1.Case Report of Penile Plastic Pedicle Skin Graft.

The number of skin graft operations following the soft tissue injuries has been increasing year after .year, as the number of industrial business is growing fast in Seoul district. We have performed one hundred and two cases of skin graft, mainly for the extremities injury in the orthopedic surgical department alone during last sixteen months (from January. l968 to Apri1, 1969) including two plastic operation for the penile skin which involves, very rarely because of an anatomical .stand point The plastic repair of penile skin was done by means of Goodwin's technic. The pedicle skin wag donated from the scrotum for the completely necrotized penile skin which was developed following the attempt of removal of the penile paraffinoma.

2.Choledochal cyst with ectopic distal location of the papilla of Vater.

In cholangiographic techniques, the close relationship between choledochal cyst and anomalous union of pancreaticobiliary duct has attracted medical attention. There have been rare cases in which the papilla of Vater was found in a position other than its normal position, and such cases have been reported sporadically. However, such cases are interesting in the anatomical context. In this review, we present our experience of choledochal cyst in a 30-month-old boy in whom the papilla of Vater was positioned in the third portion of the duodenum.

6.Well-Differentiated Papillary Mesothelioma of the Peritoneum: A case report .

Well-differentiated papillary mesothelioma (WDPM) of the peritoneum is considered to be a distinct subtype of peritoneal mesothelioma and has mostly behaved in a benign fashion. We report a case of WDPM in a 48-year-old-woman. It was incidentally found during a hysterectomy for a uterine cervical carcinoma. Grossly, the tumor was composed of multiple peritoneal nodules, measuring up to 2 cm. Microscopically, the nodules showed well-developed papillae lined by a single layer of cuboidal mesothelial cells. Immunohistochemical staining revealed a positive reaction for cytokeratin and a negative reacion for carcinoembryonic antigen and Leu-M1. Ultrastructurally, the tumor cells showed numerous long, slender microvilli and desmosomes.

7.Nevus Flammeus Associated with Angiokeratoma of Fordyce and Lymphangioma Circumscriptum.

We present a case of nevus flammeus associated with angiokeratoma of Fordyce and lymphangioma circumscriptum in a 26 year-old Korean man. We have known that the congenital hemangiomas may be associated with many syndromes such as Kasabacb-Merritt syndrome, Sturge-Weber syndrome and etc. Recently the congenital hemangiomas associated with angiokeratoma or lymphangioma were reported in several literatures, Watson and McCarthy reported that it seemed to be established that three types of benign vascular tumors had a common origin and grew in an identical fashion by projecting buds of endothelial tissue. So the congenital hemangiomas may have a close association with angiokeratoma or lymphangioma.

8.Determinants of Heart Rate Variability in General Korean Population.

BACKGROUND AND OBJECTIVES: Heart rate variability has been known to be a prognostic factor of heart disease. However, determinants of heart rate variability in general korean population without clinical heart disease have not been studied. Objectives of this study were to measure heart rate variability in general population and to investigate clinical determinants of heart rate variability. METHODS:Heart rate variability measures were obtained by LRR-03TM and MemCalcTM software (GMS, Tokyo, Japan) from public officials in a district of Seoul and their families(n=69). Predictors of heart rate variability included age, gender, heart rate, smoking status, systolic blood pressure, diastolic blood pressure, serum total cholesterol, HDL-cholesterol. Univariate analysis and analysis of variance of low frequency power(0.04-0.15 Hz), high frequency power(0.15-0.30 Hz), and total power spectrum in relation to explanatory variables were done. In order to select determinants of heart rate variability, multiple linear regression model of each heart rate variability measure was created and stepwise selection method was applied. RESULTS: Analysis of variance showed that older age, higher heart rate, body mass index > or =27, systolic blood pressure > or =140 mmHg, diastolic blood pressure > or =90 mmHg, and serum total cholesterol > or =240 mg/dl were negatively associated with one or more heart rate variability measures. Serum HDL-cholesterol > or =35 mg/dl was positively associated with low and high frequency power. Multiple linear regression analyses showed that age and heart rate were the major determinants, gender and cardiovascular risk factors such as diastolic blood pressure, HDL-cholesterol, and smoking contributed to one or more heart rate variability measures. CONCLUSIONS: Age, heart rate, gender, and cardiovascular risk factors must be considered when evaluating heart rate variability.

9.Nevus Lipomatosus Cutaneus Superficialis: Report of a Case.

Nevus lipomatosus cutaneous superficialis(NLCB) is a rare condition consisting of ectopic fat tissue in the dermis and having the predilection for the pelvic girdle, especially gluteal region. The patient was a 20 year-old female. About 8 years prior to the first visit she had noticed the rice-sized papules on her lower back, which had been gradually increased in size and coalesced to become two, cerebriform, skin colored to yellowish tumor masses of soft consistency, distributed on both sides of the midline of lower back, and measuring respectively 3.5X7.Qcm and 7.0x11.5cm in size. Jihe biopsy specimen taken from the tumor mass revealed the ectopic presence of fat lobules occupying half of the reticular dermis. The fat lobules consisted of almost mature fat cells with the presence of dilated capillaries in their centers.

10.Ramsay Hunt Syndrome.

A case of Ramsay Hunt syndrome, though more often seen at the ENT clinic because of the patients' concern with symptoms in the ear, was seen and treated at the Dermatologic Clinic of Chosun University. A 29 year old male patient presented earache, pinhead to rice sized erythematous vesicular eruption on the external auditory canal, auricle and superior auricular area, and facial paralysis, all involving the right side of the face, duration of which were 7 days, 5 days and 1 day respectiveIy. High doses of prednisolone alone made it possible to remit the otalgia, control the vesicular eruption and resolute the facial paralysis almost completely within 18 days in the order of their appearance, indicating that this is one of the more easily treated diseases for dermatologists, The 22 cases of this syndrome reported in Korean literature were reviewed and the conceptual development of its pathogenesis and definition was described.
