4.A Study on the Factors Influencing the Accuracy of Chest Compression during Cardiopulmonary Resuscitation Trying in a Moving Ambulance.

PURPOSE: The study offers the recommended guidelines for EMTs to improve their CPR techniques in an ambulance by analyzing external factors to perform more effective and accurate chest compression in a moving ambulance while patients are being transported to hospital. METHODS: Seventeen of the first class EMTs at GyeongGi- Do agreed to participate in the research. In order to analyze the factors that have influenced the accuracy of chest compression, the type of ambulance (Van-type, Truck-type), velocity of a moving ambulance (60, 70, 80 km/hr), chest compression posture (standing, knee supported) and assistive devices (Main stretcher car, CPR board, Long Spine board) for chest compression were compared to each other. The research was conducted according to the phased procedure in an attempt to evaluate the difference between the chest compression posture and the assistive devices in appropriateness of chest compression. RESULTS: The findings of this study included that chest compression posture and the use of assistive devices influenced the effectiveness of chest compression. With regard to compression depth, the chest compression that occurred on the main stretcher in ambulances while in the standing position, were the least effective. Chest compression that occurred on a Cardiopulmonary Resuscitation board (CPR board) was the most effective in the supported posture. In addition, the accuracy of chest compression was higher in a supported posture than in a standing posture. In both postures, the accuracy of chest compression was higher when a CPR board or a Long spine board (LSB) were used, than when a main stretcher was used. CONCLUSION: The study showed that the chest compression posture and the use of assistive devices had an effect on appropriateness of chest compression. As for posture during chest compression, the accuracy of chest compression increased when knees were supported. Also, the study suggested that the use of CPR board among other assistive devices for chest compression was the factor contributing to an increase in the effectiveness of chest compression depth.

5.A Case of Benign Ovarian Steroid Cell Tumor with Huge Ascites and Elevated Serum CA125.

Steroid cell tumor of ovary, first described as lipid cell tumor, is rare lesions composed entirely of cells resembling typical steroid hormone - secreting cells, that is lutein cells, Leydig cells, and adrenal cortical cells. Steroid cell tumors oftcn secret androgen and manifest themselves with symptoms of virilization. Other presentations include abdominal swelling or pain, menstrual dysfunction, postmenopausal bleeding, or rarely ascites. We experienced a case of right ovarian steroid cell tumor, not otherwise specified(NOS), manifested hirsuitism and amenorrhea in 49 - year - old patient. The tumor was about 5 cm in size, and associated with huge ascites (l3,000 ml), both pleural effusion, and elevated serum CA 125. We present a case of Meigs syndrome associated with benign ovarian steroid cell tumor with a brief review of the literature.

6.Expression Pattern of Tumor Progression and Metastasis-related Gene Proteins - CD44H, CD44v6, erbB-2, and p53 -in Gastric Carcinoma.

Immunohistochemical studies of the molecules associated with gastric tumor progression and metastasis were done to evaluate their relationship with known prognostic factors and their usefulness in assessment of the progression of gastric carcinoma in 127 gastric carcinoma tissues. The 4 antibodies used in this study were CD44H, CD44v6, erbB-2, and p53. The CD44H expression was detected in 76 (59.8%), CD44v6 in 63 (49.6%), erbB-2 in 18 (14.2%), and mutant p53 in 98 (77.2%) out of 127 cases of gastric carcinomas. There was no significant correlation between the expression rates of each four proteins. The expression rates of all 4 proteins were not significantly correlated with age and sex of the patients and lymph node metastasis, but the correlation between CD44v6 expression and the depth of tumor invasion and tumor stage was significant (p<0.05). These results suggest that CD44v6 is closely associated with tumor invasion, and high levels of CD44H, erbB-2 and p53 are associated with tumorigenesis of the stomach as they are highly expressed in early as well as in advanced gastric carcinomas. The findings also support the conclusion that the loss of control of alternative CD44 mRNA splicing resulted in production of CD44v6 splicing variant in tumor cell facilitates tissue invasion by increased adherence of the tumor cell to an extracellular matrix or by tumor cell migration. It can be expected that CD44v6 overexpression in tumor cells appears to be an important prognostic indicator for gastric tumor progression.

7.A Case of Surgical Arterial Ligation in Combination with Transarterial Embolization in the Management of Recurrent Uterine Arteriovenous Malformation.

Although uterine arterovenous malformation(AVM) is a rare cause of menorrhagia or postmenopausal bleeding, it is important to consider that curettage may precipitate life-threatening hemorrhage and therefore it is contraindicated when AVM is suspected. Special investigations such as hysteroscopy, Doppler flow ultrasound and angiography are important diagnostic tools, and transarterial embolization(TAE) has replaced hysterectomy as the treatment of choice in women who wish to retain their fertility. But due to the high incidence of collateral vessels, recurrence of the vascular malformation after TAE is common. We have experienced two cases of AVM, one of which was managed by TAE in combination with surgical arterial ligation.

8.A Case of Fibrous Histiocytoma in Cornea and Corneosclerallimbus.

Fibrous histiocytoma are complex group of tumors that feature cells resembling fibroblasts and histiocytes.In a case of fibrous histiocytoma involving the cornea and corneolimbus, we evaluated the finding of H-E, Masson-trichrome, reticulin, CD68, alpha-1-antitrypsin antibody and smooth muscle actin antibody stain after the localized excision in 26 years old male patient. In H-E stain, tumor cell included many multinucleated giant cells having abundant eosinophilic and foamy cytoplasm infiltreated by numerous lymphocyte, CD68 & alpha-1-antit-rypsin antibody staining resulted in diffuusely positive finding. Therefore examination. Good result was noted after complete excision at 6 month follow-up.
