1.Association Between Hypertension Management and Blood Pressure Screening Among Adults in 30s and 40s.

BACKGROUND: Although adults in 30s and 40s are at risk for hypertension management due to low rates of awareness, treatment, and control of hypertension, there has been a lack of study on hypertension management for this population. Thus, the purpose of this study is to examine blood pressure screening and other associated factors with hypertension management in terms of awareness, control, and treatment. METHODS: Secondary data analysis was performed on the data from the Fourth Korea National Health and Nutrition Examination Survey in 2007-2009. To examine whether blood pressure screening affects hypertension management in 929 subjects aged 30s and 40s with hypertension. Other factors, such as socio-demographics, health status, and health behaviors, were also included for data analysis. Chi-square test and multiple logistic regression analysis were carried out using SAS 9.1. RESULTS: Hypertension awareness rates were high in those who had diabetes mellitus (odds ratio [OR] 2.23; 95% confidence interval [CI] 1.27-3.90), self-rated their health status as poor (OR 2.89; 95% CI 1.82-4.58), and received a blood pressure check up (OR 5.96; 95% CI 2.90-12.25). Hypertension treatment rates were high in women (OR 2.50; 95% CI 1.47-4.27), and high in those who had diabetes mellitus (OR 2.59; 95% CI 1.49-4.47), self-rated their health status as poor (OR 2.53; 95% CI 1.54-4.14), self-reported as past smoker (OR 2.01; 95% CI 1.22-3.29), and received a blood pressure check up (OR 12.64; 95% CI 4.87-32.77). Hypertension control rates were high in women (OR 2.47; 95% CI 1.29-4.72), and high in those who self-reported as past smoker (OR 2.46; 95% CI 1.35-4.50), and received a blood pressure check up (OR 9.24; 95% CI 3.15-27.15). CONCLUSIONS: For effective hypertension management in 30s and 40s, we should encourage this population to have a regular blood pressure screening.

2.Deep Neck Abscesses in Children and Adolescents: 10 Year Experience in Two General Hospitals.

PURPOSE: The purpose of this study was to review our recent experiences with deep neck abscesses in children and adolescents and to provide helpful information in treatment and diagnosis by comparing them with those in other available literatures. METHODS: Medical records of 36 children and adolescents admitted for deep neck abscess at two hospitals from January 2000 to October 2010 were reviewed retrospectively. RESULTS: Male to female ratio was 1.4 : 1 and the mean age was 6.5 years. Painful neck swelling and fever were the most frequent symptoms in patients under thirteen years of age whereas trismus and headache were frequent symptoms in patients over fourteen years of age. Submandibular space was the most common site of deep neck abscess in patients under thirteen years of age, whereas peritonsillar space was the most common site in patients over fourteen years of age. The results of bacterial cultures were positive in 61.5% of drained cases. Staphylococcus aureus was the most commonly identified bacteria in 6 patients (37.5%) and 5 of them were under 2 years of age. Twenty six patients received surgical drainage while the others were treated with antibiotics only. There were no statistically significant differences in the durations of admission, fever after admission, and antibiotic treatment between surgical and medical treatment groups. CONCLUSION: The common sites of deep neck abscess, associated symptoms, and causative organisms were different between children and adolescents. As there were no differences in durations of admission, fever, or antibiotics treatments between surgical and medical treatment groups, surgical drainage may be avoided by early recognition and suspicion. However, if there is no improvement of symptoms or size of abscesses within 48-72 hours of antibiotic treatment, surgical drainage should be considered.

Microsportcm gypseum is a geophilic fungi abundant in soil throughout the world. The infections may occur either from animal to man(zoophilic) or from soil to man (geophilic). Patierit of 13-year-old Korean male sought medical attention with skin lesion on scalp which showed scaly patch with broken hairs and a round pilar cyst like nodule. Identification of Microsporum gypseum was made by culture on Sabouraud agar media and microscopic exarnination.

5.One-Step Ventro-Posterior Fusion via Transpedicular Approach for L1 Burst Fracture by Use of Expandable Cage.

Accepted methods of treatment of lumbar burst fractures include conservative therapy, posterior reduction and instrumentation, and anterior decompression and instrumentation. Surgery aims at the correction of the kyphotic deformity and at the decompression of the spinal cord thereby reducing pain and allowing early patient mobilization. Posterior-only procedures usually rely on ligamentotaxis or manual tamping of bone fragments for decompression of the spinal canal. Transpedicular corpectomy allow for circumferential surgery through a single posterior approach. The authors use an expandable cage to restore the normal spinal curvature and to prevent the kyphotic deformity.

6.Comedonal Darier's Disease.

Darier's disease is a well-known genodermatosis characterized by recurrent waxy, hyperkeratotic papules usually occurring over the seborrheic area. The major histopathological changes are characteristic acantholysis and dyskeratosis which are diagnostic of the disease with typical clinical features. There are less common variants including the hypertrophic, vesicobullous and linear type. However, comedonal lesions are very rare. We report an unusual case of Darier's disease, which showed prominent comedonal papules and plaques over the face, scalp and upper trunk with the typical findings of Darier's disease.

7.SURFACE ANALYSIS OF CERCE RESTORATIONS POLISHED BY DIFFERENT TECHNIQUE.

This study was performed to investigate the surface roughness of the Cerec Vita Mark II polished by various polishing techniques, compare with that of the Vintage enamel porcelain glazed by high temperature glazing technique. All of the Cerec specimen were finished with sequential use of high speed diamond burs(grit 45, 30 and 15microneter). The groups were divided into 5 groups : Group I : Cerec Vita Mark II block specimens polished with Sof-lex discs. Group II : Cerce Vita Mark II block specimens polished with Two Striper MPS. Group III: Cerce Vita Mark II block specimens polished with Enhance. Group IV: Cerce Vita Mark II block specimens polished with Porcelain laminate polishing FG kit. Group V: Vintage enamel porcelain glazed by high temperature glazing Technique. Each group was consisted of 10 specimens. The surfaces produced were examined quantitatively using a laser specular reflectance machine(Perthen RM600-s, Feinpruf Perthen GmbH., Germany) and qualitatively under SEM(JSM-5400, JEOL, Japan). The Results were as follows : 1. The arithmetic mean roughness value(Ra) in groups 1, 2, 3 and 4 was higher than that of group5. There was statistically significant difference(P<0.05). 2. The arithmetic mean roughness value(Ra) decreased in the following orders : group 1, group 2, group 4, group 3 and there was no statistically significant difference between group 1 and group 2, group 3, and group 4. There was statistically significant difference among group 1, 2, and group 3, 4 and group 5(P<0.05). 3. The maximum individual peak-to-valley-height(Rmax) decreased in the following orders : group 2, group 1, group 4, group 3, group 5 and there was no statistically significant difference between group 1 and 2, group 1 and group 4, group 3 and group 5. There was statistically significant difference among group 1, 2, and group 1, 4 and group 3, 5(P<0.05). 4. The treated surfaces of group 5 had smoother surface than that of groups 1, 2, 3, 4 with SEM.

9.Efficacy of Fluvastatin in Patients with Hypercholesterolemia

Background: Fluvastatin is the first entirely synthetic 3-hydroxy-3-methylglutaryl-coenzyme A(HMG-CoA) reductase inhibitor. Clinical data indicate that this agent exhibits the proven efficacy of its class and also has some theoretical advantages in safety for long-term use because of its unique pharmacololgic property consistent with hepatoselectivity(i.e., low systemic exposure). This study is to evaluate efficacy and safety of fluvastatin in hypercholesterolemic patients in Korea. Methods: An open clinical trial with fluvastatin was conducted in 31 subjects who continued to have high blood cholesterol levels of 6.21 mmol/L(240 mg/dl) or greater after 1 month of lipid-lowering diet plus single blind placebo period. Fluvastatin was administered for 8 weeks with the initial dose of 20 mg per day and if serum cholesterol levels did not fall below 5.20 mmol/L(200 mg/dl) after 4 weeks the dose was increased to 40 mg per day for the second 4 weeks. On each visit every 4 weeks they underwent interview and laboratory tests about side effects and tolerability.
