1.Evaluation of Nutritional Status of Inpatients with Medical Health Problems.

PURPOSE: To investigate nutritional status of inpatients by using subjective. and objective evaluation methods and to find the relationship between them. METHOD: The subjects were 101 inpatients with medical health problems at a university hospital. Nutritional status was evaluated by the Subjective Global Assessment (SGA) and physical assessment including percentage of weight loss, serum albumin, hemogloin, and hematocrit. Data were analyzed using frequency, percentage, mean, Kendall's tau. RESULTS: Grouping by the SGA, 61.4% were classified as severe malnutrition group. When applying the objective methods{physical assessment). 1.9~42.6% were diagnosed as malnutrition each item. The percentage of weight loss during previous 1-6month(r=.43~.54. P=.0001), serum albumin(r=-.26, P=.0003), hemoglobin of male(r=-.38, P=.0001), and hematocrit of male(r=-.34, P=.0001) were significantly correlated with SGA score. The coincidence rate of nutrition evaluation between the objective methods and SGA were 27.7 35.6%, 20.8%, 47.5%, 58.4% in percentage of weight loss, albumin, hemoglobin, and hematocrit. CONCLUSION: These findings showed a majority of inpatients were exposed to the risk of malnutrition. We recommend to evaluate inpatients' nutritional status periodically and to develop nursing intervention to solve their nutritional problems.

2.Differential Regulation of Antioxidant Enzymes during Monocyte Differentiation.

Antioxidant enzymes levels were determined in monocytes during phorbol myristate acetate (PMA)-induced differentiation. PMA induced the differentiation of a human monocytic leukemia cell line THP-1 into macrophage-like cells as indicated by activity of acid phosphatase and morphological changes. The level of Mn-superoxide dismutase (SOD) was selectively increased in PMA-treated THP-1 cells after one day of culture, while the levels of Cu/Zn-SOD and catalase were progressively decreased by Western blot analysis. In contrast, levels of Cu/Zn-SOD and catalase protein and enzyme activitiy remained unchanged in THP-1 cells after transforming growth factor-p, treatment. Cu/Zn-SOD is oxidatively inactivated by exposure to H,O, which is produced by PMA-treated THP-1 cells, and then the inactivated enzyme undergoes proteolysis and fragmentation as analyzed by radiolabeled method. Thus monocytes have a coordinated system for synthesis and degradation of antioxidant enzymes during PMA-induced differentiation.

5.In silico evaluation of the acute occlusion effect of coronary artery on cardiac electrophysiology and the body surface potential map.

Body surface potential map, an electric potential distribution on the body torso surface, enables us to infer the electrical activities of the heart. Therefore, observing electric potential projected to the torso surface can be highly useful for diagnosing heart diseases such as coronary occlusion. The BSPM for the heart of a patient show a higher level of sensitivity than 12-lead ECG. Relevant research has been mostly based on clinical statistics obtained from patients, and, therefore, a simulation for a variety of pathological phenomena of the heart is required. In this study, by using computer simulation, a body surface potential map was implemented according to various occlusion locations (distal, mid, proximal occlusion) in the left anterior descending coronary artery. Electrophysiological characteristics of the body surface during the ST segment period were observed and analyzed based on an ST isointegral map. We developed an integrated system that takes into account the cellular to organ levels, and performed simulation regarding the electrophysiological phenomena of the heart that occur during the first 5 minutes (stage 1) and 10 minutes (stage 2) after commencement of coronary occlusion. Subsequently, we calculated the bipolar angle and amplitude of the ST isointegral map, and observed the correlation between the relevant characteristics and the location of coronary occlusion. In the result, in the ventricle model during the stage 1, a wider area of ischemia led to counterclockwise rotation of the bipolar angle; and, during the stage 2, the amplitude increased when the ischemia area exceeded a certain size.

Cutaneous pseudolymphoma (CPL) has a microscopic appearance that resembles that of cutaneous lymphoma, but shows a clinically benign course. The differential diagnosis of CPL with cutaneous lymphoma is very important because clinical outcomes of them are quite different. We herein describe two cases of B-cell pseudolymphoma, which were difficult to differentiate from cutaneous B-cell lymphlma. All of two cases, Polymerase chain reaction of immunoglobulin heavy chain gene rearrangement showed polyclonal pattern.

7.Association between Type and Amount of Exercise with Bone Mineral Density in Adult Women.

BACKGROUND: Although lack of exercise is well known to be a risk factor of osteoporosis, it is unclear whether weight-bearing exercise has different effects on bone mineral density compared to non weight-bearing exercise, and the amount of exercise needed to prevent osteoporosis is not clearly defined. This study was performed to clarify the relations between the type and the amount of exercise with BMD. METHODS: This was a cross-sectional study of 3,661 women who had undertaken health screening including BMD measurement at Health Promotion Center of SNUH between May 1997 and March 1999. Information on sociodemographic characteristics, menarche, childbirth andexercise were gathered by means of self-reported questionnaire, and BMD was measured using DEXA. RESULTS: The number of women with lumbar osteoporosis were 601 (16.4%) and femoral neck osteoporosis 372 (10.2%). Lumbar osteoporosis occurred more frequently with increasing age and age at menarche, and in menopausal women, but less frequently with increasing fat-free mass in multiple logistic regression analysis. When adjusted for age, age at menarche, menopause and fat-free mass, lumbar osteoporosis was less common in women who exercised, but the type and the amount of exercise had no bearing. Especially, lumbar osteoporosis was least common in women engaging in moderate amount of exercise. Femoral neck osteoporosis decreased significantly only in weight-bearing exercise group and in women engaging in high amount of exercise. CONCLUSION: The results of this study suggest that lumbar osteoporosis can be prevented through exercise regardless of its type and amount, and high amount or weight- bearing exercise is most effective to prevent femoral neck osteoporosis.

8.Analysis of Visual Field Defect in Patient with Brain Lesion.

PURPOSE: In this study we analyzed the results of visual field tests in patients diagnosed with a brain lesion in an ophthalmology clinic. METHODS: The medical records of 55 patients who underwent visual field examinations and diagnosed with brain pathology using magnetic resonance imaging (MRI) from January 1, 2005 to June 30, 2014 were retrospectively investigated. RESULTS: Of the 55 patients (M:F = 30:25), 31 displayed symmetrical hemianopsia, a typical visual field defect associated with brain lesions. Twenty-four patients showed atypical visual field defect (20 showed normal visual field despite the brain lesion, 3 had bilateral narrowing of the peripheral visual field in 2 cerebral infarctions and 1 multiple sclerosis, and 1 had horizontal inferior visual field defect in 1 eye from nasopharyngeal cancer). CONCLUSIONS: Patients having a brain lesion with the optic tract invasion may present with atypical visual field defects. The peripheral visual field constriction and horizontal inferior visual field defect in 1 eye could be the visual field defect in patients with a brain lesion.

9.Two Cases of Segmantal Dilatation of the Intestine in Newborn Infants.

Segmental dilatation of small intestine or colon can induce signs of intestinal obstruction, such as abdominal distension, vomiting and constipation. There are no anatomical gross obstructive lesions, and moreover, ganglion cells are observed in both dilated and undilated distal segments of the intestine. It often accompanied by other congenital anomalies. We reported two cases of segmental dilatation of the intestine in the newborn infants, one in small intestine and the other in colon, with brief review of the related literatures.

10.Measuring the efficiency of dental hospitals with measurement of the service quality.

OBJECTIVES: Traditional nonparametric efficiency analysis was based on the relationship between the quantitative input and output measures. This study performed nonparametric efficiency analysis, which uses the results of dental hospital accreditation with quantitative output measures. METHODS: Data came from 35 hospitals between 2008 and 2010 that participated in the demonstration projects of dental hospital accreditation. The study employed two-stage double bootstrapping (TSDB) method to overcome the shortcomings and biased results of the traditional Data Envelope Analysis (DEA). Input measures included staffs, dental hygienists, other chair-side assistants, and administrative personnel, as well as dental equipment (dental chairs). Production measures were total revenues and the number of patients that visited the participating hospitals. We considered the environmental variables, which would affect the efficiency score of dental hospitals, dental hospital types, accreditation scores, and patients' satisfaction scores. RESULTS: There was a large split between the traditional DEA scores and TSDB scores. A total of 22 out of 35 hospitals had perfect efficiency in the traditional method, but none of those showed 100% efficiency in the TSDB. Overall efficiency score was the highest in the dental facility of general hospitals (71% mean efficiency), and dental teaching hospitals were the lowest (55% mean efficiency score). The factors that affected the efficiency of dental hospitals were the types of the dental hospital and the accreditation score. CONCLUSIONS: Traditional DEA scores had biased estimates and tended to be higher. The study found that organizational efforts for improving service qualities and patient safety would affect the efficiency, as well as the accreditation results.
