1.A Case of Acne Conglobata.

A case of 30 years old male with acne conglobata was reported. The skin lesions were characterized by numerous papules, pustules, abscesses, discharging sinuses and resultant hypertrophic scars on the buttocks, back, face, chest, abdomen, neck, and the both extremities. Physical examination shows chronic tonsillitis, caries, and missing of the teeth, and periodontitis. Pus culture shows only coagulase negative Staphylococcus. He was treated with tetracycline, predlnisolone and vitamin B-complex for four weeks with favorable effect.

4.Correlation between Chromosome Abnormalities and Genomic Imprinting in Developing Human 2) Independent Expression of Imprinted Genes, H19 and Insulin-like growth factor II (IGF2), in Androgenetic Hydatidiform Moles.

Human uniparental gestations such as androgenetic hydatidiform moles provide a model to evaluate the integrity of parent-specific gene expression,-i.e, genomic imprinting,- in the absence of a complementary parental genetic contribution. Several imprinted genes are characterized so far including the insulin-like growth factor-2 gene (IGF2) coding for a fetal growth factor and the Hl9 gene whose normal function is unknown but which is likely to act as an untranslated mRNA for its tumor-suppressing function. IGF2 is expressed exclusively from the paternal allele while Hl9 from the maternal allele. Such an alternate expression is quite interesting because both Hl9 and IGF2 genes are located close to each other on chromosome 11p15.5. An in situ hybridization analysis has shown strong expression of Hl9 and IGF2 alleles in nine hydatidiform moles. Especially, a prominent expression of Hl9 and IGF2 was detected in cytotrophoblast and the cellular localization was almost paralleled in Hl9 and IGF2 transcripts . Hl9 and IGF2 genes could be expressed either biallelically or monoallelically in the moles. However, IGF2 biallelic expression did not affect allele-specificity of Hl9 expression.. These results suggest that both H19 and IGF2 transcripts are expressed in the same cells and that the functional imprinting of H19 and IGF2 genes in hydatidiform moles can be controlled individually and independently of each other.

5.A Case of Surgically Corrected-Combined form of Total Anomalous Pulmonary Venous Return.

Total anomalous pulmonary venous return(connection) is a congenital heart disease. Especially, the reports of the patients with double connections of all pulmonary veins were rare. An infant who underwent operation for total anomalous pulmonary venous return had double drainge of all pulmonary veins to both left vertical vein and anomalous descending vein. This second lesion was not diagnosed preoperatively, but in operative field. Operative treatments were performed by ligation of left vertical vein and central pulmonary vein-to-left atrium anastomosis without ligation of the descending anomalous vein. For these less common variants fo total anomalous pulmonary venous return,a more detailed and precise understanding of morphology can facilitate accurate surgical repair. And also, early detection of the disease is an another issue for a good prognosis.

7.Hepatic Resection for Large Hepatic Metastasis of Adrenocortical Carcinoma after Cyberknife Treatment.

Surgical resection is the best treatment for operable metastatic liver cancer. Large metastatic cancer usually has low operability, so a debulking modality of mass is needed to increase operability. Chemotherapy and radiotherapy were commonly used as neoadjuvant treatments. Cyberknife stereotactic radiosurgery systems were only considered as one of the palliative treatment modalities for inoperable or surgically complex tumors. But, in this case, we applied Cyberknife as the preoperative debulking modality for large hepatic metastasis of adrenocortical carcinoma. After Cyberknife treatment 3 cyclex2,700 cGY, tumor size decreased (metastatic liver mass decreased to 15x9 cm from 19x12.5 cm, adrenal mass decreased to 5x3 cm from 7.4x4.5 cm). We could then resect the tumor completely by extended right hemihepatectomy & right adrenalectomy. A preoperative multidisciplinary approach, including chemotherapy and radiation therapy can be considered to increase operability. So, cyberknife can be considered an additional modality as a neoadjuvant radiotherapy.

8.A Case of Keratectasia 5 Years after Photorefractive Keratectomy.

PURPOSE: The clinically evident keratectasia has not been reported after photorefractive keratectomy (PRK) on the eyes with normal preoperative corneal topography. Herein, we present a case of keratectasia developed 5 years after PRK. METHODS: A 20-year-old man underwent the PRK for correction of myopic astigmatism in 1993. Preoperative manifest refractions were -4.00 D-0.50 D x 90degrees in the right eye and -3.75 D-0.75 D x 85degrees in the left eye. Although pachymetry was not performed, preoperative examinations including corneal topography did not show any sign of early keratoconus or forme fruste keratoconus. The operations were uneventfully performed on two eyes at an interval of one month. RESULTS: Up to 9 months after PRK, the patient's uncorrected visual acuity had been maintained 1.2 in both eyes. When the patient returned 5 years after PRK, he complained of decreasing visual acuity (0.5)in his right eye. During the next 2 years, the vision of the right eye was deteriorated to 0.04, and could not be corrected with the spectacles. The corneal topography of the right eye revealed profound steepeningjust inferior to the corneal center, and he had become contact lens intolerant. The left eye remained stable. The central corneal thickness was 419.0 micrometerin the right eye and 476.3 micrometerin the left eye.

9.A Case of Submucosal Renal Pelvic Calcification and Ossification.

We report a case of renal pelvic calcification and ossification simulating a renal pelvic stone. histopathological examination of resected kidney revealed submucosal calcification in the pelvic and ossified foci in the perivascular area. This is the first report in the Korean literature and have been reported 1 case in the English literature till now. This is very rare entity unrelated to amyloidosis.

10.Arterial blood gas analysis in asthmatic children.

The purpose of this study is to verify severity of asthma in asthmatic patients through through the arterial blood gas analysis. Subjects were consisted of 103 patients (74 boys and 29 girls), between 2~13 years of ages. Clinically, asthmatic patients were classified into 6 groups, i, e., group 0(no rhonchi), group 1(rhonchi only), group 2(mild attack), group 3(moderate attack), group 4(severe attack), group 5(respiratory failure with disturbance of consciousness). PH kept normal range in the group 0, group 1, group 1 and group 3,but began decrease in the group 4.There was linear fall in Po2 and began decrease in the group 3. HCO3- maintained normal level through the whole range. BE showed acidemia on the whole, and began distinctive decrease in the group 4 and group 5, especially. Hypoxemia, hypercapnia and acidemia were common in patients in severe attacks with disturbance of consciousness.
