the relining materials in accordance with ISO/TS 11405 standard. The shear bonding strength was measured, and then the adhesion interface was observed with a stereoscopic microscope and a scanning electron microscope. The fracture pattern was investigated through the analysis of the fragment.
Results:
In the 3D printed denture resin group, the shear bonding strength with relining materials was significantly lower than that of the heat-cured resin group (P < 0.05). The group of polymethyl methacrylate -based relining materials, high shear bonding strength was shown regardless of the type of denture. As for the fracture pattern, adhesive fracture appeared in most groups, and cohesive, mixed fracture appeared in some groups.
Conclusion
The polymethyl methacrylate -based denture relining materials showed high shear bonding strength values compared to other denture relining materials. But, for direct methods, it is considered advantageous in terms of shear bonding strength to use a isobutyl methacrylate-based denture relining materials.
3.Congenital Heart Disease and Extracardiac Anomalies.

Purpose: The diagnostic usefulness of fetal echocardiography in congenital heart disease is well known. One of the indications for fetal echocardiography is presence of extracardiac anomalies and such coexistent congenital heart disease may have important implications for obstetric and neonatal management. MATERIALS AND METHODS: 496 women with singleton pegnancies with amgenital anomalies that underwent penatal care and follow-up in Severance Hospital for 5 years from 1991 to 1995 were evaluated for the incidence of congenital heart disease. RESULTS: The incidence of congenital heart disease in fetuses with the associated extracardiac anomalies were as follows; central nervous system 6.9%(11/160), gastro-intestinal system 10.7%(8/75), genito-urinary sysem 4.7%(4/85), others 5.6%(4/72). Thirty-one of eighty-ane fetuses with congenital heart disease were found to have the associated extracardidac anomalies and ten of them chenoscenal abnormalities. CONCLUSION: We recommend that fetuses with congenital extracardiac anomalies sbould be checked for the presence of congenital heart disease and chmmosomal abnormalities, if needed.

4.The Expression of Apoptosis in Lupus Nephritis.

BACKGROUND: A few researches on apoptosis in proliferative glomerulonephritis including lupus nephritis, have been reported on the basis of animal experimental models and human renal biopsy tissues. Systemic lupus erythematosus has been reported as a disease with an apoptosis defect of lymphocyte, so that it leads to the decreased immunologic tolerance and activated autoimmunity. The lupus nephritis is frequently accompanied with a interstitial fibrosis and tubuloepithelial change, as compared to other nephitis. In addition to that, the pathologic evolution from one class to other class is also feasible and these things make us to think that the lupus nephritis is tightly linked to the apoptosis. METHOD: We evaluated the relationship between the apoptosis and pathologic characteristics, which was focused on class IV, in 22 patients with clinically and biopsy proven lupus nephritis by comparing with control specimens. The study on the apoptosis was observed through in situ DNA nick end-labeling method and the review on the medical history. RESULTS: 1) An apoptosis in glomerulus was observed in only five patients (zero in Class II, 50% in Class IV) of proliferative glomerulonephritis. But 14 patients (50% in Class II, 75% in Class IV) were detected on the site of tubulo-interstitium. 2) The apoptosis detected in tubulo-interstitial area was significantly correlated with nephrotic syndrome (P<0.025), but correlations with the complement serum level, renal function and the used amount of steroid was not determined. CONCLUSION: The apoptosis of glomerulus in the lupus nephritis was observed in 50% of proliferative cases (especially, in class IV), but in the area of tubulointerstitium, the higher rate (more than 50%) of apoptotic expressions were observed in class II and class IV. The observation on apoptosis in renal tissue could be another prognostic factor.

5.Prognostic Significance of Cyclin E and p27 in Stage I Non-Small Cell Lung Cancer.

BACKGROUND: Cyclin E plays a pivotal role in the regulation of G1-S transition and could consequently be a deregulated molecule in tumors. The activity of the cdk2-cyclin E complex is increased by degradation of cdk inhibitor p27kip1. Little is known about the expression and prognostic significance of cyclin E and p27 in non-small cell lung cancer(NSCLC). MATERIAL AND METHOD: The expression of cyclin E and p27 in eighty-one cases of resected stage I NSCLC tissues and its relation to major clinico-pathological factors, including histology, differentiation, size of tumor, pleural invasion and survival rate were studied and analyzed. Immunohistochemical analysis with monoclonal antibodies specific for cyclin E and p27 were performed by ABC method. RESULT: Expression rates of cyclin E and p27 in stage I NSCLC tissues were 29.6% and 28.4% respectively. Cyclin E was expressed higher in cases of pleural invasion(p=0.04), and p27 was expressed higher in diameter of tumor less than 3cm(p=0.015). The 5-years survival rate was lower in cases of positive expression of cyclin E than in cases of negative expression of cyclin E(44.4% vs 68.2%, p=0.015), and the 5-years survival rate was 72.2% in positive expression of p27 and 56.2% in negative expression of p27(p=0.09). The 5-years survival rate was higher in negative expression of cyclin E and positive expression of p27 than in cases of positive expression of cyclin E and negative expression of p27 (73.5% vs 36.3%, p=0.0029). In multivariate analysis, expression of cyclin E was an unfavorable prognostic factor(RR=3.578, p=0.006) and p27 was a favorable prognostic factor(RR=0.183, p=0.019) independently. CONCLUSION: Cyclin E and p27 may play a pivotal role for the biological behavior of stage I NSCLC, so that the expressions of cyclin E and p27 may be new prognostic markers.

6.A Case of Chronic Invasive Rhinocerebral Mucormycosis with Pott’s Puffy Tumor

Mucormycosis is one of the most fatal and rapidly progressive fungal infections in humans; among its many forms. rhinocerebral mucormycosis is the most common. It is known to occur as opportunistic infection in patients with uncontrolled diabetes, metabolic disorders, organ transplantation, or autoimmune disease with prolonged steroid use. Pott’s puffy tumor is the subperiosteal abscess of the frontal bone caused by trauma complication or frontal sinusitis. It is considered as a very rare complication since the dawn of antibiotic treatments. We report a case of chronic rhinocerebral mucormycosis involving Pott’s puffy tumor in a patient receiving immunosuppressive therapy after lung transplantation.
7.A case of nodal marginal zone B-cell lymphoma of the lower eyelid

A marginal zone B-cell lymphoma is a type of B-cell lymphoma which is normally located at the margins of the secondary lymph node follicles. According to 2008 World Health Organization (WHO) classification, there are three main types classified according to the location of invasion: nodal lymphoma in the lymph nodes, splenic lymphoma in the spleen, and extranodal lymphoma in other locations. Recently, we have experienced a rare case of primary nodal marginal zone lymphoma that arose in the lower eyelid. Therefore, we report this case with a review of literature.
8.Role of Crural Diaphragm after Esophagogastrectomy.

BACKGROUND: The high pressure zone(HPZ) at the gastroesophageal junction is an important barrier for prevention of gastroesophageal reflux. Smooth muscle layers in the lower esophageal sphincter mainly contributes to HPZ at the throacoabdominal junction. The purpose of this study was to investigate the manometric characteristics of the thoracoabdominal junction in patients after surgical removal of the lower esophageal sphincter. MATERIAL AND METHOD: Twenty two patients with prior esophagogastrectomy(10 Ivor-Lewis method and 12 left thoracotomy) and 30 normal adults(control group) were studied manometrically. RESULT: Esophageal manometry showed a HPZ and pressure inversion point distal to the anastomosis in 12 of 22 patients(2 of 10 patients with Ivor-Lewis method and 10 of 12 patients with left thoracotomy) and a HPZ in 30 of 30 normal adults. The location of HPZ from nostril was not significant different between the two groups(42.5+/-0.9cm in patients and 43.9+/-2.1cm in the control), while the length of HPZ was shorter in patients than in the control(2.13+/-0.6cm vs 2.83+/-0.59cm). By SPT and RPT, pressures of HPZ at rest were lower in patients(13.78+/-1.63mmHg, 28.58+/-6.06mmHg) than in control(20.3+/-4.95mmHg, 42.80+/-15.91mmHg). The HPZ relaxed partially in response to deglutition(84.4% in patient, 90.5% in control group) and contracted in response to increased intra- abdominal pressure induced by leg lifts(HPZ/ Intra-abdominal pressure= 1.81+/-0.23 in patient, 2.13+/-0.58 in control group). CONCLUSION: This study shows an HPZ at thoracoabdominal junction after surgical removal of the lower esophageal sphincter. It may be important to perform a crural myoplasty during esophageal reconstruction after esophagogastrectomy because crural diaphragm acts as sphincter like HPZ at the thoracoabdominal junction.

9.Bleeding Volume after Surgery for Trochanteric Fractures of the Femur in Patients Treated with Antiplatelet Agents: Comparison according to Surgical Timing.

PURPOSE: We evaluated the bleeding volume after surgery for trochanteric fractures of the femur in patients treated with antiplatelet agents according to surgical timing. MATERIALS AND METHODS: We selected 20 patients who had trochanteric fractures of the femur treated with antiplatelet agents from January 2009 to June 2010. Group I included 9 patients who discontinued antiplatelet medication and had delayed operations at an average of 6.5 days and Group II included 11 patients who underwent early operations within 24 hours. Group I included 2 males and 7 females; their average age was 77.8 years (range 59~86). Group II included 4 males and 7 females, with an average age of 73.5 years (range 61~84). We compared the two groups' volume of intraoperative bleeding, the preoperative and postoperative hemoglobin levels and the volume of postoperative transfusion. The Mann-Whitney U test was used for statistical analysis. RESULTS: The volume of intraoperative bleeding was 88 ml in group I and 106 ml in group II (p>0.01). The difference in the hemoglobin was a decrease of 2.4 mg% in group I and a decrease of 2.2 mg% in group II (p>0.01). The volume of postoperative transfusion was 0.6 pints in group I and 1 pint in group II (p>0.01). CONCLUSION: We found a similar bleeding volume regardless of operative timing after surgery for trochanteric fractures of the femur in patients treated with antiplatelet agents.

10.Cord-like Atresia of the Abdominal Aorta Due to Takayasu Arteritis in Middle Aged Woman: A case Report.

Takayasu arteritis is a chronic non-specific inflammatory arteriopathy that involves primarily the first branches of aortic arch but may also affect the aorta and any of its primary branches. A characteristic trait of the disease is that most of the patients are young females of mainly Asian and South American origin. Recently, we experienced a rare case of Takayasu arteritis which showed a cord-like atresia of abdominal aorta just below renal arteries in 52-year-old woman. We performed a side to side bypass graft from descending thoracic aorta to abdominal aorta just above the aortic bifurcation with a 18mm PTFE(polytetrafluoroethylene) vascular graft. The postoperative course was uneventful.
