The safety and efficacy of the CO2laser as a replacement for the steel scalpel is well documented in plastic surgery, but the use of the laser blepharoplasty has been controversy. This study compares and contrasts the use of the CO2 laser with conventional method in cosmetic upper eyelid blepharoplasty. Thirteen Patients underwent blepharoplasty in a paired comparison study. Four patients(Group A) underwent blepharoplasty by laser, another four patients (Group B) underwent blepharoplasty by conventional method, and the other five patients(Group C) underwent upper eyelid blepharoplasty using the laser on one eye and a conventional method on the other side. Preoperative factor(set up time, equipment), intraoperative factor(actual operation time, bleeding, visibility), postoperative factor(swelling, ecchymosis, immediate & short term result) were evaluated. The advantage of using the CO2 laser rather than the steel scalpel in blepharoplasty are less bleeding and superior intraoperative visibility. The disadvantage of using the laser compared with steel scalpel include the cost of purchasing and maintaining the laser equipment, the need for additional and extensive laser training for surgeons and assistants, and the need for two assistants rather than the one needed for scalpel surgery There were no complications with either the scalpel or the laser.

For many years the standard surgical treatment for carpal tunnel syndrome has been division of the transverse carpal ligament under direct vision through a palm incision. Although the effectiveness and Patient acceptability of the conventional open carpal tunnel release are good, disability from a painful palm cutaneous neuroma, and prolonged swelling are reported Endoscopic carpal tunnel release seems to result in less postoperative morbidity, and minimal serious complications. Because endoscopic carpal tunnel release leaves the overlying skin, the subcutaneous fat and the palmer fascia and its attachments to the thenar and hypothenar musculature intact its proponents maintain that their patient have less scar tenderness, quicker recovery of grip strength, and earlier to return activities of daily living and work The endoscopic approach(Agee single portal technique), which includes specific localization of the hook of hamate, flexor retinaculum and the superficial palmar arch utilizing topographical landmarks, avoids entry into Guyon's canal and common digital nerves. We experienced four cases of carpal tunnel syndrome. In four cases, patients with carpal tunnel syndrome have been performed endoscopic carpal tunnel release with minor complication.

Background and OBJECTIVE: There has been a few case reports of anaphylaxis due to honeybee in Korea. In order to observe the clinical feature of bee sting anaphylaxis. Moderials and methods: Six patients living in Kyunggi province area were referred under history of anaphylaxis after the bee sting. Atopy was defined by skin prick test result to common inhalant allergen. Serum specific IgE antibody to each bee antigen was detected by radioimmunoassay to identify the causative bee. RESULTS: All six cases were female. Three had atopy and four had combined allergic diseases such as allergic rhinitis, asthma, and urticaria. The etiologic bees consisted of yellow jacket (6 cases), paper wasp (4 cases), yellow hornet (3 cases), white faced hornet (1 case) and honey bee (1 case). Four cases had experienced anaphylaxis after ant bite and they showed positive result on specific IgE to imported fire ant. Specific immunotherapy against causative bee venom was begun using bee venom extracts from Bayer (USA) based upon results of specific IgE anti-body to bee venom. CONCLUSION: The yellow jacket is the most common cause of bee venom anaphylaxis in this area. Further studies will be needed to evaluate possible cross-reactivity between bee and ant venom.

Advancement of a fornical conjunctival flap is the method used for bleb reconstruction after excision of necrotic leaking bleb. The authors have experienced a case of corneal bleb at 4 months after necrotic leaking bleb excision and advancement of fornical conjunctiva. The patient complained of irritation. The authors sutured conjunctival bleb over the cornea tightly to the limbus with full corneal thickness. Postoperatively corneal bleb and irritation symtom disappeared and visual acuity improved, but at postoperative 1 month, small corneal bleb recurred, which was punctured with 26G needle and Nd: YAG laser. Subsequently it disappeared and did not recur.

Mitomycin C has improved the success rate of glaucoma filtering surgery in patients at high risk for surgical failure. However chronic hypotony is marked by decreased vision and a late-onset leaking bleb after filtration surgery using mitomycin C. Bleb excision and conjunctival advancement is the method of choice to repair bleb leakage and chronic hypotony. Five eyes from five patients were received glaucoma filtration surgery with topical mitomycin C. All of the patients' blebs were avascular and transparent. The reasons for bleb excision were two spontaneous bleb leaks, two traumatic bleb leaks and one case of severe irritation. The mean follow-up period was 18.4 +/- 8.3 months (ten to 29 months). Cataract surgery was combined in one eye. Postoperative intraocular pressure (IOP) increased from 2.3 +/- 1.5 mmHg to 9.5 +/- 3.7 mmHg at nine months postoperatively in four eyes. It went from 28 mmHg to 40 mmHg in one patient with uveitis, for whom a second trabeculectomy with mitomycin C; 0.4 mg/ml for 3 minutes, was performed. After surgery, IOP decreased to 4 mmHg in three months. Postoperative visual acuity improved four snellen lines in three eyes. A partially avascular bleb recurred in three eyes, a corneal bleb in one eye and blepharoptosis, which disappeared spontaneously at four months postoperatively, in one eye. Necrotic bleb excision and advancement of fornical conjunctiva were useful methods to increase IOP and to improve visual acuity for the patient experiencing irritation symptoms, and for leaking blebs, and hypotonic maculopathy.

The Mayer-Rokitansky-Kuster-Hauser (MRKH) syndrome is comprised of vaginal atresia with other variable Mullerian duct abnormalities. Its features include an absent or very short vagina and a uterus that can be absent or immaturely formed. Female with MRKH syndrome have functioning ovaries, normal external genitalia and the typical 46, XX, karyotype. MRKH syndrome is the second most common cause of primary amenorrhea, usually remains undetected until the patient presents with primary amenorrhea despite normal sexual female development. We report a case of MRKH syndrome, associated with ovarian follicular cyst, in a 14-years-old girl who visited for primary amenorrhea and sexual precosity.

Common complications in a patient with IgG subclass deficiency include recurrent respiratory infections and concomitant inflammatory lung disease. The most effective therapy in these patients is the administration of intravenous immunoglobulin. The authors report a case of severe aspirin-sensitive asthma and recurrent pneumonia with combined IgG1, and IgG3 subclass deficiency in a 19-year-old man. The patient was treated with 0.4g/kg at monthly intervals for 6 months, and is still receiving 0.2g/kg every 2 weeks at our clinic. After the replacement of intravenous immunoglobulin, the patient has clinically improved.

Glaucoma cases undoubtedly represent one of the biggest ophthalmic problems in the field of public health. We analysed statistically 456 cases of the glaucoma patients among 35150 outpatients who visited to our eye department from Jan. 1982 to Dec. 1986. Diagnosis was performed by intraocular pressure, visual acuity, visual field, fundus and gonioscopic examination. The prevalence of the galucoma was 1.3% which was similar as other reports. The following results were obtained. 1. Among the 456 cases including 218 male and 238 female patients, there was no significant sexual difference statistically. 2. The age group over 40 years old occupied 65.9%. 3. The age group 4th and 6th decade in male and 6th and 7th decade in female occupied higher prevalence rate. 4. Open angle glaucoma has the highest prevalence rate. 5. Visual acuity was not significantly correlated to the latency. 6. Intraocular pressure appeared not to have significant correlation to the latency. 7. There was no significant difference in left and right eye. 8. There was significant correlation between the visual acuity and the intraocular pressure.

In recent days, cataract surgery has trend to reduce postoperative astig-matism and acquired early visual rehabilitation.Phacoemulsification with small incision and insertion of foldable intraocular lens has been used popularly, but its high cost and intraoperative or postoperative complication especially in old age with dense nucleosclerosis could be detrimental to patients. Postoperative corneal astigmatic changes and visual rehabilitation were evaluated after extracapsular cataract extraction with 7 mmscleral tunnel incision (Group I, 7 mm-ECCE, 28 eyes)by comparing with those after phacoemulsification with 3.5 mmscleral tunnel incision (Group II, 3.5 mm-Phaco, 41eyes).One piece PMMA intraocular lens was inserted in group I, while foldable intraocular lens in group II.Surgically induced corneal astigmatism was calculated by Cravy's vector analysis.There was no statistically signifi-cant difference between group I and group IIfor surgically induced corneal astigmatism (Kt)after postoperative one month (p>0.05).In group I and group II, corneal astigmatism was with-the-rule in early postoperative days, and changed to against-the-rule in postoperative 3 months.There was no statistically significant difference between group I and group IIincorneal astigmatism during all follow-up period (p>0.05). Therefore, 7 mm-ECCE could be an effective method for old patient with high grade nucleosclerosis or when phacoemulsification was unavailable.
