Gastric glomus tumor is an uncommon benign, submucosal neoplasm and does not require radical surgical procedure. Because there are no specific clinical or radiologic features associated with the glomus tumor, it can be recognized only by its histologic characteristics. We report a 30-year-old woman who had 10 years history of epigastric hunger pain. Radiologically, a gastric submucosal tumor was discovered, which was suggestive of leiomyoma. Gastric antrectomy was performed. The tumor cells showed immunohistochemical and ultrastructural evidence of smooth muscle differentiation.

In comparison to parenchymal hamartoma, endobronchial hamartoma is rare benign neoplasm of the lung. Most parenchymal hamartomas are asymptomatic and are found incidentally. However, endobronchial hamartomas are frequently discovered through respiratory symptoms as a result of bronchial irritation or obstruction. A 47-year-old male patient was admitted to our hospital due to dry cough 1 month prior to admission. On bronchoscopic examination, a polypoid mass was found completely obliterating the anterior segmental bronchus of the left upper lobe. We report a case of endobronchial chondroid hamartoma, which was resected by anterior segmentectomy of the left upper lobe.

BACKGROUND/AIMS: Transforming growth factor-a(TGF-a) is a polypeptide cytokine related to cell proliferation and transformation. TGF-a binds to EGF receptor and stimulating DNA synthesis in liver cell. The hepatitis B virus (HBV) by itself is also believed to play a role in the hepatic carcinogenesis. Recently, it was reported that TGF-a and HBV were synergistic in action with rapid appearance of hepatocelluar carcinoma in bitransgenic mice. Although TGF- a is thought to play an important role in hepatocarcinogenesis, its expression during the natural history of HBV hepatitis was poorly understood. This investigation was performed to elucidate the dynamic changes and istinct immunohistochemical staining patterns in the course of chronic HBV hepatitis with specific reference to hepatocelluar carcinoma and to explain the role of TGF-a in the pathogenesis of hepatocelluar carcinoma. MATERIALS/METHODS: Employing TGF-a monoclonal antibody, signal detection was carried out by peroxidase-conjugated streptavidin in deparaffinized liver tissue sections taken from HBsAg positive patients. All of the liver tissue sections were proven HBV DNA positive by in situ hybridization. Immunohistochemical staining was performed in the tissue sections obtained from four normal controls, six from patients with chronic persistent hepatitis, five with chronic active hepatitis, eight with liver cirrhosis and eleven with hepatocellular carcinoma. RESULTS: The patterns of TGF-a immunoreactivity were cytoplasmic-grain types in normal controls and chronic persistent hepatitis, honeycomb types in chronic active hepatitis, occasional cytoplasmic-flooding types in liver cirrhosis, and cytoplasmic-grape types in hepatocellular carcinoma. A Shapiro-Wilk W test for frequency table analysis for the expression of TGF-a in these different disease groups was statistically significant. CONCLUSION: These data suggest that step-wise distinct expression of TGF-a enhancement in HBV associated chranic liver diseases which eventually resulted in the development of hepatocellular carcinoma were conceivably due to dysregulation of liver cell cycles by both HBV and TGF-a during the persistent repetition of cell cycles.

Esophageal schwannoma is very rare and almost of all cases are diagnosed as esophageal submucosal tumor preoperatively. Final diagnosis is made by postoperative immunohistochemical (IHC) staining of the surgical specimen. We experienced two cases of esophageal submucosal tumor, one was 63 year old female suffering from three months of dysphagia and another was 39 year old female complaining of two months of intermittent dysphagia. Two esophageal tumors were completely removed by esophagectomy and enucleation through right thoracotomy respectively. Postoperative IHC staining demonstrated S-100 positive without mitotic figures and confirmative diagnosed was made as benign esopphageal schwannoma.

Esophageal hemangioma is an extremely rare benign tumor that causes dysphagia and massive upper gastrointestinal bleeding. Although certain abnormalities seen on a barium swallow esophagography or at endoscopy may suggest an esophageal hemangioma, a contrast CT and radionuclide angiography using a blood-pool radiopharmaceutical can characterize the intense vascularity of the tumor. We experienced the ase of a 7x7x3.5 cm in size giant cavernous hemangioma of the lower 1/3 of esophagus in a 40 year old man. A mural cavernous hemangioma was diagnosed with a barium swallowed esophagogaphy, endoscopy, and a contrast CT. It was treated successfully by transthoracic esophagectomy including the tumor and esophagogastrostomy.

To evaluate the antiproteinuric effect and antihy-pertensive effect of angiotensin converting enzyme inhibitor according to the angiotensin converting enzyme(ACE) and endothelial nitric oxide synthase (ecNOS) genotypes, 72 proteinuric patients and 121 control subjects were examined. Patients who were on oral steroids or who had taken antihypertensive or antiproteinuric drug within four weeks before the base-line period were excluded from this study. Systolic blood pressure, diastolic blood pressure, 24 hour urine protein, and 24 hour urine creatinine level were measured before drug administration. The patients were given a four-week supply of enalapril 10mg if no cough were occurred. Systolic blood pressure, diastolic blood pressure, 24 hour urine protein, and 24 hour urine creatinine level were measured repeatedly after drug administration. In genotype distribution of the ACE gene poly- morphism, there was no significant difference between controls and proteinuric patients. The antiproteinuric effect was dominent in DD genotype. But, there were no significant association between antiproteinuric effect and ecNOS polymorphism in prote-inuric patients. We also failed to detect any signi- ficant association between the ACE and ecNOS ge-ne-polymorphisms and blood pressure lowering effect of angiotensin converting enzyme inhibitor. Our result suggested that antiproteinuric effect of enalapril in proteinuric patients was more prominent in DD genotype. But, a further study with larger number of patient population would be necessary.

Diffuse esophageal spasm (DES) is a rare disease seen in 4% of all patients studied in an esophageal motility laboratory, and its diagnosis and surgical management is still controversial. Recently, we treated two patients by extended esophageal myotomy for diffuse esophageal spasm which was diagnosed by the clinical symptoms of patients, esophagoscopy, esophagography, and esophageal manometry. The successful result of treatments was proved with subsidence of previous clinical symptoms (dysphagia and chest pain), postoperative esophagography and esophageal manometry. We present the results together with the review of literatures.
