2.A Case of Celiae Plexus Bloek with Alcohol for Relief of Intractable AMominal Pain due to Gastric Cancer .

Successful block of the celiac plexus denervates the denervatives of the foregut and therefore can successfully interrupt nociceptive stimuli from the pancreas, stomach, liver, etc. Although permanent neurolytic block is much less successful for malignant chronic pancreatitis syndrome, it is specifically useful for treatment of intractable pain that often accompanies carcinoma of the pancreas or other upper abdominal visceral tumors. This was a case report in which intractable pain due to malignant gastric cancer was successfuly controlled by the permanent neurolytic celiac plexus block.

3.Postoperative Pain Control in Laminectomized Patients with Epidural Morphine .

Many studies have shown that epidural narcotics produce prolonged analgesic action. We instillated 2mg of morphine on the dura just before wound closure which was exposed for lumbar laminectomy. Sixty patients scheduled for laminectomy were devided into two groups i.e., morphine group (30 patients) and control group(30 patients). The results of this study were as follows: 1) In morphine group, 17 patients (56.6%) tolerated the postoperative pain without narcotic injection for over 20 hours, but 13 patients(43.3%) of the control group required narcotic injection within 5 hours. 2) Severe respiratory depression was noted in two cases from the morphine group. 3) It is a very simple and effective method for postoperative pain control.

4.The Operative Treatment of the Resistant clubfoot: Comparative study between modified Turco's Operation and combining calcaneocuboid release

The 17 patients with 22 resistant clubfeet were treated with modified Turco's operation with or without combining release of calcaneocuboid joint at Department of Orthopaedic Surgery, Hanyang University School of Medicine between 1896 and 1991. They were analysed preoperatively and postoperatively with chinical and radiological methods. In patients who were treated with combined procedures, the latetal talocalcaneal angle and anteroposterior talo-first metatarsal angle in radiological findings were showed better improvement(p < 0.05), and anteroposterior talocalcaneal angle was also showed better improvement but not significant statistically, than those who were treated with modified Turco's operation alone. The final results of combined procedures were better than that of modified Turco's operation. And the most common characteristic physical findings in resistant clubfoot were small heel, deep longitudinal crease on the medial aspect of midfoot area and a single transverse crease on the posterior heel just proximal to the insertion of the tendocalcaneus. So we suggest these physical findings may be expectance for the resistance of the congenital clubfoot to conservative treatment, but it need further comparative study.

5.CT Analysis of Intratumoral Gas Formation after Hepatic Tumor Embolization.

PURPOSE: To evaluate the prevalence and the patterns of sterile gas shown at computed tomography(CT) after transarterial embolization(TAE) for primary hepatic tumor. MATERIALS AND METHODS: Among 102 patients who performed TAE for hepatoma, thirty-four in whom follow-up CTwas underwent constituted the basis of our study. At CT, we evaluated the patterns and locations of intratumoral gas. We also reviewed the clinical data to exclude an infectious origin of intratumoral gas. RESULTS: Of 34 patients, intratumoral gas was detected in 11 patients(32%), in all of whom Gelfoam was used as an embolic material. The initial tumor size measured at pre-TAE CT was larger in patients with intratumoral gas than in patients without it(p < 0.005). No specific patterns or locations of intratumoral gas were noted on CT scans. No patients had clinical signs and symptoms that suggested infection. CONCLUSION: Intratumoral gas formation without clinical evidence of infection is not an infrequent finding after TAE for hepatoma, especially when Gelfoam is used and when the tumor is large in size. This finding may be a part of postinfarction syndrome and should not be misinterpreted as an postprocedural abscess formation.

6.Foreign Body Induced Bladder Stone After Bladder Neck Suspension: A Case Report.

Bladder stone formation may be associated with the intravesical foreign body. Frequency of bladder foreign body in female is less than male. We present a case of bladder stone formation for nylon suture materials in a 47-year-old female patient who has been treated by bladder neck suspension for stress incontinence. Patient was successfully treated by surgical removal of suture materials without recurrence of stress incontinence.

7.US-guided percutaneous biopsies with a biopsy gun.

Core tissue for histologic study is believed by many pathologists to be more diagnostic than material from needle aspiration. Recently introduced automated biopsy gun simplifies core biopsies with increased quantity and quality of samples. Authors performed 38 percutaneous biopsies from 38 patients with 18G automated biopsy guns under US guide. Diagnostic target tissues were obtained in 33 biopsies(87%), inadequate tissues in 4 (11%), and adequate but not of target tissue in 1(3%). There was no major complication requiring treatment, but pain needing analgesics and pain with nausea/vomiting were experienced in 2 and 1 biopsies respectively. Average number of needle passes was 1.5(1-5) We concluded that US-guided gun biopsy was a easy and safe way to obtained tissue samples of good quantity and quality, especially useful in hospitals without constant availability of specialists in cytopathology.

8.Nerve Compression Syndromes of the Upper Extremities

Nerve compression syndromes can be caused by compression of the peripheral nerve from interior or exterior of the nerve. In the upper extremities, brachial plexus, ulnar nerve and median nerve can be compressed in thoracic outlet syndrome, cubital tunnel syndrome and carpal tunnel syndrome respectively. Because these syndromes are caused by compression, decompressive management can be expected to be the principle of treatment. From Jan. 1980 to Dec. 1988, 94 patients were admitted and treated for nerve compression syndromes of the upper extremities in Seoul National University Hospital. Among these, 52 patients suffered from carpal tunnel syndrome and operation was performed to 86 hands. Twelve patients suffered from thoracic outlet syndrome and 25 from cubital tunnel syndrome. Ten patients were treated with conservative method and 84 with decompressive surgery. Satisfactory results were obtained in 92.3% of carpal tunnel syndrome and 84% of cubital tunnel syndrome. Of 4 operated cases of thoracic outlet syndrome, 3 cases were satisfactory.

9.CT Measurement of Wall Thickness of Early Gastric Cancer: Correlation with Upper GI Series.

PURPOSE: The purpose of this study is to determine the detection rate of early gastric cancer and to measure the focal gastric wall thickening in computed tomography. MATERIALS AND METHODS: From November 1991 to November 1993, 19 patients (age;33--76 years, male ;female:4;5) with surgically proven early gastric cancer were examined by upper gastrointestinal series and abdominal computed tomography. Twenty lesions in these patients were included in this study. Computed tomography was performed with conventional technique using gastrografinR (17 cases) or water filling (2 cases) method in supine position. Additional scans were obtained either in right down decubitus (15 cases) or supine position (4 cases) with an ingestion of gas forming agent. We determined the detection rate of early gastric cancer at computed tomography and measured the maximum thickness of the lesion. RESULTS: The pathologic types of the early gastric cancer were type lib + IIc in 5 cases, type IIc in 6, type III in 3 and type I, I + IIc, Ila, lib, lib + III and IIc + III in one, respectively. Seventy-five percent (15 cases) of early gastric cancer was detected at upper gastrointestinal series and fifty percent (10 cases) at computed tomography. Computed tomography could detect 100% of elevated carcinomas (3/3) and 41% of depressed carcinomas (7/17). The maximum thickness of the lesion ranged from 7 mm to 11 mm (mean ;8mm). CONCLUSION: In a well-distended stomach, CT could detect focal gastric wall thickening (mostly, less than 1 cm) caused by early gastric cancer in half of the cases.

10.A Case of Lupus Vulgaris Following BCG Vaccination.

We report a case of lupus vulgaris following BCG vaccination in a 12-year-old male. The patient was vaccinated against tuberculosis on right shoulder 6 months ago. The erythemstous papular eruptions began to develop on the BCG vaccinated site 2 weeks after vaccination. The eruptions progressed to be pruritic, erythematous plaque which was flat, crescent shaped, and centrally covered by adherent scales for 6 months. Histopathological findings revealed tubercles and tuberculoid structures composed of epitheloid cells and Langhans giant, cells in the dermis. Caseation necrosis within the tubercles was slight or absent. In addition, there was an infiltrate of mononuclear cells around the tubercles. The skin lesions cleared after 6 months of treatment with oral isoniazid 300mg daily.
