1.A Case of Adenomyotic Cyst Within Myometrium Accompanied with Endometriosis.

Adenomyotic cyst is very rare disease, their sizes are mostly lesser 5mm. The intrauterine adenomyotic cyst may arise from progressive expansion of cyst due to progressive menstrual bleeding. Authors experienced a case of large adenomyotic cyst within myometrium occuring in a l9-year-old woman, and who was accompanied with endometriosis. The cyst was about 3 x 3em sized, and had chocolate colored thick viscous contents, We experienced one case of adenomyotic cyst which was thought to be degenerated uterine myoma, so we report the case with a brief review of the concerned literatures.

2.Comparison of Pathologic Findings by Seawater or Fresh Water Drowning on the Experimental Animals.

Death by drowning is a major cause of unnatural death worldwide. It is therefore important to conduct forensic examination of immersed bodies following drowning, in order to determine the diagnosis of drowning, because no specific methods have been established thus far. Therefore, we performed a series of rat experiments to compare autopsy findings between seawater and fresh water drowning cases, which included the presence of pleural effusion and histologic findings of the lung. The results showed that the volume of pleural effusion increased in the seawater drowning group compared to the fresh water drowning group, and the total weight of lung was affected by the type of drowning medium and postmortem interval. However, histologic findings of the lung showed no significant difference between the 2 types of drowning mediums.

3.A Changes of Electrolytes and Biochemical Components during Hemodialysis.

To investigate the changes of electrolytes and bio-chemical components during hemodialysis in 15 patients(7 men and 8 women) with end-stage renal disease undergoing long-term hemodialysis were studied. The mean age of 15 patients undergoing long-term hemodialysis was 45 year(range ; 26-62). The mean duration of hemodialysis was 58 months(range: 19-143) at time of the study. Hemodialysis was performed with acetate dialy- sate having 37mEq/1 acetate and 8.57g/100ml glu-cose(group A), with bicarbonate dialysate having 8 mEq/1 acetate and 30mEq/1 bicarbonate without glu-cose(group B) for 4-hour. Each blood sample was drawn for blood gas analysis, blood glucose, blood urea nitrogen and electrolytes from the arterial line at 0, 1, 2, 3, and 4-four during hemodialysis. In group A, the body temperature measured at 1 hour increased significantly compared with that of predialysis. The blood glucose level increased pro-gressively during dialysis. In group B, the body temperature measured at 1 hour also increased sig- nificantly compared with that of predialysis. However, the blood glucose level increased progressively during dialysis as removing urea in blood during dialysis. These results suggested that blood urea nitrogen may be a cause of hypothermia. Mean corpuscular volume(MCV) increased significantly at 2-hour in group A and returned to baseline values at 4-hour. In group B, the mean level of glucose value in dialysate of outlet line of dialyzer increased to 38.3mg/dl from zero in inlet line, and PaC&z decreased significantly because of diffusion of COz across the dialyzer. In Group A, PaOz decreased significantly at 1-hour and returned to baseline values at 4-hour, whereas HCC4 was increased significantly at 4-hour. The intracellular potassium content may decreased in long-term hemodialysis patients cornpared with normal control. In Group A, hernoglobin diphosphoglycerate(2, 3-DPG) concentraion increased from 10.9 pmoVg before dialysis to 19.9pmol/g after dialysis. Despite substantial anemia, hemoglobin 2, 3-DPG prior to dialysis was significantly lower than the value obtained in the normal control(17.5+4.3 p moVg). In group A, the blood lactate level decreased significantly at 1-hour and returned to baseline values at 4-hour. Our data suggested that body temperature might be increased due to removal of urea nitrogen, and blood glucose may be decreased because of the dif- fusion across dialyzer. In addition, this study showed that intracellular potassium content was lowered, the MCV of RBC was increased during hemodialysis. Transient decrement of PaCOz during early phase of dialysis and decreased hemoglobin 2, 3-DPG of despite anemia before dialysis improved with the increment of blood pH and HCC4 at the end of dialysis.

6.Metallic Failures after Compression Plate Fixation Clinical Analysis of 12 Cases

The twelve cases of metallic failure after compression plate fixation which were observed and treated at Department of Orthopedic Surgery, Busan National University Hospital were clinically analysed. The results were summerized as follows; 1. Age and sex of the patients was male in his active age in life. 2. The causes of fracture were traffic accidents and industrial accidents. The sites of fracture were femur in 11 cases and tibia 1 case. 3. The types of fracture were comminuted in 11 cases and short oblique in 1 case. 4. The 6 patients were not cooperative and began walking at their will after postoperative 4 weeks despite doctors order of nonweight bearing. Average duration of immobilization was 6 weeks. 5. Metallic failure occurred as screw failure in 1 case, plate failures through empty hole in 4 cases and plate failures through hole filled with screw nearest to the sites of fracture in 7 cases. 6. Interval between operation and metallic failure was 6.25 months average. The 75% of the metallic failure occurred between 4 to 7 months. 7. Treatments of metallic failures were cast immobilization with bone grafting in 1 case at tibia, intramedullary nailing with bone grafting in 1 case at femur and compression plate fixation with bone grafting in 10 cases at femur. 8. Causes of failure after internal fixation 1). Insufficient bone support Inadequate inter-fragmentary compression 9 cases Absence of cancellous bone graft 8 cases Empty plate hole 4 cases Remaining defects 3 cases 2). Inadequate postoperative treatment 6 cases.

7.Clinical Evaluation of Degenerative Spondylolisthesis

The instability of the degenerative spondylolisthetic lumbar spine was clinically evaluated by patients symptoms and serial radiograms. With X-ray films of patients lumbar spine, the lumbosacral angle, the sacral angle, the vertebral height difference between anterior and posterior surface of the 5th lumbar vertebra and the level of intercristal line were analysed. And the results of treatment were analysed too. The Results were as follows: 1) In degenerative spondylolisthetic L3 and L4 spines, facets were arranged in sagittal plane and sacrum was vertically arranged, but in L5 facets were coronally arranged and sacrum and in horizontal plane. 2) The lumbosacral angle was 149.9 and the average difference between anterior and posterior height of L5 vertebral body was 3mm. These results revealed that sacrum was more vertical and the 5th vertebral body was more rectangular, so lumbar lordosis was decreased as a whole. 3) The intercristal line was passed at IA or L4–5 interspace in 72.1% of cases(normal:95.8%), so L4 vertebra was placed in higher level. 4) In group A posterolateral fusion was done in 2 cases. Among group B and C, operation was done in 9 cases;4 cases with laminectomy and 5 cases with laminectomy and posterolateral fusion. Two years after surgery, slipping and instability were far advanced in 2 laminectomized cases for whom posterolateral fusion was followed later. 5) Unstable group of degenerative spondylolisthesis with severe neurological symptoms and claudication must be treated by decompressive laminectomy and posterolateral fusion, especially in patients under 60 years of age. 6) Young woman having flattened lumbar curve without slipping, narrowed disc space of L4–5, high-seated L4 and vertical sacrum is thought to be in prespondylolisthetic stage of degenerative origin; therefore, intensive back muscle exercise to prevent the slipping should be performed and periodic X-ray examinations should be followed up to detect the progress of the disease.

8.A Clinical Study of the Severe Ankle Fracture

The primary goal of treatment of the adult ankle fracture is to effect the returning of normal func tion that is accomplished by the accurete reduction and rigid internal fixation followed by the early institution of motion. The authors have reviewed a series of 41 cases of the bimalleolar and trimalleolar ankle fractures that treated in Orthopedic Department of Eulji General Hospital, Seoul, from January 1982 to June 1986. With the duration of follow-up of average 13.2 months, we summarized the obtaind results as followings. l. Among the forty one patients, thirty one was male patient(M: F =3.1: 1). The average age was 33.2 years old. 2. The main cause of the fracture was traffic accident, 18 cases(43.9%) and other causes were slipping down(24.4%), fall from a height(14,6%), industrial injury(12.2%) and other injuries(4.9%). 3. According to the modified Lauge-Hansens classification, the supination-external type was most common (41.5%). 4. As a diagnostic method, the computerized tomography was also available. 5. All cases were treated by open reduction and internal fixation with the good radiologic results of 91.3% in bimalleolar fractures and 88.9% in trimalleolar fractures. 6. Of the bimalleolar fractures, ten patientz showed normal range of motion in 2 to 4 weeks, average 18.4 days, by early joint exercise.

9.A Clinical Study of the Subacute Osteomylitis in Children

Subacute osteomylitis is far less common than acute osteomyelitis, characterized by insidious onset of the symptoms of mild local pain or discomfort without any acute systemic illness. Radiographic findings are not infrequently confused with benign or malignant bone tumors. From Jan, 1983 to Dec. 1991, we experienced twelve patients with subacute osteomylitis of long bones. Our clinical observations were as follows. 1. There were 11 boys and one girl with an average age of 9.7 years(range, 30months-16years). 2. The involved sites were proximal in 1, distal radius in 3, midshaft of femur in 3, distal femur in 2, proximal tibia in 1, and distal tibia in 2 cases. 3. All patients had insidious onset of mild to moderate pain. ESR was increased in 9 cases(75%) with a mean value of 44 mm/hr, but leukocytosis was not found. 4. According to the Green and Edwards' classification, there were type 1 in 1, type 2 in 3, type 3 in 3, and type 6 in 5 cases. 5. Eleven patients had operative treatment. The remaining one patient was treated by antibiotic treatment. 6. Primary treatment was successful in 11 patients who were followed for and average 9.5 months. One patients was lost to follow-up.

10.Treatment of Carpal Scaphoid Fracture.

Fracture of the carpal scaphoid bone is the most common fracture of the carpus. Unfortunately, nonunion are common since the symptoms do not alert patents to seek early medical treatment and the diagnosis is easily missed. Fracture of the scaphoid is a common condition whose management remains controversial. The conservative treatment has many complications, these include inability to word while in plaster, stiffness of the wrist afterwards, muscle wasting, weakness and malunion. So various operative treatments of scaphoid fracture have been developed. Open reduction by use of screws suggested by Maclaughlin first in 1954. In 1984 a new and simple operative technique has been developed to provide rigid internal fixation for all types of fractures of the scaphoid by T.J. Herbert. This involves the use of a double-treaded bone screw which provides good fixation that, after operation, a plaster cast in rarely required and most patients are able to return to work within a few weeks. Authors have experienced 10 cases of scaphoid fracture and accomplished good result in all cases by internal fixation using Herbert screw. The results are as follows Of these 10 fractures, 2 were fresh fractures and 8 were nonunions of scaphoid fracture. In nonunion cases, the time interval between fracture and operation was from 5 months to 5 years. The postoperative immobilization period was average of 4 weeks. Short period of immobilization achieved early functional recovery of the wrist. The bony unions in roentgenogram were seen from 3 months to 9 months after operation. In 2 cases the fracture gap was seen after 9 months. But in these cases the symptoms such as pain and range of motion of wrist were improved.
